Die Hypothese, dass muzinöse und nichtmuzinöse Karzinome organübergreifend
zwei Gruppen mit unterschiedlichen genetischen Läsionen bilden, wurde an zwei
Merkmalen überprüft: an der Expression des Tumorsuppressorproteins p53 und des
Muzins MUC2 an je 20 Tumoren des jeweiligen Typs in Kolon, Mamma, Ovar und
Pankreas. Die muzinösen Karzinome von Mamma, Ovar und Kolon wiesen eine
detektierbare p53-Expression mit einer Häufigkeit von 39-64% auf, gegenüber
der Häufigkeit von 57-72% in nichtmuzinösen Karzinome der jeweiligen Organe.
Damit besteht ein Trend zur selteneren p53-Expression in muzinösen Karzinomen
von Brust, Ovar und Kolon. Dieser Unterschied ist jedoch nur zwischen den
muzinösen und nichtmuzinösen Kolonkarzinomen statistisch signifikant. Die
MUC2-Expression von muzinösen und nichtmuzinösen Karzinomen von Mamma, Ovar,
Pankreas und Kolon ergab in allen untersuchten muzinösen Karzinomen eine
statistisch signifikant häufigere MUC2-Expression als in nichtmuzinösen
Karzinomen. In den Karzinomen von Mamma, Ovar und Pankreas handelt es sich bei
der MUC2-Expression um eine de novo-Expression, da die Normalgewebe dieser
Organe kein MUC2 exprimieren. D.h. die MUC2-Expression ist in diesen Organen
mit der malignen Transformation assoziiert. 52% der als nichtmuzinös
definierten Pankreas- und 9% der nichtmuzinösen Ovarkarzinome exprimieren
MUC2. Dies zeigt, dass die aufgrund der Morphologie als muzinös
klassifizierten Tumore von Pankreas, Ovar und Kolon auch auf der molekularen
Ebene Gemeinsamkeiten haben, die sie von den nichtmuzinösen Tumoren
unterscheiden, ohne jedoch zwei diskrete Gruppen zu bilden. Lediglich die
muzinösen Mammakarzinome unterscheiden sich in 100% der Fälle von den
nichtmuzinösen durch de novo MUC2-Expression. Eine inverse Korrelation
zwischen der p53- und MUC2-Expression als Hinweis auf eine mögliche
Interaktion zwischen p53 und MUC2 konnte ausgeschlossen werden. Zum Nachweis
einer humoralen Immunantwort auf das überexprimierte MUC2 wurden Seren von 18
Patienten mit muzinösen Karzinomen auf Reaktivität mit einem Peptid aus dem
MUC2-Kernprotein untersucht. Keines der untersuchten Tumorseren reagierte
spezifisch mit dem MUC2-Peptid.

Are mucinous and nonmucinous carcinomas two distinct groups with different
genetic lesions? To confirm this hypothesis the expression of MUC2 and of p53
was immunhistochemically investigated in 20 mucinous and nonmucinous
carcinomas of the colon, pancreas, breast and ovary. P53-Expression was
detectable in 39-64 per cent of the mucinous carcinomas of the colon,
pancreas, breast and ovary, and in 57-72 per cent of the nonmucinous
carcinomas of these organs. Thus, the mucinous carcinomas showed expression of
the p53 gene with a lower frequency than the nonmucinous carcinomas, but this
difference was significant only in the subgroup of colonic carcinomas. The
mucinous carcinomas of the colon, pancreas, breast and ovary showed MUC2
expression with a higher frequency than the nonmucinous carcinomas. This
difference was overall significant. In ovarian, pancreatic and breast tissue
where MUC2 is normally absent, the development of a mucinous carcinoma is
associated with de novo expression of MUC2. This shows, that the tumors of the
pancreas, ovary and colon classified upon their morphology as mucinous tumors
share similarities also on the molecular level. These characteristics
differentiate the mucinous carcinomas from the nonmucinous carcinomas but
without forming two discrete groups. Only the mucinous carcinomas of the
breast differ in 100 per cent from the nonmucinous carcinomas by de novo
expression of MUC2. An inverse correlation between expression of p53 and MUC2
as an indicator of possible interaction between p53 and MUC2 could be
excluded. To detect humoral immunoreactivity against overexpressed MUC2 18
sera of patients exhibiting mucinous carcinomas were investigated if they show
reactivity with a peptide out of the MUC2 core protein. None of the
investigated patient?s sera did react specifically with the MUC2 peptide.
