Angiotensin II (Ang II) vermittelt seine Effekte im Wesentlichen über zwei
Rezeptor-Subtypen – den AT1- und den AT2-Rezeptor (AT1R bzw. AT2R). Für den
AT1R konnte bereits eindeutig gezeigt werden, dass dieser beispielsweise durch
die Aktivierung pro-inflammatorischer Zytokine entzündungsfördernde Effekte
von Ang II vermittelt. Im Gegensatz dazu geht man beim AT2R, unter anderem
durch die Induktion anti-inflammatorischer Effekte, von einer
gewebeprotektiven Wirkung aus. Dank der Synthese des ersten selektiven, nicht-
peptidischen und oral aktiven AT2R-Agonisten Compound 21 (C21) konnte in
dieser Arbeit erstmals durch direkte AT2R-Stimulation die anti-
inflammatorische Wirkung des AT2Rs in vitro als auch in vivo nachgewiesen
werden. So führte die Stimulation mit C21 zur Repression einer durch
Tumornekrosefaktor (TNF)alpha induzierten Interleukin (IL) 6, monocyte
chemoattractant protein (MCP)-1 und TNFalpha mRNA-Expression in dermalen
Fibroblasten sowie Nabelschnurendothelzellen. Der Zytokin inhibierende Effekt
von C21 konnte darüber hinaus in vivo in einem kutanen Entzündungsmodell in
Mäusen nach einer durch Bleomycin induzierten toxischen Inflammation
verifiziert werden. In normotensiven Wistar Ratten führte C21 nach einem
experimentell induzierten Myokardinfarkt zu einem signifikanten Rückgang der
IL1ß, IL2 und IL6 mRNA-Expression im Periinfarktgewebe sowie der MCP-1- und
Myeloperoxidase-Level im Plasma. Der anti-inflammatorische Effekt von C21
konnte unter Verwendung des spezifischen AT2R-Antagonisten PD123319
vollständig aufgehoben werden und war in AT2R defizienten Zellen nicht
detektierbar. Im Rahmen der Charakterisierung der an der AT2R vermittelten
Reduktion pro-inflammatorischer Mediatoren wie z.B. von IL6 konnten folgende
Signaltransduktionsmechanismen identifiziert werden a) die Aktivierung von
Protein-Phosphatasen, b) die Inhibition des Transkriptionsfaktors nuclear
factor kappa B (NFkappaB) sowie c) die Cytochrom P450-abhängige Epoxidation
von Arachidonsäure zu 11,12-Epoxyeicosatriensäure (EET). Im Zuge der
Untersuchungen konnte ebenfalls eine mit Hydrocortison vergleichbar effektive
Blockade der IL6-Promoteraktivität durch C21 beobachtet werden. Der
Arachidonsäuremetabolit 20-Hydroxyepoxyeicosatriensäure (20-HETE) wurde als
Second Messenger in der AT1R gekoppelten pro-inflammatorischen Signalkaskade
von Ang II identifiziert. Ferner konnte Poly (ADP-ribose) polymerase-1
(PARP-1) eine Rolle als upstream Regulator des AT2Rs nachgewiesen werden. Die
in dieser Arbeit erstmals beobachtete anti-inflammatorische Wirkung des AT2Rs
in vitro und in vivo mittels pharmakologischer Stimulation mit dem ersten oral
aktiven, nicht-peptidischen AT2R-Agonisten C21 lässt an eine zukünftige
therapeutische Anwendung bei pathologischen Ereignissen, welche die Reduktion
inflammatorischer Mediatoren wie beispielsweise von IL6 oder die Inhibition
von NFkappaB erfordern, denken.

dc.description.abstract
It is commonly known that the Angiotensin AT1-receptor (AT1R) mediates
inflammatory effects of Angiontensin II (Ang II), while the role of the
Angiotensin AT2-receptor (AT2R) in this context is less established. There are
some evidences about anti-inflammation. In 2004, the synthesis of the first
selective, non-peptide and orally active AT2R-agonist, Compound 21 (C21), has
been published. The present study is the first to show the anti-inflammatory
effect of direct AT2R-stimulation in vitro and in vivo. The stimulation with
C21 led to a significant reduction of tumor necrosis factor (TNF) alpha-
induced interleukin (IL)6, monocyte chemoattractant protein (MCP)-1 and
TNFalpha mRNA-expression in dermal fibroblasts and human umbilical vein
endothelial cells and inhibited a bleomycin-induced toxic cutaneous
inflammation in vivo. In normotensive Wistar rats C21 decreased the cardiac
IL1, IL2 and IL6 mRNA-expression as well as serum level of MCP-1 and
myeloperoxidase (MPO) after experimental induced myocardial infarction. The
anti-inflammatory effect of C21 was inhibited by the specific AT2R-antagonist
PD123319 and was absent in AT2R-deficient cells. Three signaling mechanisms
were revealed to play a crucial role in the AT2R-mediated reduction of pro-
inflammatory molecules a) the activation of protein-phosphatases, b) the
inhibition of the transcription factor nuclear factor kappa (NFkappa)B and c)
the cytochrome P450-dependent epoxitation of arachidonic acid to
epoxyeicosatrienoic acid (EET). Furthermore, C21 showed a with hydrocortisone
comparable inhibitory effect on TNFalpha-induced IL6-promoter activity. The
arachidonic acid metabolite 20-hydroxyeicosatetraenoic (20-HETE) was
identified as second messenger in the AT1R-mediated inflammation [P1].
Moreover, (ADP-ribose) polymerase-1 (PARP-1) was shown to act as an upstream
regulator of the AT2R. In sum, this study shows the anti-inflammatory effects
of direct AT2R-stimulation in vitro and in vivo by the orally active, non-
peptide AT2R-agonist C21. These data suggest that pharmacological stimulation
of the AT2R may be a future therapeutic option in pathophysiological events
requiring the decrease of IL6 or blockade of NFkappaB.
