Infektionen und Abstoßungsreaktionen sind die häufigsten Komplikationen nach
Lebertransplantation.
Transforming Growth Factor-ß (TGF-ß) hat komplexe Wirkungsmechanismen in der
Zellproliferation, Zelldifferenzierung, Zellwachstum, sowie in der
Immunmodulation. Um die Bedeutung von TGF-ß als diagnostischer Parameter nach
Lebertransplantation zu untersuchen, wurden bei 81 Patienten mit 85
Transplantationen in der vorliegenden prospektiven Studie TGF-ß Spiegel zu
definierten Zeitpunkten im Plasma nach Lebertransplantation gemessen.
28 Patienten entwickelten eine akute Abstoßung (AR), von denen 14 steroid-
resistent (SR) waren und eine zusätzliche Therapie mit FK 506 oder OKT 3
benötigten. 4 Patienten entwickelten eine frühe chronische
Transplantatabstoßung (CR).
Bei 10 Patienten trat eine schwere Infektion auf, die in 5 Fällen letal
endete. Eine asymptomatische oder milde Cholangitis wurde bei 11 Patienten
diagnostiziert und 37 Patienten hatten einen weitgehenden unauffälligen
postoperativen Verlauf.
Bei steroid-resistenten Abstoßungsreaktionen kam es zu keiner Erhöhung des
TGF-ß im Vergleich zur unauffälligen Vergleichsgruppe. Im Falle von steroid-
sensitiven Abstoßungen war TGF-ß jedoch signifikant zu diesen beiden Gruppen
erhöht.
Patienten mit chronischer Abstoßung hatten einen signifikant erniedrigten
TGF-ß Spiegel. Bei den Patienten ohne Abstoßungsreaktion imponiert besonders
ein TGF-ß Peak, der im Verlauf jedoch rasch wieder abfällt.
Die TGF-ß Spiegel bei den unterschiedlichen initialen Transplantatfunktionen
und im Vergleich zu leber- und zentralvenösen Blutes differierten nicht
wesentlich.
TGF-ß könnte sich als wichtiger Faktor zur frühen Differenzierung von steroid-
senitiven und steroid-sensiblen Abstoßungen sowie der frühen Diagnostik von
chronischen Abstoßungsreaktionen erweisen. Als zusätzlicher Routineparameter
zur Verlaufskontrolle nach Lebertransplantation erscheint der Wert von TGF-ß
jedoch nur beschränkt sinnvoll.

Severe infections and rejections are the most frequent complications following
liver transplantation.
Transforming Growth Factor-ß (TGF-ß) has a complex array of functionalities in
the proliferation, differentiation and growth of almost all human cells, as
well as an established effect in the immune response.
In order to assess the significance of TGF-ß as a potential diagnostic
parameter following liver transplantation we studied the TGF-ß serum levels as
well as other routine parameters in 81 patients with 85 liver transplantations
at defined pre-, intra- and postoperative points of time.
28 patients (34,6%) developed an acute rejection (AR), of which 14 proved to
be steroid resistant and required further treatment with FK 506 or OKT 3. 4
patients developed an early chronic rejection (CR) within the first
postoperative year.
10 patients suffered from a severe postoperative infection, out of which 5
patients died in the course of this complication. Further 11 patients
developed a mild or asymptomatic cholangitis, while 37 patients had an
uneventful postoperative course.
We observed no elevation of TGF-ß in the cases of steroid resistant rejections
in comparison to the group with the uneventful postoperative course. However,
patients with steroid sensitive rejections had a significant elevation of
TGF-ß serum levels, in comparison with the two other groups.
Patients with early chronic rejection showed significant low levels of TGF-ß.
Patients without any signs of rejection experience a significant peak of TGF-ß
which falls to normal levels within a short period of time.
There was no significant difference in TGF-ß levels in respect to the
different initial functioning of the transplant, as well as no difference in
central-venous and liver-venous blood.
TGF-ß could prove to be an important parameter for early differentiation of
steroid sensitive and resistant acute rejections. Further TGF-ß could play a
potential role in the early diagnostic of chronic rejections. However the
value of TGF-ß as an additional routine parameter following liver
transplantation does not seem to be sufficiently high.
