Die Trockenblutanalyse der Acylcarnitine mittels Tandem-Massenspektrometrie
(MS/MS) ist ein für das Neugeborenenscreening etabliertes Verfahren zur
Diagnose von Fettsäurenoxidationsdefekten (FAO) und Organoazidopathien (OA).
Ziel dieser Arbeit war die Entwicklung und Evaluation einer neuen Methode zur
Bestimmung nicht derivatisierter Acylcarnitine und Acylglycine im Urin mittels
Fließinjektion und tandem-massenspektrometrischer Detektion (FIA-MS/MS). 614
Urinproben von gesunden Probanden (n = 359) sowie von Patienten mit MCAD-
Defekt (MCADD) (n = 98), Methylmalonazidurie (MMA) (n = 53), Propionazidämie
(PA) (n = 19), Isovalerianazidämie (IVA) (n = 19), Glutarazidurie Typ II (GA
II) (n = 15), Glutarazidurie Typ I (GA I) (n = 12), 3-Methylcrotonylglycinurie
(3-MCG) (n = 11), anderen FAO (n = 10), Tyrosinämie Typ I (Tyr I) (n = 10) und
Ahornsiruperkrankung (MSUD) (n = 8) wurden mit einer Fließinjektionsmethode
mittels Triple-Quadrupol Massenspektrometrie auf 37 Acylcarnitine und 17
Acylglycine untersucht. Die Stoffwechselerkrankung MCADD war mit 94%
Sensitivität und 93% Spezifität anhand folgender Marker des Acylcarnitin- und
Acylglycinprofils zu detektieren: P-C3G, C5OHG, C6G, C7G, C8DCG, C8OHG, C6, C8
und C10:1. Die MMA wurde mit 94% Sensitivität und 96% Spezifität (Marker: C3),
die PA mit 100% Sensitivität und 100% Spezifität (Marker: C3), die IVA mit
100% Sensitivität und 98% Spezifität (Marker: C5, C5G), die GA I mit 92%
Sensitvität und 94% Spezifität (Marker: C5DC) und die 3-MCG mit einer
Sensitivität und einer Spezifität von jeweils 100% (Marker: C5:1G, C5OHG,
C5OH, C8OHG) diagnostiziert. Andere charakteristische Marker waren: C4DC für
MMA, C5OHG, C0, C5:1G, C8OHG für PA und C8OHG für IVA, sowie C5:1, C7G und C5G
für 3-MCG. Für die Krankheitsgruppen GA II, andere FAO, Tyr I und MSUD ließen
sich keine charakteristischen Parameter evaluieren. Mit schneller
Probenaufarbeitung und hohem Probendurchsatz sind Acylcarnitine und
Acylglycine mittels FIA-MS/MS im Urin sicher detektierbar. Spezifische
Ausscheidungsmuster lassen mit hoher Sensitivität und Spezifität die Diagnose
bestimmter FAO und OA zu. Als zusätzliches Analyseverfahren in der
Routinediagnostik könnte diese Methode in Zukunft eine schnelle Konfirmation
von Verdachtsdiagnosen ermöglichen und zu mehr Sicherheit in der
Diagnosestellung beitragen.

The diagnosis of inborn errors of metabolism by measuring acylcarnitines in
dried blood spots by electrospray ionization tandem mass spectrometry (ESI-
MS/MS) is a well-established method which is used for newborn screening
programs. The aim of this study was the evaluation of a fast and reliable
method for confirming the diagnosis of fatty acid oxidation disorders (FAO)
and organic acidemias (OA) by determining not derivatized acylcarnitines and
acylglycines in urine with a new flow injection and tandem mass spectrometry
detection (FIA-MS/MS) method. We determined 37 acylcarnitines and 17
acylglycines in 614 urine samples from healthy individuals (n = 359), and in
255 samples from patients with FAO and OA: patients with medium-chain acyl-CoA
dehydrogenase deficiency (MCADD) (n=98), methylmalonic aciduria (MMA) (n =
53), propionic aciduria (PA) (n = 19), isovaleric aciduria (IVA) (n = 19),
glutaric aciduria type II (GA II) (n = 15), glutaric aciduria type I (GA I) (n
= 12) and 3-methylcrotonylglycinuria (3-MCG) (n = 11), other FAO (n = 10),
tyrosinemia type I (Tyr I) (n = 10) and maple syrup urine disease (MSUD) (n =
8). Age-related reference values for acylcarnitines and acylglycines were
defined in healthy individuals and disease-specific acylcarnitines and
acylglycines were evaluated. MCADD was identified with 94% sensitivity and 93%
specificity on the basis of the following acylcarnitine and acylglycine
profile: P-C3G, C5OHG, C6G, C7G, C8DCG, C8OHG, C6, C8 and C10:1. MMA was
diagnosed with 94% sensitivity and 96% specificity (metabolite: C3), PA with
100% sensitivity and 100% specificity (metabolite: C3), IVA with 100%
sensitivity and 98% specificity (metabolites: C5, C5G), GA I with 92%
sensitivity and 94% specificity (metabolite: C5DC) and 3-MCG with 100%
sensitivity and 100% specificity (metabolites: C5:1G, C5OHG, C5OH, C8OHG).
Other characteristic metabolites were: C4DC for MMA, C5OHG, C0, C5:1G and
C8OHG for PA, C8OHG for IVA as well as C5:1, C7G and C5G for 3-MCG. In GA II,
other FAO, Tyr I and MSUD no characteristic metabolites were identified. These
data show that determination of acylcarnitines and acylglycines in urine by
FIA-MS/MS is a reliable and fast method for the diagnosis of FAO and OA.
Therefore FIA-MS/MS may be established as a new routine method for the
confirmation of FAO and OA.
