Einleitung Die Gelenkendoprothetik ist eine der erfolgreichsten operativen
Verfahren. Trotz kontinuierlicher Verbesserung des Materials und der OP-
Technik ist die Standzeit der Endoprothese begrenzt. Nach jahrelanger Funktion
des Kunstgelenkes kommt es oft zur Auslockerung des Implantats aus dem
Knochen. Es gibt viele Hinweise darauf, dass Materialabrieb ein wichtiger
Faktor des Lockerungsprozesses ist. Das Abriebmaterial sammelt sich in der
Gelenkkapsel und induziert die Bildung einer Bindegewebsschicht zwischen
Implantat und Knochen („SLIM“). Die Abriebpartikel lassen sich in diesen
Geweben nachweisen und induzieren dort eine lymphozytäre Entzündung
(„CD3Response“). In der vorliegenden Arbeit wurde untersucht: 1. Der Einfluss
der unterschiedlichen Abriebpartikel auf die Quantität der CD3positiven
T-Lymphozyten, 2. der mögliche Einfluss positiver implantatmaterial-
allergologischer Befunde auf das lymphozytäre Infiltrat, 3. und die
Standzeiten von Präparaten mit Supramakropartikeln im Vergleich zu den
Standzeiten von Präparaten ohne Supramakropartikel. 4. Des Weiteren wurde in
Zusammenarbeit mit VmScope Berlin GmbH eine CD3Quantifizierungssoftware
(„CD3-Quantifier“) entwickelt. Material/Methode In der vorliegenden
histologischen Untersuchung wurden insgesamt 249 Präparate analysiert, die
anlässlich einer Revisionsoperation bei Endoprothesen des Hüft- und
Kniegelenkes gewonnen worden waren. Die Präparate wurden hinsichtlich der
Konsensusklassifikation nach Morawietz, Krenn et al. (2006) kategorisiert, die
Partikelidentifikation erfolgte gemäß des Partikelalgorithmus.
Immunhistochemisch wurden die CD3-positiven Lymphozyten dargestellt und
quantifiziert. Die CD3Quantitäten wurden den Gleitpaarungen gegenübergestellt.
Ergebnisse Die CD3-Quantität war bei den Me-Me Gleitpaarungen (n=49) am
stärksten (Mittelwert von 1367.6 gezählten Zellen). Die lymphozytäre Reaktion
bei Me-PE (Mittelwert von 243.3 gezählten Zellen) und Ker-PE Gleitpaarungen
(Mittelwert von 182.9 gezählten Zellen) war hochsignifikant geringer,
besonders bei der Ker-PE Gleitpaarung (n=44, p=1,888 x 10-10). Bei den Ker-Ker
Gleitpaarungen (Mittelwert von 124.6 gezählten Zellen) war die lymphozytäre
Reaktion hochsignifikant am geringsten (n=14, p=4,908 x 10-11). Bei Patienten
mit nachgewiesener Kontaktallergie auf das verwendete Implantatmaterial konnte
keine signifikante erhöhte CD3-Lymphozytose festgestellt werden (p=0.1492).
Die Standzeiten der Endoprothesen, bei denen Supramakropartikel vorlagen,
waren hochsignifikant höher (p=0.0009). Schlussfolgerung Das Vorliegen einer
starken CD3-Quantität bei Me-Me Gleitpaarungen könnte die schlechten
klinischen Ergebnisse dieser Materialkombination erklären. Alle anderen
Gleitpaarungen zeigten hochsignifikant kleinere Lymphozytosen. Ob die geringen
Unterschiede zwischen den Nichtmetall-Gleitpaarungen klinische Relevanz haben,
ist unklar. Zur objektivierten Beurteilung der CD3-Quantität als Substrat
einer adversen Reaktion wurde ein Grenzwert pro HPF ermittelt („CD3-Fokus-
Score“), dieser kann in der histopathologischen Diagnostik eingesetzt werden.
Das Auftreten von supramakropartikulärem Abriebmaterial bei Endoprothesen mit
sehr langer Standzeit scheint multifaktorielle Ursachen zu haben. Der
„CD3-Quantifier“ brachte einen Zeitgewinn, war aber nicht so zuverlässig wie
die manuelle Auszählung

Introduction Joint arthroplasty is a very successful surgical procedure.
However the lifespan of an endoprosthesis is limited whereby the most frequent
cause of failure is aseptic loosening. There is a great deal evidence that
material abrasion is an important factor of the loosening process. The
abrasion material accumulates in the joint capsule and induces the formation
of a tissue layer between implant and bone (“SLIM”). Abrasion particles can be
detected in these tissues and revise to trigger a lymphocytic inflammation
(“CD3-response”). The objectives of this study were: 1. The analysis of the
lymphocytic infiltrate depending on material. 2. The possible influence of
positive allergological findings on the lymphocytic infiltrate. 3. The
lifespan of endoprostheses with supramacroparticles compared to the lifetime
of endoprostheses without supramacroparticles. 4. Furthermore, in
collaboration with VmScope Berlin GmbH, a “CD3-Quantifier” was developed.
Material/Method For the histological study at hand, 249 preparations in total
were evaluated, which were obtained on the occasion of revision surgeries of
endoprostheses of hip and knee joints. The preparations were categorized
according to the consensusclassification of Morawietz, Krenn et al. (2006),
the particleidentfication occurred according to the particle algorithm. The
CD3-positive lymphocytes were displayed immunohistochemically and quantified.
Finally, the CD3-quantities were faced to the tribological pairings. Results
The Me-Me pairings (n=49) showed the highest amount of CD3-lymphocytes
(average of 1367.6 counted cells). The lymphocytic reaction of Me-PE (average
of 243.3 counted cells) and Ker-Pe pairings (average of 182.9 counted cells)
was significantly smaller, in particular for Ker-PE pairings (n=44, p=1,888 x
10-10). Cases with Ker-Ker pairings (average of 124.6 counted cells) showed
the smalles amount of CD3-lymphocytes (n=14, p=4,908 x 10-11). Patients with
verifiable contact allergy for the implant material used did not have a
significantly increased CD3-lymphocytosis. The lifetimes of the
endoprostheses, which showed supramacroparticles, were revealed to be
significantly prolonged (n=27, p=0.0009). Conclusion The presence of a strong
CD3-response for Me-Me pairings might explain the bad clinical results of this
material combination. It remains unclear, whether the slight differences for
the non-metal endoprostheses have clinical relevance. For an objective
assessment of the CD3-response as a result of an adverse reaction, a limit
value/HPF was established (“CD3-focus-score”). The appearance of
supramacroparticulate abrasion material for endoprostheses with a long
lifetime, seems to have multifactorial causes. The “CD3-Quantifier” lead to
time saving but did not have the same accuracy as the direct enumeration under
the microscope.
