Beim Nierenzellkarzinom (NZK) bedeutet die Diagnose „Knochenmetastasen“ für
die betroffenen Patienten, dass eine Kuration ausgeschlossen ist. Durch
Knochenmetastasen werden die Therapieform, die Lebensqualität und häufig der
Todeszeitpunkt entschieden. Beim Knochenaufbau und -abbau entstehen Produkte,
deren Nachweis im Serum als Knochenmarker möglich ist. Veränderungen des
Knochenstoffwechsels können durch die Knochenmarkerkonzentration im Blut
widergespiegelt werden. Bisher diente die Skelettszintigraphie als Methode der
Wahl zur Diagnostik skelettaler Läsionen. Aufgrund zahlreicher Nachteile bei
der Skelettszintigraphie, könnten zukünftig nicht-invasive Parameter als
Indikatoren zur Detektion und Progression von Knochenmetastasen beim NZK
genutzt werden. In der vorliegenden Untersuchung wurden 41 Patienten mit NZK
und ossären Metastasen untersucht. Alle Patienten erhielten ein Jahr lang alle
drei Wochen eine Infusion mit 4 mg Zoledronsäure und eine tägliche Zugabe von
Kalzium und Vitamin D. Als primärer Endpunkt wurde das Auftreten von ≥ 1 SRE
(skeletal-related events) während 12 monatiger Zoledronsäuretherapie
definiert. Als SRE werden pathologische Frakturen, Wirbelkörperkompressionen
und die chirurgische oder strahlentherapeutische Intervention bei ossären
Metastasen bezeichnet. Zur Evaluierung wurden die gesamte alkalische
Phosphatase tALP, ihr knochenspezifisches Isoenzym bALP und Prokollagen I
N-terminales Propeptid (PINP) als Marker der Knochenformation sowie
carboxyterminales quervernetztes Typ I-Kollagen-Telopeptid (ICTP) als Marker
der Knochenresorption ausgewählt. Die Patienten wurden in Gruppen mit und ohne
SRE im Studienverlauf eingeteilt. Ein Problem stellten die vorzeitigen
Studienabbrüche bei Patienten mit SRE dar. Die Ausgangsmessungen vor
Zoledronsäuretherapie ergaben bei den Patientengruppen mit
Verlaufsuntersuchungen und SRE signifikant höhere bALP- und ICTP-Ausgangswerte
im Vergleich zu den Patientengruppen ohne SRE. Leider lässt sich anhand dieser
Werte keine Korrelation zur Klinik erkennen. Vor Zoledronsäuretherapie waren
die Ausgangswerte von PINP und tALP weder bei Patienten mit SRE noch bei
Patienten ohne SRE signifikant erhöht. Während der Studiendauer korrelierte
das Verhalten von tALP, PINP und ICTP nicht mit dem Krankheitsverlauf der
Patienten mit ossär metastasiertem NZK. Die im Serum gemessenen individuellen
bALP-Ausgangswerte (sowohl bei Patienten mit SRE, als auch bei Patienten ohne
SRE) zeigten sich im Verlauf der Zoledronsäurebehandlung rückläufig, eine
Signifikanz war jedoch nur bei den Patienten ohne SRE zu einigen Zeitpunkten
feststellbar. In der Gruppe der Patienten mit SRE ist allerdings zu
berücksichtigen, dass nur eine kleine Fallzahl (n = 7) untersucht wurde und
sogleich vorzeitige Studienabbrecher vorlagen, so dass eine Übertragung der
Ergebnisse nur eingeschränkt möglich ist. In dieser Studie war weder eine
Prädiktion von SRE noch die Verlaufskontrolle der Knochenmetastasierung beim
NZK unter Zoledronsäuretherapie mittels Knochenmarkerbestimmung möglich.

dc.description.abstract
In the case of renal cell carcinoma (RCC), the diagnosis “bone metastases”
means that a complete cure is highly unlikely. Bone metastases often determine
the form of therapy, quality of life and time of death. During the process of
bone formation and bone resorption specific substances are produced, which can
later be used as bone markers in the serum. Changes in bone metabolism may be
reflected by the concentration of these specific bone markers in the blood.
Until now, bone scintigraphy has been used as the method of choice for the
diagnosis of skeletal lesions. Due to numerous disadvantages of bone
scintigraphy, non-invasive parameters (such as bone markers) could be used as
indicators for the detection and progression of bone metastases in RCC in the
future. In this study, 41 patients with RCC and bone metastases were examined.
During one year, all patients received every three weeks an infusion of
zoledronic acid 4 mg and a daily dose of calcium and vitamin D. The primary
end point was the incidence of ≥ 1 SRE (skeletal-related events) during the 12
months of zoledronic acid therapy. Pathological fractures, spinal cord
compression and the need of surgical or radiotherapeutic intervention because
of bone metastases were defined as SRE. The total alkaline phosphatase (TALP),
its bone-specific isoenzyme (BALP) and procollagen type I amino-terminal
propeptide (PINP) were used as markers of the bone formation process, whereas
carboxyterminal telopeptide of type I collagen (ICTP) was implemented as a
marker of the bone resorption process. During this study, the patients were
divided into groups with and without SRE. Early discontinuation among patients
with SRE represented a problem of this study. The baseline concentrations of
BALP and ICTP before zoledronic acid therapy were significantly higher within
the group of patients with follow-up examinations and SRE compared to the
group without SRE. Unfortunately, there was no significant statistical
correlation between the concentrations of bone markers and the clinical
development in this study. Neither in the group of patients with SRE nor
without SRE were the baseline concentrations of PINP and TALP before
zoledronic acid therapy increased. During the study period, there was no
statistical correlation between the changes in the concentrations of TALP,
PINP and ICTP with the clinical development of patients with osseous
metastatic RCC. The individual BALP baseline concentrations (both in patients
with SRE, as well as in patients without SRE) declined, but showed only a
statistical significance in the group of patients without SRE at certain
points during the study. It should be noted that in the group of patients with
SRE, only a small number of patients (n = 7) were examined, and early dropouts
led to a limited transferability of the results. In this study, neither a
prediction of the RCC-related SRE nor the development of bone metastasis
during zoledronic acid therapy based on bone markers was possible.
