Andreas Schapowal","n","2008-04-11","2018-06-07T22:02:01Z","2008-04-21T00:00:00.649Z","2008-06-13","2008","Gesamtdissertation","Aus klinischen Fallberichten über eine markante Immunglobulin-E-Spiegel senkende Wirkung von Cefadroxil im Zusammenhang mit der therapeutischen Cefadroxil-Applikation bei superinfizierten atopischen Krankheitsbildern lässt sich der Schluss ziehen, dass Cefadroxil über seine antibiotische Wirkung hinaus in die Regulation der Synthese des zentralen Vermittlers der allergischen Sofortreaktion, dem Immunglobulin E, eingreift. In-vitro- Untersuchungen zeigen die Einflussmechanismen über eine Stimulation der Prostaglandin E2-Synthese und damit einer vermehrten Aktivierung von T-Supressorzellen auf. Des weiteren stimuliert Cefadroxil eine vermehrte Interferon-g-Sekretion und hemmt die IL-4 abhängige CD23-Expression. Betrachtungen zu den verschiedenen Regulationsmechanismen der T-Zellpopulationen, des g-Interferon und des niedrigaffinen IgE-Rezeptors CD23 in der IgE-Synthese machen die oben genannten klinischen und experimentellen Beobachtungen plausibel. Dieser IgE-senkende Effekt des Cefadroxils lässt sich jedoch in unserem gewählten Studiendesign an klinisch gesunden Probanden unter einer postoperativen Antibioseprophylaxe mit Cefadroxil weder im Pricktest noch in den Messungen der IgE-Spiegel statistisch nachweisen. Eine Betrachtung der Erkenntnisse über die immunmodulatorische Wirkung von Viren und Bakterien, die in den o. g. klinischen Beobachtungen zugrunde lagen, zeigt einen erheblichen immunmodulatorischen Einfluss im Sinne einer Stimulation der IgE- Synthese sowohl spezifscher, gegen den Erreger selbst und erregerunabhängiger Antigene, wie z.B. die in dieser Studie untersuchten Inhalationsallergene, als auch unspezischer Immunglobuline. Besonders solide Daten liegen hier für Staphylococcus aureus und Respiratory Syncytial Virus vor. Aber auch Beschreibungen anderer Erreger, wie Parainfluenzaviren, Zytomegalie-Virus, Hepatitis-Vakzine und Bordetella pertussis, weisen auf eine derartige Co- Stimmulation der IgE-Synthese hin. Aus diesen Überlegungen heraus ergibt sich für das Cefadroxil ein synergistischer Effekt auf die Regulation der IgE- Synthese; zum einen aus der Senkung der erregerinduzierten vermehrten IgE- Synthese, einfach durch deren Reduktion, und zum anderen aus einem eigenen immunmodulatorischen Einfluss über die Stimulation von T-Suppressorzellen und die Veränderung der CD23- und g-INF-Expression. Dieser synergistische Effekt durch die multilaterale Immunmodulation fehlt bei klinisch gesunden Patienten ohne Infektion und lässt sich daher in unserer Studie nicht nachweisen. Die immer mehr zu Tage tretende Erkenntnis über die immunmodulatorische Wirkung, auch anderer Antibiotika, wird in Zukunft die Erkenntnisse der eigentlichen Antibiotikawirkung über deren direkten Einfluss auf die Erreger hinaus und deren Stellung in der Behandlung von atopieassoziierten Infektionen verändern. Inwieweit sich solche synergistischen Effekte für das Cefadroxil nachweisen lassen und inwieweit sich daraus neue therapeutische Ansätze ergeben, bleibt zunächst der weiteren Forschung überlassen.","Clinical case reports on a significant reduction of Immunglobulin-E-levels of cefadroxil in connection with the application of cefadroxil in superinfected atopical deseases allow the conclusion that cefadroxil has not only a antibiotic effect but in addition to it it effects the regulation of the synthesis of IgE which is the central agent in the immediate hypersensitivity. In-vitro-studies show that the effector of this Inhibition is the stimulation of the prostaglandin-E2-synthesis inducing an increased activity of T-supressing cells. Further more is cefadroxil stimulating the secretion of g-interferron and inhibiting the IL-4 dependent expression of CD23. Considerations on the population of T-cells, interferron-g and the minor affinit receptor for CD23 and their role in the regulation of the Ige- synthesis make these clinical and experimental findings reasonable. In the present study design the IgE lowering effects of cefadroxil on clinical healthy individuals in a postoperativly situation can not be confirmed neither in the prick testing nor in the IgE blood level. Considerations on the immune-modulating effects of different viruses and bacteria which were causative in the clinical case reports show a significant immune-modulating impact in the sense of stimulating the IgE-synthesis of specific antigens of the exciter as well as exciter independent antigens as those inhalativ antigens which were used in our study. Solid data are available especially for staphylococcus aureus and respiratory syncytical virus. Descriptions of other pathogenic germs as parainfluenca virus, cytomegaly virus hepatitis vaccine and bordutella pertussis also give the indication of a co-stimulating impact on the IgE-synthesis. Due to these considerations there seems to be a synergistic effect on the regulation of the IgE-synthesis using cefadroxil. On one hand by the decrease of the pathogen induced IgE-synthesis via simply reducing the number of germs and on the other by its own immune-modulating effects via the stimulation of T-supressing cells and the alteration of the CD23- and g-INF-expression. This synergistic impact through the multilateral immunmodulation is therefore missing in our study on clinical healthy patients without any acute infection. These insights on a immunmodulating effect of also other antibiotics will in the future change our knowledge on the impact of antibiotics beyond their original direct effect on the pathogen germ and their role in the treatment of allergy associated infections.
