Die lymphoproliferative Erkrankung post transplantationem (posttransplant
lymphoproliferative disease, PTLD) ist eine schwerwiegende Komplikation. Die
Mortalität der Erkrankung liegt zwischen 40 und 60 %, wobei die Inzidenz des
Transplantatsversagens bis zu 60 % betragen kann. Eine Reihe von früheren
Studien zeigte eine Assoziation zwischen PTLD und EBV-Infektionen. Nach dem
Zeitpunkt der Lymphom-Manifestation kann man zwischen einer frühen PTLD (PTLD-
Entwicklung innerhalb der ersten 3 Jahre nach Transplantation) und einer
späten PTLD (Entstehung der Lymphome bei Patienten mit dem Transplantatalter >
3 Jahre) unterscheiden. Ähnlich wie frühe PTLD haben Patienten mit später PTLD
meistens eine erhöhte EBV Viruslast im Blut. Jedoch entwickeln nur einige
Patienten mit hoher EBV Viruslast eine PTLD. Die Definition von Risikofaktoren
würde das Monitoring erleichtern und evtl. eine preemptive antivirale Therapie
gezielter ermöglichen. Eine wichtige Rolle in der Kontrolle einer EBV-
Infektion sowie maligner Erkrankungen nimmt die zelluläre Immunantwort ein.
Insbesondere der Balance in der Produktion pro- und anti-inflammatorischer
Zytokine durch die Immunzellen scheint den Verlauf dieser Erkrankungen
bedeutend zu beeinflussen. Die Stärke der Zytokinexpression wird im
Wesentlichen durch genetisch determinierte Polymorphismen in kodierenden und
nicht-kodierenden Bereichen der Zytokingene bestimmt. Unter Verwendung von
SSP-PCR (Sequence-Specific-Primer-PCR) für die Genotypisierung wurde in der
vorliegenden case-control Studie der prädiktive Wert von Einzelnukleotid-
Polymorphismen von -1082IL-1O,-308TNF- α, TGF-ß1 (codon 10,25), und
+874IFN--Genen für die Entstehung von späten EBV-assoziierten
Posttransplantations-Lymphomen untersucht. 38 Patienten mit später EBV-
assoziierter PTLD und 408 gematchte Empfänger solider Transplantate als
Kontrollgruppe wurden in die Studie inkludiert. Die molekular-genetische
Analyse von Einzelnukleotid- Polymorphismen zeigte, dass TGF-ß1 (codon 25) CC-
Genotyp, das eine niedrige Zytokinproduktion kodiert, signifikant häufiger bei
Patienten mit PTLD vorlag als in der Kontrollgruppe (OR=0,34; 95% CI 0.17 to
0.69, P=0,0022). Zusätzlich war die Häufigkeit von -1082 IL-10 GG-Genotyp, das
mit einer hohen Zytokinproduktion assoziiert ist, in der Kontrollgruppe höher
als bei Patienten mit PTLD (OR=0,5; 95% CI 0.23 to 1.0, P=0,048). Wir fanden
keine Assoziationen zwischen -308TNF- α, TGF-ß1 codon 10 und +874IFN-
Einzelnukleotid-Polymorphismen und PTLD. Des Weiteren bestand kein
Zusammenhang zwischen Erkrankungsverlauf bzw. -Ausgang und den untersuchten
Zytokinpolymoprhismen. Unsere Daten demonstrieren somit klare Evidenzen für
die wichtige Rolle der zwei Th2-Pathway-Schlüsselzytokine als Risikofaktoren
für die PTLD-Entwicklung. Dies kann auf einer Seite zum besseren Verständnis
der Immunpathogenese der späten EBV assoziierten PTLD beitragen, andererseits
bieten Ergebnisse dieser Arbeit eine Grundlage für die Risikostratifizierung
transplantierter Patienten sowie der Patienten, die auf einer
Transplantationswarteliste stehen.

Posttransplant lymphoproliferative disorder (PTLD) is a serious complication.
The disease can lead to mortality which rate ranges between 40 and 60%. In
this, the incidence of transplant failures amounts up to 60%. A series of the
leading surveys report the association of PTLD with EBV infection. When
lymphoma has been detected, one can differentiate between an early PTLD (PTLD
development within the first three years upon the transplantation) and late-
occurring PTLD (the patients with transplant age of more than three years
demonstrated lymphomas development). Both groups of patients, with early PTLD
and with late-occurring PTLD, have an elevated EBV load in the blood. However,
only some patients with an elevated EBV load demonstrated PTLD. The
identification of risk factors could facilitate the monitoring, and, finally,
provide for the target preemptive antiviral therapy. Cellular immune response
plays an important role in the control over EBV infection and malignant
diseases. In particular, a balance in the production of pro- and anti-
inflammatory cytokines by immune cells seems to be a significant factor to
affect the course of these diseases. The strength of cytokine gene expression
in coding and non-coding cytokine genes mainly depends on genetically
determined polymorphisms. This case – control study analyzed predictive values
of single-nucleotide polymorphisms for -1082IL-10, -308TNF-α, TGF-β1 (codon
10,25), and +874IFN-γ genes by means of SSP – PCR (Sequence Specific Primer -
PCR) for genotyping, in terms of development of the late onset EBV-associated
posttransplant lymphoma. There were 38 patients with late onset EVB-associated
PTLD and 408 matched solid transplant recipients enrolled in the study as a
control group. The molecular – genetic analysis of single-nucleotide
polymorphisms demonstrated that TGF-β1 (codon 25) CC genotype, that coded low
cytokine production, was detected much more frequently in PTLD patients than
in the control group (OR=0,34, 95% Cl 0.17 to 0.69, P=0,0022). Furthermore,
the frequency of -1082 IL-10 GG genotype, which is associated with high
cytokine production, was greater in the control group than in PTLD patients
(OR=0,5, 95% Cl 0.23 to 1.0, P=0,048). We have not found any associations
between 308TNF-α, TGF-β1 codon 10 and +874IFN-γ single-nucleotide
polymorphisms and PTLD. Moreover, no associations were found between the
disease course and outcome, and the cytokine polymorphisms investigated. Our
research provides, therefore, vivid evidence of the significant role of the
two Th2 pathway key cytokines as risk factors for PTLD development. On the one
hand, this contributes to better understanding of immunopathogenesis of late
onset EVB-associated PTLD, and, on the other hand, the results of this study
can serve as a basis for risk stratification in transplant patients and in
patiets still on the transplantation waiting list.
