Einleitung: Die vorliegende Arbeit beschäftigt sich mit historischen Autopsiepräparaten von neuroepithelialen Hirntumoren aus der Pathologischen Sammlung des Berliner Medizinhistorischen Museums der Charité (BMM) und ihrer Funktion als Wissenschafts- und Lehrobjekt. Die Arbeit ist an der Schnittstelle zweier betreuender Institutionen verankert: Das BMM liefert die historischen Präparate sowie die zur Katalogisierung nötige Infrastruktur, das Institut für Neuropathologie ermöglicht die Durchführung der Aufarbeitung ausgewählter Präparate, um die zentrale Frage zu beantworten, welche Diagnosen sich mit modernen Analysemethoden an den Präparaten heute stellen lassen. Darauf aufbauend wird untersucht, ob die vorliegenden Präparate heute noch von potentiellem Nutzen für Forschung oder Lehre sind. Methoden: Anhand der aktuellen WHO-Klassifikation für Tumoren des zentralen Nervensystems wurden Tumoren neuroepithelialen Ursprungs als Einschlusskriterium für das zu untersuchende Kollektiv bestimmt. Gemäß dieser Definition wurden 113 Präparate mit der historischen Diagnose eines Glioms in der Sammlung des BMM verortet, in die Studie eingeschlossen und wissenschaftlich katalogisiert unter Verwendung der GOS-Datenbank. Für die Neubefundung mittels histologischer, immunhistochemischer und molekularer Methoden wurden 29 Präparate ausgewählt, um vor allem infratentoriell gelegene Tumoren im Kindes- und Jugendalter mit (aus heutiger Sicht) zweifelhafter historischer Diagnose zu überprüfen.
Ergebnisse: Als Ergebnis der wissenschaftlichen Katalogisierung ist ein Kollektiv aus 113 Präparaten entstanden, das für weitere wissenschaftliche, didaktische und museale Arbeiten genutzt werden kann. Die 29 Neubefundungen führten zu insgesamt 11 aktuellen Diagnosen: Das Gewebe war grundsätzlich noch gut zur Erstellung eines Schnittpräparates geeignet, sodass in allen Fällen HE-Färbungen angefertigt werden und jeweils eine histomorphologische Charakterisierung sowie eine histologische Einordnung gemäß der aktuellen WHO- Klassifikation erfolgen konnten. Von den immunhistochemischen Markern hingegen konnten an dem Gewebe aus technischen Gründen nur GFAP/NEUROFILAMENT angewandt werden. Für die molekularen Analysen wiederum war das Material noch gut geeignet.
Diskussion und Ausblick: Der wissenschaftliche Katalog demonstriert den didaktischen Wert der historischen Präparate. Auf der Grundlage der in dieser Arbeit untersuchten und erschlossenen Präparate des BMM wurde daher ein objektzentriertes Lehrkonzept für eine modulare Unterrichtseinheit im Rahmen des Modellstudiengangs Medizin der Charité erarbeitet. Schließlich erscheint es unverzichtbar, den reichen Bestand an Präparaten des BMM für künftige Generationen zum Zwecke der Lehre und Forschung zu erhalten.

Introduction: This study deals with historical pathological preparations of human brains with neuroepithelial brain tumours from the pathological collection of the Berlin Museum of Medical History at the Charité (BMM) and their function as science and teaching objects. The project is located at the intersection of two host institutions: The BMM offers access to the historical pathological preparations and the use of a specialized data base (GOS) in order to establish a scientific catalogue of all specimens. The Department of Neuropathology at the Charité provides the neuropathological re-evaluation of selected cases in order to answer the main question which diagnostic results are to be found in these historical specimens using modern analytical techniques. Depending on the results we investigate the potential use of these historical specimens in today’s medical research and teaching as a practical outcome.
Methods: With regards to the 2016 WHO Classification of Tumours of the Central Nervous System, the defined group of gliomas or neuroepithelial brain tumours was established as the main inclusion criterion for the historical preparations. According to this definition, 113 brains with a historical diagnosis of a glioma were found in the BMM and included in this study. They were scientifically catalogued using the GOS data base. Regarding the re-evaluation with state- of-the-art histology, immunohistochemistry and molecular methods, we selected a number of 29 cases consisting predominantly of infratentorial paediatric glial tumours.
Results: As a result of the scientific cataloguization using the GOS data base, a digital catalogue including all 113 preparations of brains with neuroepithelial brain tumours of the BMM was established. Furthermore, the 29 cases that were neuropathologically re-evaluated resulted in 11 modern, representative diagnoses with respect to the 2016 WHO Classification.
Discussion and Conclusion: The scientific catalogue demonstrates the didactical value of the historical specimens as they show oncological diseases in a way that is hardly to be found in modern macroscopic preparations, e.g. full-blown clinical pictures of gliomas. While it could be shown that the historical diagnoses had to be revised in 62% of the re-evaluated cases thus suggesting that historical specimens should not be used in modern medical teaching without re- evaluation, one can strongly recommend the use of properly re-evaluated cases in today’s medical teaching. Finally, future methods may eventually offer new research options making the BMM’s specimens worth to be conserved for future generations of doctors and scientists.
