The CellModule, bioartificial component of the modular extracorporeal liver support system (MELS), is a hollow-fibre based bioreactor inoculated with human liver cells. In addition to artificial detoxification, it is aimed to provide a biological element to patients in liver failure. In a retrospective analysis, 47 bioreactor cultures were analysed concerning their biochemical performance. A method of classification based on the daily urea production within the time span of potential clinical application was developed. Additionally, a morphological analysis of the bioreactor war performed and a case report was presented as example for clinical application in patients with liver failure.
Das CellModule ist die bioartifizielle Komponente des modularen extrakorporalen Leberunterstützungssystems (MELS), und dient dazu, Patienten im Leberversagen neben rein artifiziell entgiftenden Systemen auch eine biologische Komponente zur Verfügung zu stellen. Der auf Hohlfasern basierte Bioreaktor wird mit primären humanen Leberzellen befüllt. In einer retrospektiven Analyse der biochemischen Leistung wurden die Stoffwechselparameter von 47 Bioreaktoren verglichen und eine Methode zur Auftrennung in leistungsfähige und weniger leistungsfähige Systeme auf der Basis der täglichen Harnstoffproduktion während des potentiellen Therapiezeitraums vorgeschlagen. Zusätzlich wurde das System immunhistochemisch und elektronenmikroskopisch analysiert und die Fallstudie einer Patientin als Beispiel für die Klinikanwendung vorgestellt.
