Einleitung Die Schwere der Sepsis und ihre zunehmende Inzidenz erfordert
Diagnostik, die einen schnellen Erregernachweis liefert, um frühzeitig eine
effektive Therapie zu initiieren. Ziel dieser randomisiert kontrollierten
klinischen Studie (RCT) ist der Vergleich initialer Diagnostiken bei
Intensivpatienten mit pulmogener oder abdomineller Sepsis und die Darstellung
der Zeitersparnis sowie Therapiemodifikation durch Nutzung des auf multiplex
PCR basierten Lightcycler® SeptiFast Tests (Roche Diagnostics GmbH, Mannheim,
Deutschland) gegenüber der Standarddiagnostik mit Blutkulturen. Methoden Der
RCT wurde von August 2010 bis März 2012 auf sechs beteiligten
Intensivstationen der Charité Universitätsmedizin Berlin durchgeführt.
Eingeschlossene Patienten erhielten parallel zur Blutkulturdiagnostik die
Abnahme von 1,5ml EDTA-Blut für den SeptiFast-Test. In die Interventionsgruppe
randomisierte Patienten erhielten die zusätzliche PCR Diagnostik, deren
Ergebnis telefonisch an die Intensivstation übermittelt wurde. Der RCT wurde
mit der Studiennummer ISRCTN 70694559 registriert. Ergebnisse Es wurden 78
Patienten eingeschlossen, davon 37 in die Kontroll- und 41 in die
Interventionsgruppe. Die Basischarakteristika beider waren vergleichbar. In
der Interventionsgruppe betrug die durchschnittliche Zeitdauer von
Probengewinnung bis Resultatübermittlung 15,9 Stunden. Die PCR konnte in 24,4%
der Fälle einen Erreger detektieren. Die Ergebnisse der Kontrollgruppe
erschienen durchschnittlich 22 Stunden später (p<0.001). Eine
Therapiemodifikation nach positivem SeptiFast-Resultat erfolgte in 40% der
Fälle (zwei invasive Mykosen, einmal Staphylococcus aureus, einmal Pseudomonas
aeruginosa). Die dafür benötigte Zeit ab initialer Diagnostik betrug in der
Interventionsgruppe durchschnittlich 18 Stunden und in der Kontrollgruppe 38
Stunden. Schlussfolgerung Der Lightcycler® SeptiFast-Test zeigte gegenüber der
Blutkulturdiagnostik eine signifikante Zeitersparnis von Probengewinnung bis
Resultatübermittlung mit Einfluss auf die Therapiesteuerung. Molekulare PCR-
basierte Methoden scheinen eine nützliche zusätzliche Diagnostikoption
darzustellen, insbesondere bezüglich invasiver Mykosen.

dc.description.abstract
Introduction The severity of sepsis and its increasing incidence requires a
fast diagnostic method in order to initiate early and effective therapy. This
RCT aims to compare the multiplex PCR Lightcycler® SeptiFast test (Roche
Diagnostics GmbH, Germany) with the standard blood culture diagnostic in ICU
patients with abdominal and pulmonary sepsis demonstrating considerable time
saving which could possibly lead to therapeutic modifications in the
subsequent treatment. Methods This randomized controlled trial, run from
August 2010 until March 2012, took place in six ICUs at the Charité University
hospital, Berlin. Parallel to the standard blood culture diagnostic, an
additional 1.5ml blood sample was taken from participating patients for the
SeptiFast PCR. Patients randomized into the intervention group were
additionally exposed to the multiplex PCR diagnostic with results immediately
passed on by telephone. This RCT was authorized and registered (ISRCTN
70694559). Results Altogether, a total of 78 patients participated in this
study, 37 were assigned to the control group (CG) and the other 41
participants randomized to the intervention group (IG). There was no
significant difference in basic characteristics between both groups. In the
IG, pathogens were detected by PCR in 24.4% of the cases with an average
duration of 15.9 hours from the initial blood sampling to the final
transmission of PCR-results to the corresponding ICU. By contrast, the CG
received their test results with an average of 22 additional hours (p<0.001).
Therapy modification was performed in 40% of the positive cases (two invasive
mycosis, one finding of Staphylcoccus aureus and one of Pseudomonas
aeruginosa).The mean time required for this alteration amounted 18 hours for
the IG and 38 hours for the CG, measured from the initial diagnostic test.
Conclusion This RCT showed that utilizing the Lightcycler® SeptiFast – Test
compared to a standard blood culture diagnostic can significantly reduce the
timespan from the initial blood sampling to the final result transmission
impacting therapy management. Molecular PCR-based methods thus appear to be a
useful and beneficial diagnostic test, especially for cases of suspected
mycosis.
