Die Aktivierung der Gerinnungskaskade während myokardialer Ischämie und Reperfusion trägt, vor allem über die Generation von Thrombin, zur postischämischen inflammatorischen Reaktion bei. Wir beurteilten den Effekt der Antikoagulanzien unfraktioniertes Heparin (UFH), niedermolekulares Heparin (LMWH) und r-Hirudin (direkter Thrombininhibitor) auf leukocytäre Adhäsion und Emigration nach Ischämie und Reperfusion in einem Rattenmodell. Der Cremaster- Muskel männlicher Ratten wurde für intravitalmikroskopische Untersuchungen vorbereitet. Nach einer Ischämiedauer von 120 Minuten folgten 90 Minuten Reperfusion. Physiologische Kochsalzlösung (Kontrolle), UFH, LMWH oder r-Hirudin wurden 15 Minuten vor Beginn der Reperfusion verabreicht und bis zum Ende der Beobachtungszeit infundiert. Die Dosierungen waren wie folgt (Bolus, Infusion): Kochsalz 3ml, 3ml/h; UFH 400 IE, 100 IE/h; LMWH 100 IE, 3ml/h Kochsalz; r-Hirudin 0,3mg, 0,15mg/h. In postkapillären Venolen wurden anhand der intravitalmikroskopischen Aufnahmen die rollenden, adhärenten und extravasierten Leukocyten gezählt. Alle drei Antikoagulanzien verringerten gleichermaßen die postischämische Adhäsion von Leukocyten an das Endothel. Im Gegensatz dazu inhibierte nur r-Hirudin die Leukocytenemigration. Die Emigrationseffizienz adhärierter Leukocyten (Kontrolle 1,21) war unverändert nach UFH (1,74) und LMWH (1,51), aber signifikant vermindert in der mit r-Hirudin behandelten Gruppe (0,12). Die unterschiedliche Effizienz der drei Antikoagulanzien in Bezug auf die Emigration von adhärenten Leukocyten legt eine spezifische Rolle der direkten Thrombininhibition bei der Verringerung der postischämischen inflammatorischen Reaktion nahe. Dieser Effekt könnte zu dem beobachteten überlegenden Effekt der direkten Thrombininhibitoren in klinischen Studien zur Behandlung von Akuten Koronarsyndromen beitragen.
Activation of the coagulation cascade during myocardial ischemia and reperfusion may contribute to the postischemic inflammatory response, mostly via generation of thrombin. We assessed the effect of the anticoagulants unfractionated heparin (UFH), low molecular weight heparin (LMWH) and r-hirudin on leukocyte adhesion and emigration after ischemia and reperfusion in rats. The rat cremaster muscle was prepared for intravital microscopy. One hundred and twenty minutes of ischemia were followed by 90 min of reperfusion. Saline (control), UFH, LMWH or r-hirudin were given 15 min prior to reperfusion and infused for the rest of the observation period. Dosages per kilogram of body weight were (bolus, infusion): saline, 3 ml, 3 ml/h; UFH, 400 IU, 100 IU/h; LMWH, 100 IU, 3 ml/h saline; or r-hirudin, 0.3 mg, 0.15mg/h. In collecting venules, rolling, adherent, and extravasated leukocytes were counted from recordings of the intravital microscopy. All three anticoagulants similarly attenuated post-ischemic endothelial leukocyte adhesion. In contrast, emigration of leukocytes was only attenuated by r-hirudin. The emigration efficiency of adherent leukocytes (control, 1.21) was unchanged after UFH (1.74) and LMWH (1.51) but decreased after r-hirudin treatment (0.12). The different efficacy of the three anticoagulants in affecting emigration of adherent leukocytes suggests a specific role for the direct thrombin inhibitor r-hirudin in attenuating the postischemic inflammatory response. This effect may contribute to the benefits of direct thrombin inhibitors seen in clinical studies after treatment for acute coronary syndromes.
