Es wurde das klinische Outcome von 126 Patienten, die in einem Zeitraum von
Juli 1993 bis Mai 2001 nach der Operation in Caspari-Technik bei
Schulterluxation an der Schulter versorgt wurden, betrachtet. Einbezogen
wurden ausschließlich vordere Schulterluxationen, die auf ein adäquates Trauma
zurückzuführen waren. Das Patientenkollektiv (n=126) teilt sich in 37 Frauen
und 89 Männer auf. Darunter sind 33 Patienten, die bereits nach der ersten
traumatischen Schulterluxation der Operation zugeführt (Erstluxierer) und 93
Patienten, die erst nach rezidivierenden Schulterausrenkungen operiert wurden.
Es liegt ein durchschnittliches follow-up von 6,4 Jahren vor. Anhand von
Reluxationsrate, klinischen Scores und subjektiven Faktoren soll das
postoperative Outcome von primär und sekundär versorgten Schulerluxationen
gegenübergestellt und bewertet werden. Zusätzlich soll der Stellenwert der
Schulterstabilisierungsoperation nach Caspari anhand der aktuellen Literatur
gegenüber offenen Verfahren und arthroskopischen Verfahren mit
Nahtankertechnik bestimmt werden. Im Gesamtkollektiv zeigt sich eine
Reluxationsrate nach Operation nach Caspari von 21,4% (11,1% inadäquate
Reluxationstraumen). Im Vergleich dazu ergibt sich eine Gesamtreluxationsrate
von 6,1% in der Gruppe der Erstluxierer (3% inadäquate Reluxationstraumen). In
der Gruppe der Patienten mit rezidivierenden traumatisch bedingten Luxationen
zeigt sich eine Rezidivrate von 27,7% (14% inadäquate Reluxationstraumen).
Patienten, die jünger als 30 Jahre alt sind, voroperiert wurden und mehrere
präoperative Luxationen erlitten haben, haben ein deutlich höheres Risiko ein
Rezidiv zu erleiden. Es wurde im Gesamtkollektiv ein Durchschnittswert von
28,3 Punkten im subjektiven Teil des ASES-Scores, 94 Punkten im Constant-
Score, 88,6 Punkten im Rowe-Score und ein Durchschnittswert von 32,5 Punkten
im UCLA-Score ermittelt. Bei der postoperativen subjektiven Einschätzung der
Schmerzempfindung gaben 92,9% an, zumindest zufrieden zu sein, in der
Kategorie subjektives Kraftgefühl 87,9%, subjektive Funktionalität 86,5%,
Beweglichkeit 84,6% und subjektive Stabilität 79,4%. In der Gruppe der
Erstluxierer zeigten sich in allen Scores bessere Ergebnisse. Lediglich in der
subjektiven Einschätzung der postoperativen Beweglichkeit und Funktionalität
der Schulter waren prozentual weniger Patienten zumindest zufrieden. In der
Gruppe der rezidivierenden traumatisch bedingten Luxationen wurden
entsprechend schlechtere Ergebnisse als in der Gruppe der Erstluxierer
erzielt. Eine geringere präoperative Luxationszahl, ein kürzeres Zeitintervall
zwischen Erstereignis und Primäreingriff, eine professionelle Reposition bei
der Erstluxation, ein Alter von über 30 Jahren und eine postoperative
Schulterimmobilisationszeit von weniger als 6 Wochen führte im Gesamtkollektiv
zu besseren Score-Ergebnissen. In der Gruppe der Erstluxierer führte eine
Ruhigstellungszeit von weniger als 6 Wochen und ein Alter von über 20 Jahren
zu besseren Score-Ergebnissen. Es konnte gezeigt werden, dass bezüglich der
Reluxationsrate, der Score-Ergebnisse und der subjektiven postoperativen
Zufriedenheit der Patienten in der Gruppe der primär versorgten Luxationen
deutlich bessere Resultate erreicht werden konnten, als in der Gruppe der
sekundär versorgten Patienten. Unter Betrachtung der Rezidivrate, der Score-
Ergebnisse und der Wiedererlangung des gleichen präoperativen
Aktivitätsniveaus, kann festgehalten werden, dass die arthroskopische
Operationstechnik nach Caspari nahezu gleichwertig der arthroskopischen
Nahtankertechnik, aber den offenen Verfahren unterlegen ist.

We evaluated 126 patients at a mean of 6.4 years following arthroscopic
shoulder repair in transglenoidal suture technique. There were 37 female and
89 male patients with mean age of 29.9 years (range, 15 to 68 years). The
outcome of treatment was evaluated with the UCLA-Score, Rowe-Score, Constant-
Score, modified Rowe-Score and a part of the ASES-Score. 33 patients were
treated after the first traumatic shoulder dislocation, 93 patients were
treated after more than one shoulder dislocation. By using redislocation-rate,
clinical scores and subjective measurements we opposited primary and secondary
treated traumatic anterior shoulder dislocations. The second purpose of this
study was to compare the results of shoulder stabilization in caspari
technique with the arthroscopic suture anchor repair and open stabilization
treatments in view of the common literature. The average ASES-score was 28.3.
The average Rowe-score was 88.6. The average Constant-score was 94. The
average UCLA-score was 32.5. There were 21.4% of redilocations (11.1%
atraumaic redislocations). Patients, who were treated primary after the first
traumatic shoulder dislocation, redislocated in 6.1% (3% atraumatic
redislocations) and presented better results in all asked scores than
secondary treated patients. There is a higher risk for redisocation after
arthroscopic transglenoidal shoulder stabilization for patients who were
younger than thirty years, who had a lot of redislocations before arthroscopic
treating and who were treated by revision operations. There are better results
in scores for patients with a low number of preoperativ dislocations, with
professional reposition at the first dislocation, who were older than thirty
years, with a short time between dislocation and operativ treatment and with
shoulder immobilization less than 6 weeks. We showed that patients, who were
treated primary after the first shoulder dislocation obtained better results
considering redislocation-rate, score-results and subjective parameters than
patients who were treated secondary. We presented comparable results to the
suture anchor technique but less results to open stabilization treatments
considering scores, redislocation and subjective parameters.
