Es wurde der Zusammenhang zwischen Greifkraft, Knochendichte und biochemischen Parametern des Knochenstoffwechsels anhand von 503 Probanden der Berliner Altersstudie II untersucht. Derzeit konnte kein Zusammenhang zwischen Greifkraft und Knochendichte festgestellt werden. Eine Relation zwischen Greifkraft und biochemischen Parametern des Knochenstoffwechsels war ebenfalls nicht nachweisbar. Dies mindert die Aussicht, mittels kosteneffizienten Greifkraftmessungen verlässliche Einschätzungen des Osteoporoserisikos vornehmen zu können. Für Desoxypyridinolin, als auch für Osteocalcin konnten wir signifikante Korrelationen zwischen den biochemischen Markern und der Knochendichte feststellen. Desoxypyridinolin war bei Frauen, als auch bei Männern negativ mit der Knochendichte assoziiert. Osteocalcin korrelierte jedoch nur bei Frauen mit der Knochendichte, was für das Vorliegen einer High- Turnover-Osteoporose bei Frauen spricht. Die Ergebnisse stärken die Bedeutung von Osteocalcin und Desoxypyridinolin bei der Diagnostik der Osteoporose. Anhand von Knochendichtemessungen des Schenkelhalses, des gesamt proximalen Femurs und der lumbalen Wirbelsäule wurde eine Prävalenz der Osteoporose von 20% für Frauen und von 10% für Männer im Alter von ungefähr 70 Jahren festgestellt. Als wichtiges Ergebnis kann festgehalten werden, dass es große Defizite in der Diagnostik der Osteoporose gibt. So wurde bei den Teilnehmern, die von Osteoporose betroffen waren, die Erkrankung bei 66% der Frauen und 79% der Männer erstmals diagnostiziert. Alarmierend ist, dass nur 11% der an Osteoporose erkrankten Frauen und keiner der an Osteoporose erkrankten Männer eine antiresorptive Osteoporosetherapie in Form von Bisphosphonaten beziehungsweise selektiven Östrogenrezeptormodulatoren erhielten. Auch für Vitamin D und Calcium zeigte sich eine unzureichende Abdeckung. Von den 503 Probanden nahmen lediglich 7% der Frauen und 4% der Männer ein Vitamin D-Präparat zu sich. Calcium substituierten nur 8% der 318 Frauen und 5% der 185 Männer. Die erhobenen Daten belegen, dass Morbidität und Mortalität durch osteoporotische Frakturen derzeit noch im deutschen Gesundheitssystem unterschätzt werden. Es wird ausdrücklich darauf hingewiesen, dass eine frühzeitige und umfangreiche Diagnostik der von Osteoporose betroffen Altersgruppen mit Knochendichtemessungen dringlichst notwendig ist. Zusätzlich ist sowohl eine umfangreichere spezifische medikamentöse Therapie, als auch eine bessere Vorbeugung durch Vitamin D und Calcium erforderlich.
Based on 503 participants of the Berlin Aging Study II, we investigated the relation between grip strength, bone mineral density and bone biochemical markers. At present there was no significant association between grip strength and bone mineral density. No relation was observed between grip strength and bone biochemical markers. The findings diminish our hopes in using cost- effective grip measurements for the risk assessment of osteoporosis. We could observe significant correlations between bone mineral density and deoxypyridinoline as well as between bone mineral density and osteocalcin. Deoxypyridinoline was negatively related to bone mineral density in both men and women. Osteocalcin was associated to bone mineral density only in women. This finding indicates that women are affected by high-turnover-osteoporosis. The results underline the importance of osteocalcin and deoxypyridinoline in the assessment of osteoporosis. Using bone mineral density measurements of the femoral neck, the total hip and the lumbar spine we observed that 20% of the women and 10% of the men aged approximately 70 years were affected by osteoporosis. As an important result of the study big deficits in the diagnosis of osteoporosis could be shown. Of those individuals affected by osteoporosis 66% of the women and 79% of the men were diagnosed with osteoporosis for the first time. It is alarming that only 11% of the women and none of the men affected by osteoporosis received specific antiresorptive therapy in the form of bisphosphonates and selective estrogen receptor modulators. An inadequate coverage for calcium as well as vitamin D was also observed. Of the 503 participants only 7% of the women and 4 % of the men took a vitamin D preparation. Calcium was substituted by only 8% of the 318 women and 5 % of the 185 men. The collected data proof that morbidity and mortality due to osteoporotic fractures are still underestimated regarding the German health system at present. It is pointed out clearly that the age groups affected by osteoporosis urgently require extensive diagnostic measures in the form of bone mineral density measurements on early stage. In addition extensive specific medical therapies as well as a better prevention through vitamin D and calcium are necessary.
