Zusammenfassung
Hintergrund: Die dekompressive Kraniektomie ist eine etablierte und verbreitete Methode mittels Teilentfernung des knöchernen Schädels, konservativ nicht zu beherrschenden Hirndruck zu behandeln. Dieser Maßnahme folgt regelhaft die Kranioplastie zur Defektdeckung. Die Kranioplastie ist mit einer für neurochirurgische Operationen hohen Komplikationsrate verbunden. Die postoperative Infektion und die Autolyse des Kalottenimplantats haben an den Komplikationen den größten Anteil.
Ziel der vorliegenden Untersuchung war es, anhand von klinischen Parametern Prädiktoren zu ermitteln, welche die Wahrscheinlichkeit des Auftretens der Komplikationen, postoperative Infektion und Autolyse erhöhen. Ein berücksichtigen dieser Prädiktoren könnte im klinischen Alltag zur Senkung der Komplikationsrate führen.
Methode: In einer monozentrischen retrospektiven Untersuchung wurden in der Klinik für Neurochirurgie, BG Klinikum Bergmannstrost Halle die Daten der zwischen 2004 und 2016 bei pathologischem Hirndruck mit dekompressiver Kraniektomie und im weiteren Verlauf mit Kranioplastie operierten Patienten ermittelt. Dies ergab 200 auswertbare Fälle. Die epidemiologischen, peri- sowie postoperativen Daten wurden kategorisiert und Anhand von Hypothesenprüfung statistisch in Bezug zum Auftreten von postoperativer Infektion und Autolyse des reimplantierten Kalottenstückes untersucht.
Ergebnisse: Einige der untersuchten Parameter wiesen eine knappe Nichtsignifikanz auf und können, unter Berücksichtigung ähnlicher Ergebnisse in der Literatur als Tendenz gewertet werden. So konnte eine Tendenz bezüglich des Auftretens einer postoperativen Autolyse des Kalottenimplantats und der OP-Dauer (p=0.057) sowie der jüngeren Altersgruppen (p=0.069) festgestellt werden. Der Zustand des Kalottenimplantates (fragmentiert/intakt) scheint einen Einfluss auf das Auftreten von postoperativen Infektionen zu haben (p=0.077).
Insgesamt konnten aber keine klaren signifikanten Zusammenhänge zwischen den epidemiologischen und peri- sowie postoperativen Parametern und dem Auftreten der Komplikationen postoperative Infektion oder Autolyse nachgewiesen werden.
Schlussfolgerung: Die vorliegende Arbeit spiegelt die Ergebnisse in der aktuellen Literatur wieder. Dieser sind keine eindeutig klassifizierten Ursachen für das Auftreten dieser Komplikationen zu entnehmen. Es ist zu empfehlen, dass zukünftig prospektive, randomisierte multizentrische Studien sich des Themas aufgrund seiner Relevanz weiter annehmen um das Risiko der nicht zu vermeidenden Kranioplastie zu minimieren.

Abstract
Background: Decompressive craniectomy has become an established and common method for treatment of uncontrollable elevated intracranial pressure. This procedure is regularly followed by cranioplasty for cranial reconstruction. For a neurosurgical procedure, cranioplasty is associated with an extraordinary high rate of complications. Infection and necrosis of the reimplanted skull flap have the greatest share among complications. Goal of this study was to identify possible predictors among clinical parameters causing the complications infection and necrosis of the bone flap following cranioplasty. Considering these predictors might help to reduce complications.
Method: In this retrospective monocentric study patients who underwent decompressive craniectomy and cranioplasty between 2004 and 2016 in the neurosurgical department at “BG Klinikum Bergmannstrost Halle“ were identified. Of these 200 resulting cases epidemiological, peri- and postsurgical parameters were statistically analysed relating to infection and necrosis of the bone flap.
Results: Among the epidemiological, peri- and postoperative parameters no significant relationship concerning postoperative infection or necrosis of the bone flap could be found. Some parameters though showed a close non-significance and can be interpreted as tendency considering similar results in literature. Predictors with a trend concerning necrosis of the bone flap are longer duration of surgery for cranioplasty (p=0.056) and younger age group (p=0.069). Concerning infection of the bone flap the condition of the implant (fragmented/intact) seems to be a potential predictor (p=0.077).
Conclusion: This present study reflects the situation in current literature. It has to be recommended, that future prospective multicentre randomised studies should further investigate complications following cranioplasty to identify and minimize risk factors of this unavoidable procedure.
