Hintergrund: Die genaue Bestimmung der Entfernung zwischen dem führenden Teil
des kindlichen Kopfes und der Interspinalebene der Mutter ist entscheidend, um
bei einem Geburtsstillstand den Geburtsmodus festzulegen. Allerdings zeigt
sich, dass die bisherigen Methoden zur Höhenstandsdiagnose durch eine hohe
Fehlerrate gekennzeichnet sind. Daher muss die diagnostische Genauigkeit
verbessert werden. Hierzu wurde der Zusammenhang zwischen dem durch
Ultraschall gemessenen Angle of Progression und dem durch das offenen MRT
gemessenen Höhenstand der knöchernen Leitstelle bei Frauen am Termin
untersucht. Methoden: Die anatomischen Referenzstrukturen, die Spinae
ischiadicae, sind im Ultraschall nicht sichtbar. Deshalb musste der Abstand
der Leitstelle zur Interspinalebene im MRT gemessen werden. Es wurden 31
Schwangere ab einem Gestationsalter von < 37 SSW mit Schädellage
eingeschlossen. Zunächst wurde der Abstand zwischen dem führenden Teil des
kindlichen Schädels und der mütterlichen Interspinallinie im offenen MRT
gemessen. Direkt im Anschluss wurde der Angle of Progression mittels
translabialen Ultraschalls gemessen. Dabei wurde die Körperhaltung der
Patientin nicht verändert. Ergebnisse: Es konnte eine signifikante Korrelation
zwischen dem sonographisch gemessenen Angle of Progression und dem Abstand der
fetalen Leitstelle zur Interspinalebene errmittelt werden (y = - 0,51x + 60,8;
r2 = 0,38; p < 0,001). Es zeigt sich, dass ein Höhenstand auf Interspinal,
welcher dem Engagement des fetalen Kopfes entspricht, einem Angle of
Progression von 120° entspräche. Schlussfolgerung: Die vorliegende Studie
demonstriert die Korrelation zwischen dem Angle of Progression und dem Abstand
der Leitstelle zur Interspinalebene. Hierbei entspricht die Interspinalebene
einem Winkel von 120°. Allerdings stützt sich diese Korrelation bisher nur auf
statistischen Annahmen und muss in weiteren klinischen Studien validiert
werden.

Background: Precise determination of the distance between the leading part of
the fetal skull and the level of the maternal ischial spines is crucial when
intervention is required for a prolonged second stage of labor. However,
obtaining this information by clinical means has many shortcomings.
Transperineal ultrasound has been used to improve diagnostic accuracy. During
labor, transperineal sonography is increasingly used to evaluate fetal head
descent. The aim of this study was to compare the angle of progression
assessed by open magnetic resonance imaging and transperineal ultrasound.
Study Design: A total of 31 pregnant women at term (< 37 weeks), who were not
in labor, underwent MRI in an open 1.0 tesla system. A midsagittal plane of
the maternal pelvis was stored. Immediately afterwards, without changing the
suspine position, a transperineal ultrasound was performed. The angle of
progression was measured offline by transperineal ultrasound and MRI. Results:
The angles of progression measured by transperineal ultrasound (mean, 79,05
degrees; SD 11,44) and the distance from the presenting part to the ischial
spine plane in open MRI (mean, 80,48 degrees; SD 11,06) correlated
significantly (y = -0,51x + 60,8; r2 = 0,38; p < 0,001). The intraclass
correlation coefficient between the two methods was 0,89 (95% confidence
interval, 0,78-0,94). The X-intercept for the linear regression line showed
that station 0, which would correspond to fetal head engagement, is expected
to occur when the angle of progression reaches 120°. Our results should be
interpreted with caution as linear extrapolation is frequently subject to
uncertainties. Conclusion: The angle of progression measurements obtained by
transperineal ultrasound and open MRI correlated very well. The present study
demonstrated a predictable correlation between the angle of progression
obtained by transperineal ultrasound and the traditional scale to quantify
fetal head descent. Based on our results, station 0 would correspond to a 120°
angle of progression. However, this correlation is based on statistical
assumption only and has to be proven in future studies.
