Einleitung: Einer der wichtigen Aufgabenbereiche in der Funktionsdiagnostik ist die Bestimmung von Asymmetrien. Obwohl bereits in einer Vielzahl von Studien die Abbildungsfehler vom OPG untersucht und bestätigt wurden, werden funktionsdiagnostische Untersuchungen bis heute zum Großteil mit Hilfe von OPGs durchgeführt. Das Ziel der vorliegenden Untersuchung bestand darin festzustellen, mit welchem radiologischen Verfahren (OPG oder DVT) Asymmetrien im Bereich des aufsteigenden Unterkieferastes bestmöglich untersucht werden können. Zur Messung und Auswertung der Asymmetrien wurden die Asymmetrie- Indizes von Habets et al. und Kjellberg et al. sowie ein eigens entwickelter, modifizierter Asymmetrie-Index verwendet. Methodik: Die Untersuchung erfolgte retrospektiv anhand der OPG und DVT Aufnahmen von 79 Patienten. Zunächst mussten die Messungen auf Reproduzierbarkeit überprüft werden. Dazu wurden die Aufnahmen von 30 Patienten dreimal im Abstand von je einer Woche vom gleichen Untersucher vermessen. Während den Messungen zur Reproduzierbarkeitsermittlung konnte festgestellt werden, dass die genaue Lage einiger Punkte aufgrund von Überlagerungen nicht zu 100% identifiziert werden konnte. In diesem Rahmen haben sich neue Messstrecken und damit ein modifizierter Asymmetrie-Index entwickelt. Da jeweils ein OPG und ein DVT von jedem Patienten vorhanden waren, wurden nun die Aufnahmen systematisch mit den Asymmetrie-Indizes nach Habets et al., Kjellberg et al. und dem modifizierten Asymmetrie-Index vermessen und anschließend miteinander verglichen. Da OPG und DVT jedoch nicht im gleichen Maßstab vorlagen, mussten Ratios gebildet werden, um den Maßstab auszuschalten und so einen direkten Vergleich zu ermöglichen. Es wurden sowohl die einzelnen Strecken als auch die Indizes von OPG und DVT miteinander verglichen. Außerdem wurden die im DVT ermittelten Asymmetrie-Indizes hinsichtlich ihrer Aussage miteinander verglichen. Die erhobenen Messdaten wurden in eine Excel Tabelle eingetragen, gegenübergestellt und mittels Bland- Altman-Plots, Kreuztabellen, Cohens Kappa und dem GFK ausgewertet. Ergebnisse: Sowohl im OPG als auch im DVT konnte die Reproduzierbarkeit der Messungen bestätigt werden (Reliabilitätskoeffizienten nach Houston von 88,19% bis 99,05%). Beim Vergleich der einzelnen Messstrecken von Kjellberg et al. wurden Abweichungen zwischen OPG und DVT von bis zu 39% festgestellt. Beim Vergleich der Asymmetrie-Indizes (Habets et al. Kjellberg et al., modifizierter Asymmetrie-Index) wurden Abweichungen zwischen OPG und DVT von bis zu 35 % festgestellt (höchste Abweichung von 35% bei Habets AICH, kleinste Abweichung von 9% beim ModAI). Innerhalb der DVT- sowie OPG-Messungen konnten sehr hohe Übereinstimmungen zwischen Kjellberg´s SI1 und SI2 festgestellt werden(Cohens Kappa Wert von 0,949). Der Vergleich aller Asymmetrie-Indizes innerhalb der DVT ergab mit einem GFK von 0,192 eine leichte Übereinstimmung. Schlussfolgerung: Die Bestimmung von Asymmetrien im Bereich des aufsteigenden Unterkieferastes ist im OPG nur unzureichend möglich. Um eine vollständige funktionsdiagnostische Untersuchung zu gewährleisten sollte eine DVT angefertigt werden. Die Untersuchung auf mögliche Asymmetrien sollte mit Hilfe einer DVT und den Asymmetrie-Indizes nach Kjellberg et al. und/oder dem modifizierten Asymmetrie-Index erfolgen.
Introduction: One of the important tasks of functional diagnostics is the determi-nation of asymmetries. Although the imaging errors of an OPG have already been examined and approved in a variety of studies, functional diagnostic testing is up to the present day still being carried out with the help of OPG. The aim of this study was to determine which radiological procedure (OPG or CBCT) is most efficient to examine asymmetries in the region of the ascending ramus. In order to measure and evaluate the asymmetry, we used the asymmetry indices of Habets et al., Kjellberg et al. as well as a specially developed, modified asymmetry index. Methods: The study was carried out retrospectively using the OPG and CBCT images of 79 patients. First of all the measurements had to be checked for reproducibility. Therefore the images of 30 patients were measured three times by the same examiner at intervals of one week. During the measurements for the reproducibility check we found out, that the exact location of some landmarks could not be identified to 100% due to overlays. In this context, new landmarks and thus a modified asymmetry index were developed. Since there was one OPG and one CBCT present for each patient, the recordings were systematically measured with the asymmetry indices by Habets et al., Kjellberg et al. and the aforementioned modified asymmetry index. Subsequently the recordings were compared with each other. Since OPG and CBCT were not present on the same scale, ratios had to be set in order to neglect the scale. This way, a direct comparison was made possible. Both, the individual sections as well as the indices of OPG and CBCT were compared. In addition, the asymmetry indices collected with the CBCT were compared in terms of their outcome. The data collected was entered in an Excel spreadsheet, subsequently compared and finally evaluated by means of Bland - Altman plots, crosstabs, Cohen's kappa and the GFK. Results: Both the OPG and CBCT confirmed the reproducibility of the measurements (reliability coefficient to Houston from 88.19 % to 99.05%). When comparing the individual measurement sections of Kjellberg et al., deviations between OPG and CBCT up to 39% were observed. When comparing the asymmetry indices (Habets et al. Kjellberg et al., modified asymmetry index) deviations between OPG and CBCT up to 35% were observed (highest deviation of 35% at Habets AICH, the lowest deviation of 9% for the ModAI). Within the CBCT and OPG measurements a very high correlation was found between Kjellberg’s SI1 and SI2 (Cohen's Kappa value of 0.949). The comparison of all asymmetry indices within the CBCT (with a GFK of 0.192) resulted in a low correlation. Conclusion: The determination of asymmetries in the region of the ascending ramus with the help of OPG is insufficient. In order to guarantee a thorough functional diagnostic examination a CBCT should be applied. The examination of possible asymmetries should be carried out with the help of a CBCT and the asymmetry indices by Kjellberg et al. and/or the modified asymmetry index.
