dc.description.abstract
Osteoporose ist die häufigste systemische Skeletterkrankung weltweit. Die WHO
definiert sie anhand der Knochendichtemessung per DXA (Duale
Röntgenabsorptiometrie). Diese misst die Flächenmineraldichte (BMD) in g/cm².
Diverse Studien zeigten jedoch, dass die Erkrankung Osteoporose nicht nur auf
einem Rückgang der Knochendichte, sondern auch auf einer Veränderung der
Mikroarchitektur beruht. BUA (Broadband Ultrasound Attenuation) und SOS (Speed
of Sound) als Parameter des Quantitativen Ultraschalls werden im Gegensatz zu
BMD möglicherweise durch qualitative Veränderungen des Knochens beeinflusst.
Ziel der Studie war es, an einem postmenopausalen Kollektiv von 481 weiblichen
Probandinnen (67,16 ±7,08 Jahre) zwei neue calcaneale Ultraschallgeräte (UBIS
5000 und DTU-one) auf ihre Korrelation untereinander und mit DXA zu
untersuchen, sowie ihren diskriminativen Wert hinsichtlich des Vorliegens
osteoporotischer Wirbelfrakturen zu erfassen. Die In-vivo-Kurzzeitpräzision
zeigt gute Werte für die beiden Parameter BUA und SOS (in CV SD% DTU-one:
BUA:1,534, SOS:0,098; UBIS: BUA:0,578, SOS:0,177). Das Gerät UBIS zeigt zudem
eine sehr gute In-vitro-Langzeitpräzision des Wertes BUA (0,17-0,2 CV SD%),
das Gerät DTU weist gute Ergebnisse für SOS (0,687 CV SD%) auf. BUA des DTU
ist nicht ausreichend (4,44 CV SD%). DTU und UBIS korrelieren moderat bis
stark miteinander (R=0,664 bis 0,814), eine Vergleichbarkeit der Daten im
klinischen Alltag ist jedoch nicht gegeben (Median: BUA in dB/MHz: 63,45 ±5,14
(UBIS) vs. 46,58 ±8,23 (DTU); SOS in m/s: 1497,48 ±27,45 (UBIS) vs. 1545,29
±10,6 (DTU)). Auch bei der Einteilung der Messergebnisse in ein oberes,
mittleres und unteres Drittel (Terzile) zeigt sich zwischen den beiden QUS-
Geräten lediglich eine schwache bis mittlere Übereinstimmung (Kappa-score
K=0,355-0,595). BUA und SOS korrelieren schwach bis moderat mit den DXA-
Werten, wobei die Korrelation mit dem proximalen Femur (UBIS BUA
R=0,424-0,504; UBIS SOS R=0,394-0,494; DTU BUA R=0,368-0,435; DTU SOS
R=0,420-0,480) durchweg höhere Ergebnisse als mit der Wirbelsäule (UBIS BUA
R=0,293-0,342; UBIS SOS R=0,287-0,362; DTU BUA R=0,242-0,313; DTU SOS
R=0,247-0,322) ergibt. Die Übereinstimmung der Einordnung der Messergebnisse
in ein oberes, mittleres und unteres Drittel durch QUS und total hip DXA ist
schwach bis sehr schwach (K=0,193-0,234). SOS beider Geräte und BUA des UBIS
5000 diskriminieren signifikant zwischen der Gruppe mit und ohne
osteoporotischer Wirbelfraktur (UBIS BUA: p=0,015; UBIS SOS: p=0,002; DTU SOS
p=0,001). Lediglich BUA des DTU-one (p=0,253) diskriminiert nicht zwischen den
beiden Probandinnengruppen. Aufgrund der geringen Unterscheidung der
Mittelwerte zwischen Probandinnen mit und ohne osteoporotische Wirbelfraktur
(jeweils ohne vs. mit osteoporotischer Wirbelfraktur: UBIS BUA 63,64 ±5,16
dB/MHz vs. 62,04 ±4,85 dB/MHz; UBIS SOS 1498,79 ±27,60 m/s vs. 1488,16 ±25,11
m/s; DTU SOS 1546,79 ±10,56 dB/MHz vs. 1542,37 ±9,96 dB/MHz) und der
Überlappung der Bereiche der Standardabweichungen, ergibt sich derzeit keine
Möglichkeit der Einordnung von Einzelpersonen in Wirbelfraktur-Risikobereiche.
