dc.description.abstract
Der Einfluss einer hochintensiven Kurzzeitbelastung auf Gerinnung und
Fibrinolyse wurde unter besonderer Berücksichtigung der belastungsabhängigen
Veränderungen der Blutlaktatkonzentration (BLK), des pH-Wertes und des
Plasmavolumens untersucht. Fünfzehn männliche gesunde Freizeitsportler (28.3 ±
6.9 Jahre, 183.7 ± 5.7 cm, 82.4 ± 8.7 kg) absolvierten einen 30-s Wingate
Anaerobic Test (WAnT), eine spezielle Fahrradergometrie, bei der die
Energiebereitstellung zu ca. 80% anaerob ist. Venöse Blutabnahmen erfolgten
vor (T0) und direkt nach (T1) dem WAnT, sowie neun (T9) und 30 Minuten (T30)
nach Belastungsabbruch. Als Messgrössen der Gerinnung wurden die aktivierte
partielle Thromboplastinzeit (aPTT), die Prothrombinzeit (TPZ), die Aktivität
des Gerinnungsfaktors VIII (FaVIII), Fibrinogen und die Fibrinmonomere (FM)
sowie zur Beurteilung der Fibrinolyse der Gewebetyp Plasminogen Aktivator
(t-PA) und die D-Dimere bestimmt. Änderungen des Plasmavolumens wurden anhand
des Gesamteiweisses abgeschätzt. Die BLK-Bestimmungen erfolgten aus Kapillar-
und die pH-Messungen im venösen Blut. Der WAnT, mit einer durchschnittlichen
Leistung von 700.5 ± 60.8 W, führte zu einem signifikanten Anstieg der BLK
(max. BLK ca. 14 mmol/l ca. sechs Minuten nach Belastungsabbruch), einer
Abnahme des pH (T0 7.39 ± 0.02) um ca. 0.2 zu T1 und T2 (p<0.001) und einer
Hämokonzentration von ca. 10% (T1). Die Verkürzungen von aPTT (T0 29.36 ± 2.29
s, T1 22.98 ± 2.70 s, p<0.001; T9 22.55 ± 3.15 s, p<0.001; T30 23.08 ± 3.32 s,
p<0.01) und TPZ (T0 9.34 ± 0.63, T1 9.15 ± 0.62, p<0.01; T9 9.17 ± 0.59,
p<0.01) sowie der FaVIII-Anstieg (T0 127.9 ± 22.1; T1 255.0 ± 77.2%, p<0.001;
T9 292.7 ± 90.3%, p<0.001) zeigen eine Aktivierung des intrinsischen und
extrinsischen Gerinnungssystems mit Schwerpunkt im intrinsischen System an.
Der Anstieg von Fibrinogen (ca. 12 %, p<0.001) und speziell der der FM (n=12,
T0 1.52 ± 2.01 µg/ml; T1 ca. 1600%, p<0.001) unterstreicht die
Gerinnungsaktivierung und spricht für eine Thrombinwirkung und Fibrinbildung.
Der Anstieg von t-PA (T0 3.41 ± 2.72 ng/ml; T1 12.31 ± 8.46 ng/ml, p<0.001; T9
10.88 ± 10.27 ng/ml, p<0.001; T30 7.86 ± 8.90 ng/ml, p<0.01) und D-Dimeren
(p<0.05) spricht für eine aktivierte und effektive Fibrinolyse. Die
Hämokonzentration konnte bei keiner der hämostatischen Messgrössen und die BLK
nur bei den FM (T9 52%, T30 46%) zur Erklärung der Varianz beitragen. Hingegen
wurden zu T1 bei der aPTT 66%, beim FaVIII 46% und beim t-PA 43%, zu T9 bei
der aPTT 37% und beim t-PA 32% sowie zu T30 beim t-PA 33% der Varianz durch
den pH erklärt. Die Ergebnisse zeigen, dass hochintensive Kurzzeitbelastungen
zu deutlichen Aktivierungen im Hämostasesystem führen. Während des gesamten
Beobachtungszeitraums scheint der aktivierten Gerinnung eine adäquat
gesteigerte Fibrinolyse gegenüberzustehen. Der stoffwechselbedingten pH-
Veränderung kommt als Ursache für die Aktivierungen grosse Bedeutung zu.

dc.description.abstract
Aims of the presented study were to investigate the effect of predominantly
anaerobic exercise on coagulation and fibrinolysis and to assess the
importance of singular factors which are related to anaerobic metabolism and
supposed to cause exercise-induced activation in the hemostatic system.
Fifteen healthy males (28.3 ± 6.9 years, 183.7 ± 5.7 cm, 82.4 ± 8.7 kg)
performed a 30-s Wingate Anaerobic Test (WAnT), a specific cycle ergometer
test which requires approximately 80% anaerobic energy. Venous blood samples
were obtained before (T0) and after (T1) the WAnT as well as nine (T9) and 30
minutes (T30) after termination of the test. To get information about the
coagulation system the activated partial thromboplastin time (aPTT),
prothrombin time (TPZ), activity of the clotting factor VIII (FaVIII),
fibrinogen and fibrinmonomers (FM) and to evaluate fibrinolysis the tissue-
type plasminogen activator (t-PA) and D-dimers were measured. Additional
measurements included total protein for calculation of plasma volume changes,
the pH-value and in capillary blood the blood lactate concentration (BLK). The
30-s WAnT (Mean-Power 700.5 ± 60.8 W) led to an increase in BLK (maximum BLK
app. 14 mmol/l about six minutes after the WAnT), a decrease in pH (T0 7.39 ±
0.02) by 0.2 (T1, p<0.001) and hemoconcentration of about 10% (T1). Shortening
of aPTT (T0 29.36 ± 2.29 s, T1 22.98 ± 2.70 s, p<0.001; T9 22.55 ± 3.15 s,
p<0.001; T30 23.08 ± 3.32 s, p<0.01) and TPZ (T0 9.34 ± 0.63, T1 9.15 ± 0.62,
p<0.01; T9 9.17 ± 0.59, p<0.01) demonstrate activation in both the intrinsic
and extrinsic system. The distinct reaction of aPTT and the clear increase in
FaVIII by about 100% (T1 255.0 ± 77.2%, p<0.001; T9 292.7 ± 90.3%, p<0.001)
seems to suggest an emphasis in the intrinsic part of the clotting cascade.
Fibrinogen (T0 232.7 ± 28.3 mg/dl) increased (T1 12%, p<0.001). The increase
in FM (n=12, T0 1.52 ± 2.01 µg/ml; T1 1600%, p<0.001) underlines the
activation of coagulation and serves as an indicator for thrombin- and fibrin
formation. Activation of fibrinolysis is demonstrated by an increase of t-PA
(T0 3.41 ± 2.72 ng/ml; T1 12.31 ± 8.46 ng/ml, p<0.001; T9 10.88 ± 10.27 ng/ml,
p<0.001; T30 7.86 ± 8.90 ng/ml, p<0.01) and D-dimers (p<0.05).
Hemoconcentration did not explain the variance of any of the hemostatic
variables, BLK contributed to the explanation of the FM response (T9 52%, T30
46%). The pH contributed to the explanation of the variance of aPTT (T1 66%,
T9 37%), FaVIII (T1 46%) and t-PA (T1 43%, T9 32%, T30 33%). The results
demonstrate a marked activation of the hemostatic system due to high-intensity
short-time exercise. During the whole observation time an adequately activated
fibrinolysis counterbalanced the activated coagulation. The metabolism related
pH-decrease seems to play a key-role for this activation.
