Dennoch bleibt das Gesamtüberleben unter der Gemcitabintherapie mit 5-9 Monaten enttäuschend. Diese Studie untersucht die Fragestellung ob eine Kombinationstherapie mit Gemcitabin/5-Fluorouracil als 24h-Infusion und Folinsäure (GFF) einer Monotherapie mit Gemcitabin (GEM) hinsichtlich des Gesamtüberlebens, bei weiterhin guter Verträglichkeit, überlegen ist. Die Kombination aus Gemcitabin 1000 mg/m2 und 5-FU 750 mg/m2 als 24–stündige Dauerinfusion, die durch Folinsäure 200 mg/m2 moduliert wurde und an den Tagen 1, 8, 15, 22 eines 6-wöchigen Zyklus verabreicht wurde, wurde hier in einer randomisierten Phase III gegen die Gemcitabin Standardtherapie 1000 mg/m2 initial wöchentlich 7q8w, dann 3q4w, geprüft. Das primäre Zielkriterium war das Überleben, welches Anhand der Einjahres-Überlebensrate und der medianen Überlebenszeit überprüft wurde. Als sekundäre Zielparameter wurde sowohl das progressionsfreie Überleben als auch die Toxizität der beiden Behandlungen erhoben. Die Randomisierung wurde stratifiziert nach Tumorstadium (III, IVa und IVb) und Karnofsky-Performance Status ( 60-80% vs. 90-100%). Ergebnisse: Zwischen August 2000 und Juli 2002 wurden 304 chemonaive Patienten eingeschlossen. 144 Patienten wurden in den Kombinationsarm randomisiert, 160 Patienten in den Monotherapiearm. Die Patientencharakteristika waren ausgeglichen. 16 Patienten konnten bezüglich des Überlebens nicht ausgewertet werden. Es gab keine Unterschiede in der Einjahres-Überlebensrate (GFF 25% vs. GEM 20%) als auch in der medianen Überlebenszeit (GFF 6,5 Monate vs. GEM 6,3 Monate, p=0,42). Im Kombinationsarm war das progressionsfreie Überleben signifikant verlängert gegenüber der Monotherapie (GFF 3,7 Monate vs. GEM 3,1 Monate, p=0,047), allerdings konnte sich das Ergebnis nicht im Gesamtüberleben wiederspiegeln. Patienten mit einem KPS>/=90% zeigten Hinweise für ein verlängertes Überleben unter der Kombinationstherapie ( GFF 10,3 Monate vs. GEM 7,8 Monate), wohingegen die Patienten mit einem KPS<90% eher von der Monotherapie profitiert haben (GFF 3,8 Monate vs. GEM 4,9 Monate). Die Toxizitäten waren in beiden Armen gering, jedoch war unter der Kombinationstherapie ein höherer Prozentsatz im Sinne einer Myelosuppression III/IV zu verzeichnen. Schlussfolgerung: Die Therapieintensivierung mit der Kombination GFF konnte trotz guter Verträglichkeit nicht zu einer Verbesserung des Überlebens von Patienten mit fortgeschrittenem Pankreaskarzinom führen.","Single-agent Gemcitabine is standard treatment for advanced and metastatic cancer of the pancreas. But with a median survival of 5-9 months the therapy of advanced disease with gemcitabine alone remains still unsatisfactory. This study examines the question wether a combination with gemcitabine, 5-FU and Folinic acid can further improve overall survial and one-year survival rates. Patients were randomly assigned to receive either gemcitabine 1000 mg/m2 followed by folinic acid 200 mg/m2, 5-FU 750 mg/m2 administered as a 24-hour continuous infusion on days 1, 8, 15, 22 of a 6 week cycle (GFF) or gemcitabine alone at a dose of 1000 mg/m2 on days 1,8,15,22,29,36 and 43 of an initial 8-week schedule, followed by a 4-week schedule with therapy on days 1,8 and 15 (GEM). The primary endpoint was overall survival and the one-year survival rate; secondary endpoints were progression-free survival and toxicity. Results: From August 2000 until July 2002 304 chemonaive patients with advanced, inoperable pancreatic cancer were enrolled into the study, 144 pat. into the combination arm (GFF), 160 pat. into the standard treatment arm (GEM). The baseline characteristics were well balanced between treatment arms.Combination treatment was associated with a prolonged median progression free survival (3,7 months vs. 3,1 months, p=0,047), but median overall survival as well 1 year survival did not reach statistical significance. The median survival time was 6,5 months for combination and 6,3 for gemcitabine alone (p=0,42), 1 year survival was 25% for GFF and 20% for GEM. There were no high toxicity. Patients with a good KPS>/=90% had a survival benefit when treated with the combination ( GFF 10,3 months vs. GEM 7,8 months). By contrast patients with a poor KPS<90% did not benefit more from the combination as they did from monotherapy (GFF 3,8 months vs. GEM 4,9 months).