Die individuelle Aussage der QUS-Messungen mit DTU-one und UBIS 5000 ist daher
stark eingeschränkt. BUA und SOS beider Geräte korrelieren mit dem Lebensalter
(R=�0,331 bis �0,377), ebenso korrelieren die BUA-Daten mit den
anthropometrischen Werten Körpergröße (0,136-0,162) und Gewicht (0,304-0,338).
UBIS BUA, UBIS SOS und DTU SOS korrelieren auf geringem Niveau mit der
Herzfrequenz (R=-0,99 bis -0,119). Eine Korrelation hinsichtlich der Jahre
seit Menopause (R=-0,250 bis -0,307) kann nur vor Korrektur auf Lebensalter
ermittelt werden.

dc.description.abstract
Osteoporosis is the most common systemic skeletal disease. It is defined by
the WHO by means of DXA (Dual X-ray Absorptiometry) measurements which
determine the bone mineral density. Several studies show that osteoporosis is
not only due to a decline in bone density, but also to a change of the
microstructure. BUA (Broadband Ultrasound Attenuation) and SOS (Speed of
Sound) as parameters of Quantitative Ultrasound are unlike DXA possibly
influenced by not only quantitative but also qualitative (structural) changes.
The aim of the study was to test two new calcaneal ultrasound devices (UBIS
5000 and DTU-one) on a postmenopausal population of 481 women (67,16 ±7,08
years). Their correlation respectively as well as with DXA was tested. Further
we investigated their discriminative ability regarding pre-existing vertebral
fractures. BUA and SOS show good in-vivo-short-term precision (CV SD% DTU-one:
BUA:1,534, SOS:0,098; UBIS: BUA:0,578, SOS:0,177). The UBIS 5000 displays good
results for the in-vitro-long-term precision of the parameter BUA (0,17-0,2 CV
SD%). The DTU-one reveals good results for SOS long-term precision (0,687 CV
SD%). The parameter BUA of the DTU-one does not produce sufficient results for
the in-vitro-long-term precision (4,44 CV SD%). BUA and SOS correlate
moderately to strongly with each other (R=0,664 to 0,814), but there is no
comparability of the two devices in clinical practice (mean: BUA in dB/MHz:
63,45 ±5,14 (UBIS) vs. 46,58 ±8,23 (DTU); SOS in m/s: 1497,48 ±27,45 (UBIS)
vs. 1545,29 ±10,6 (DTU)). When being partitioned into an upper, medium and
lower third, the results of the 2 ultrasound devices show only a weak to
medium concordance (Kappa-score K=0,355-0,595). BUA and SOS correlate weakly
to moderately with the DXA results, the correlation with the DXA measurements
of the proximal femur (UBIS BUA R=0,424-0,504; UBIS SOS R=0,394-0,494; DTU BUA
R=0,368-0,435; DTU SOS R=0,420-0,480) is higher than the correlation with the
ones of the lumbar spine (UBIS BUA R=0,293-0,342; UBIS SOS R=0,287-0,362; DTU
BUA R=0,242-0,313; DTU SOS R=0,247-0,322). The concordance when partitioning
the results of DXA and QUS in an upper, medium or lower third respectively is
weak to very weak (K=0,193-0,234). SOS of both devices as well as BUA of the
UBIS 5000 discriminate significantly between the group of women with and
without vertebral osteoporotic fractures (UBIS BUA: p=0,015; UBIS SOS:
p=0,002; DTU SOS p=0,001). Only BUA of the DTU-one does not discriminate
between the two groups (p=0,253). Due to the little difference between the
means of the QUS results of the two groups with and without vertebral
fractures (without vs. with osteoporotic vertebral fracture: UBIS BUA 63,64
±5,16 dB/MHz vs. 62,04 ±4,85 dB/MHz; UBIS SOS 1498,79 ±27,60 m/s vs. 1488,16
±25,11 m/s; DTU SOS 1546,79 ±10,56 dB/MHz vs. 1542,37 ±9,96 dB/MHz), there is
no possibility to classify individuals in high risk groups by means of
Quantitative Ultrasound. BUA and SOS correlate with age (R=-0,331 to -0,377),
BUA also correlates with the anthropometric values body height (0,136-0,162)
and weight (0,304-0,338). UBIS BUA, UBIS SOS and DTU SOS correlate on low
level with the heart rate (R=-0,99 bis -0,119).
