Einleitung: Der Goldstandard der Therapie terminaler Lebererkrankungen ist die Lebertransplantation. Nonalcoholic steatohepatitis (NASH) nimmt als Ursache für eine Lebertransplantation in der westlichen Welt zu und kann im Transplantat rezidivieren. Ziel dieser Arbeit war die Evaluierung von langfristigen histologischen Veränderungen in Lebertransplantaten und die Korrelation dieser Veränderungen mit laborchemischen und klinischen Parametern, insbesondere dem Körpergewicht der Empfänger. Methodik: Es wurden die Leberbiopsiebefunde von 414 Patienten, die zwischen 2005 und 2011 in der Charité ein Lebertransplantat erhielten, retrospektiv ausgewertet. Neben klinischen und laborchemischen Parametern wurden die vier histologischen Merkmale Fibrose, Steatose, Inflammation und Hämosiderose evaluiert und statistisch untersucht. Insgesamt konnten 1494 Leberbiopsien eingeschlossen werden. Ergebnisse: Die Prävalenz des hepatozellulären Karzinoms (HCC) korrelierte mit dem Body-Mass-Index (BMI) der Patienten: In der Gruppe der Adipösen hatten 54,1% bis zum Zeitpunkt der Transplantation ein HCC entwickelt, in der Gruppe der Normalgewichtigen waren es nur 39,1% (p<0,01). Nach der Transplantation kam es zu einer signifikanten Gewichtszunahme: Nach einem Jahr waren 87 (34,4%) Patienten übergewichtig und 52 (20,6%) adipös, nach 5 Jahren waren es 122 (38,5%) beziehungsweise 94 (29,7%) Patienten (p<0,01). Männer hatten insgesamt einen signifikant höheren BMI als Frauen. Nach einem Jahr waren 114 Patienten an Diabetes mellitus erkrankt, nach 5 Jahren 137 Patienten (p<0,01). Der BMI korrelierte mit dem Verfettungsgrad des Transplantats. Die Inflammation im Organ war ein signifikanter Vorbote der Fibrosierung, die in ihrer Prävalenz über die ersten 5 Jahre zunahm (p<0,01). Jedoch konnten sich manche Patienten mit höhergradiger Inflammation oder Fibrose verbessern. Zwischen Steatose und Fibrose bestanden keine signifikanten Zusammenhänge. Diabetes mellitus, Triglyceride und die Nüchternglukose waren signifikant mit der Leberverfettung assoziiert (p=0,02; p=0,03; p<0,05). Schlussfolgerung: Rezidive und de-novo nonalcoholic fatty liver disease (NAFLD) sind mit einem hohen BMI sowie einer starken postoperativen Gewichtszunahme verknüpft. Im Transplantat führen ähnliche Prozesse wie im nativen Organ zu Steatose und NAFLD. Diabetes mellitus muss als eigenständiger Risikofaktor beachtet werden, der unabhängig vom BMI eine Verfettung und Inflammation der transplantierten Leber bedingt. Beide Bestandteile des metabolische Syndrom müssen somit als gefährliche Komplikation für einen Lebertransplantationspatienten angesehen werden.
Background: The gold standard therapy of terminal liver disease is liver transplantation. Nonalcoholic steatohepatitis (NASH) is becoming the leading cause for liver transplantation in the western world and is able to recidivate in the transplant. The aim of this work was the long term retrospective evaluation of histological changes in liver transplants and the correlation of these changes with clinical and laboratory parameters. Special attention should be paid to the body weight. Methods: The findings of the liver biopsies of 414 patients who received a liver transplant at the Charité between 2005 and 2011 were evaluated. Besides clinical and laboratory parameters the four histological categories fibrosis, steatosis, inflammation and hemosiderosis were statistically explored. A total of 1494 liver biopsies were included. Results: The prevalence of the hepatocellular carcinoma (HCC) correlated with the body mass index (BMI) of the patients: 54.1% of the obese patients had developed HCC before the transplantation took place, but only 39.1% of the normal weighted patients (p <0.01). After the transplantation, significant weight gain occurred: at the one-year checkpoint, 87 (34,4%) patients were overweighed and 52 (20,6%) obese; after 5 years, 122 (38,5%) and 94 (29,7%) patients, respectively (p <0.01). Men showed higher BMI than women overall. 114 patients showed diabetes mellitus after one year. This number increased to 137 patients after 5 years (p<0.01). The BMI correlated with the degree of steatosis of the graft. Inflammation was a precursor of fibrosis. Prevalence of fibrosis increased over the first 5 years (p<0.01). But some patients with a higher initial degree of inflammation or fibrosis were able to improve. There were no significant correlations between fatty degeneration and fibrosis. Diabetes mellitus, triglycerides and fasting glucose were significantly associated with fatty liver disease (p=0,02; p=0,03; p<0,05). Conclusion: Relapse of and de-novo nonalcoholic fatty liver disease (NAFLD) are connected to high BMI and postoperative weight gain. The same processes as in the original organ lead to steatosis and NAFLD in the graft. Diabetes mellitus must be considered a separate risk factor which causes fatty degeneration and inflammation in the transplanted liver regardless of the BMI. Overall, both components of the metabolic syndrome must be considered a dangerous complication for each patient receiving liver transplantation.
