Humane glomeruläre Entzündungen sind ist durch Proteinurie gekennzeichnet und können mit Natriumretention, Bluthochdruck und Ödembildung einhergehen. Hohe Mengen an filtriertem Protein gelangen in den Nierentubulus, wo sie den epithelialen Transport verändern können. Eine durch Proteinurie erhöhte Proteinendozytose mit zellulärer Überladung kann den proximalen Tubulus beeinträchtigen, jedoch wurden auch intrinsische Funktionsstörungen distaler Epithelien beschrieben. Um die mögliche Beteiligung einzelner Tubulussegmente an Funktionsstörungen des Natrium- und Volumenhaushalts bei proteinurischer, glomerulärer Erkrankung aufzuklären, wurde eine anti-GBM-Glomerulonephritis (GN) in einer Mauslinie mit einer nierenspezifischen Megalin-Teildefizienz induziert, die eine verminderte rezeptorvermittelte Endozytose aufweist. GN- Mäuse mit bzw. ohne durch Megalin-Teildefizienz bedingten Endozytosedefekt wiesen bei einer Proteinurie von 10-15 mg/24 h eine reduzierte fraktionelle Natriumexkretion und einen erhöhten systolischen Blutdruck auf. Der Endozytosedefekt rief im proximalen Tubulus eine erhöhte epitheliale Expression von NaPi-IIa und AQP1 hervor, aber keine Veränderung von NHE3. Die GN bewirkte, unabhängig von Megalin, jedoch eine generell verminderte NaPi- IIa- und AQP1-Expression, sowie eine verringerte NHE3-Aktivität, die vermutlich durch eine andere NHE-Isoform kompensiert war. Hingegen konnten im Verbindungstubulus und im Sammelrohr eine erhöhte ENaC-Expression bei tubulärer und glomerulärer Proteinurie gefunden werden. Immunblots zeigten neben den charakteristischen full-length-Banden der alpha- und gamma- Untereinheiten von ENaC signifikante Zunahmen zusätzlicher, kleinerer ENaC- Spaltprodukte. Die Auswirkungen auf diese Zunahmen waren bei der glomerulären Proteinurie stärker als bei einer durch verminderte rezeptorvermittelte Endozytose hervorgerufenen niedermolekularen Proteinurie. Die vermehrt auftretenden, kleineren alpha- und gamma-ENaC-Banden weisen auf eine verstärkte proteolytische Spaltung des ENaC hin, welche zur Erhöhung der ENaC- Aktivität beitragen kann. Erhöhte immunhistochemische Signale der alpha- Na,K-ATPase im Verbindungstubulus deuten zusammen mit einer erhöhten apikalen ENaC-Expression und dem Auftreten vermutlicher ENaC-Spaltprodukte auf eine lokal verstärkte Natriumretention hin. An Affinitätsbeads immobilisierter Sammelurin aus Megalin-teildefizienten Mäusen wurde mit künstlichen Peptiden mit Sequenzen aus den extrazellulären Schleifen der alpha- und der gamma-ENaC- Untereinheit inkubiert. Die Peptide wurden danach mit der MALDI-TOF- Massenspektrometrie analysiert. Gefundene Schnittstellen belegen die proteolytische Wirkung der Proteasen Plasmin, TMPRSS4 und Kallikrein 1. Die gewonnenen Ergebnisse stützen das Konzept einer Natriumretention sowohl bei tubulärer wie auch bei glomerulärer Proteinurie als mögliche Ursachen für Bluthochdruck und Ödembildung bei glomerulären Entzündungen.
Human glomerular inflammatory diseases are accompanied by sustained proteinuria and may involve sodium retention, hypertension, and edema formation. High quantities of filtered protein enter the renal tubule, where they may alter epithelial transport functions. Exaggerated protein endocytosis caused by proteinuria, and consequent protein overload may affect proximal tubules, but intrinsic malfunction of distal epithelia has also been reported. In order to elucidate a possible involvement of particular tubule segments in disturbances of sodium and volume handling occurring in proteinuric glomerulopathy, an anti-GBM glomerulonephritis (GN) was induced in a mouse strain with a kidneyspecific megalin-deficiency, featuring blunted receptor mediated endocytosis. Treated mice with or without endocytosis defect caused by megalin-deficiency showed a reduced fractional sodium excretion, and elevated systolic blood pressure when proteinuria had reached 10-15 mg/24 h. Blunted endocytosis caused in part an increased epithelial expression of NaPi- IIa and AQP1, but did not change NHE3 abundance. However, GN reduced NaPi-IIa and AQP1 expression independently from megalin, and decreased NHE3 activity as well, which was probably balanced by another NHE isoform. However, in the connecting tubule and the collecting duct, an increased ENaC expression was found both under tubular and glomerular proteinuria. Immunoblots of ENaC showed, in addition to the characteristical full-length bands of its alpha- and gamma-subunits, significant increases of additional, smaller cleavage products of the channel. The effects of glomerular proteinuria dominated over those of a low molecular weight proteinuria, caused by blunted proximal endocytosis, in their contributions to the increments of smaller bands of the ENaC subunits. Increased abundances of smaller alpha- and gamma-ENaC bands suggest a proteolytic cleavage of ENaC, which contribute to an elevated channel activation. With GN, enhanced immunohistochemical signals of alpha- Na,K-ATPase in the connecting tubule in conjunction with increased ENaC surface expression, and the abundance of the putative ENaC cleavage products point to locally elevated sodium retention. Accumulated urine from megalin- deficient mice was immobilized to affinity beads and incubated with synthesized peptides, containing sequences from the extracellular loops of the alpha- and gamma-ENaC subunits. The peptides were subsequently analyzed by MALDI-TOF mass spectrometry. Cleavage sites found account for the proteolytic action of the filtered plasma protease plasmin and proteases secreted from the tubular epithelium like TMPRSS4 and kallikrein 1. The data obtained thus support the concept of sodium retention in both tubular and glomerular proteinuria as possible causes for hypertension and edema formation occurring in glomerular inflammatory diseases.
