Thromboembolische Ereignisse treten bei Patienten mit DCM vermehrt auf und
haben einen erheblichen Einfluss auf die Prognose der Erkrankung. Aber auch
bei Patienten mit akutem Myokardinfarkt und erfolgter Koronarintervention ist
insbesondere nach Absetzen von periinterventionell verabreichten GP IIb/IIIa-
Rezeptorantagonisten das Thromboserisiko deutlich erhöht. Durch Gabe von ADP-
Inhibitoren soll nach Stent-Implantation bei Patienten mit KHK eine
Stentthrombose verhindern werden. In unserer Studie untersuchten wir,
inwiefern 1) eine erhöhte Plättchenaktivierung bei Patienten mit DCM zu finden
und dies mit einer Entzündung im Myokard assoziiert ist. 2) die Kombination
von einem Statin und Clopidogrel im Vergleich zu einer Clopidogrel-
Monotherapie die Plättchenfunktion verändert und 3) die Gabe von Abciximab
während der Koronarintervention die in vivo Thrombozytenaktivität und die
Agonist-induzierte Aggregabilität beeinflusst. Zur Messung der
Thrombozytenfunktion wurde den Patienten mit DCM und mit KHK venöses Blut
entnommen und die Expression von Thrombozytenoberflächenproteinen
durchflußzytometrisch quantifiziert. Die Agonist induzierte
Thrombozytenreaktivität wurde im Vollblut und an Thrombozytensuspensionen
bestimmt. Eine erhöhte Plättchenaktivierung zeigt sich im zirkulierenden Blut
von Patienten mit Kardiomyopathie verglichen mit gesunden Kontrollen. Die
verstärkte Thrombozytenaktivierung ist dabei mit dem Vorhandensein von
entzündlichen Infiltraten im Myokard der Patienten assoziiert. Eine
antithrombozytäre Therapie könnte hier eventuell von Nutzen. Eine Reduktion
der Plättchendegranulation und eine Senkung der Thrombozytenagglutination
erfolgt sowohl unter einer Monotherapie mit Clopidogrel als auch unter der
Kombination von Statinen und Clopidogrel. Die in einigen Studien beschriebenen
Interaktionen zwischen Atorvastatin und Clopidogrel resultieren nicht in einer
gestörten Plättchenhemmung. Abciximab reduziert die
Thrombozytenaggregationsfähigkeit deutlich, jedoch ist demgegenüber ein
signifikanter Anstieg der Thrombozytenaktivierung zu verzeichnen. Diese trägt
zu einer verstärkten Thrombogenität des Blutes bei und könnte den mit der
Beendigung einer Abciximabtherapie assoziierten Rebound-Effekt erklären.
Insgesamt wird in diesen Arbeiten die Bedeutung der Thrombozytenfunktion für
die Behandlung und Prognose kardiologischer Patienten verdeutlicht.

dc.description.abstract
Thrombotic events frequently occur in patients with cardiomyopathy and have an
important influence on the prognosis. Similarly the risk for thrombotic events
is clearly increased in patients with acute myocardial infarction undergoing
percutaneous coronary intervention particularly after stopping therapy with
the platelet inhibitor abciximab. Anti-platelet drugs such as clopidogrel
inhibit platelet reactivity and reduce the occurrence of stent thrombosis in
patients with coronary heart disease. These studies were performed to answer
whether 1) the platelet activation in blood is increased in patients with
cardiomyopathy compared to patients without myocardial disease, 2) the
combination of atorvastatin and clopidogrel is as effective as clopidogrel
alone in reducing platelet reactivity upon stimulation and 3) abciximab
affects in vivo platelet activation and the use of heparin in patients with
acute myocardial infarction (AMI) post PCI. To assess the platelet activity
venous blood was drawn and the platelet activation markers were flow
cytometrically measured. Whole blood platelet aggregation was tested by a
platelet function assay. Significantly more activated platelets were detected
in patients with cardiomyopathy than healthy controls. Platelet activation was
higher in patients with cardiomyopathy and myocardial infiltrates compared to
patients without myocardial inflammation. Measurement of platelet reactivity
may be useful to identify patients with cardiomyopathy at risk for thrombotic
events. The combination of clopidogrel with atorvastatin as well as
clopidogrel alone reduced the platelet degranulation and agglutination.
Atorvastatin does not result in a disturbed platelet inhibition, when given in
combination with clopidogrel. Abciximab reduced significantly the ability of
platelet to aggregate. However, a therapy with abciximab was associated with
increased in vivo platelet activation in the circulating blood of patients
with acute myoacardial infarction after assetion of abciximab infusion. This
may contribute to an increased thrombogenity of blood and may explain the
rebound effect associated with stopping abciximab therapy. Overall these
studies point the importance of platelet activation in the therapy and
prognosis of cardiovascular diseases.
