Bekannt ist zudem, dass die Inzidenz der bilateralen Leistenhernie mit steigendem Patientenalter zunimmt. Vor diesem Hintergrund stellt sich die Frage, ob für die bilaterale TAPP ähnlich gute Ergebnisse in der Breite der chirurgischen Leistenhernienversorgung zu erzielen sind. Außerdem ist unklar, ob eine Indikation zur prophylaktischen, bilateralen TAPP-Versorgung bei Patienten mit unilateraler Leistenhernie gegeben ist. Methoden: Zwischen September 2009 und April 2013 wurden 15 176 Patienten der multizentrischen Qualitätssicherungsstudie Herniamed in TAPP- Technik operiert. Davon wurden 10 887 Patienten unilateral (71,7%), 4 289 Patienten bilateral (28,3%) versorgt und im Hinblick auf perioperative Komplikationen verglichen. Die univariablen Analysen wurden unter Anwendung des exakten Tests nach Fisher und des robusten t-Test durchgeführt. Die Ergebnisse der univariablen Analyse wurden durch multivariable Analysen überprüft. Ergebnisse: Die Rate postoperativer Komplikationen lag mit 4,9% für die bilaterale TAPP signifikant höher als in der unilateralen Vergleichsgruppe (p=0,009), für welche sich eine Rate von 3,9% darstellen ließ. Komplikationen führten in 0,9% der unilateralen und 1,9% der bilateralen TAPP-Operationen zur Reoperation, woraus sich eine signifikant höhere Revisionsrate im bilateralen TAPP-Kollektiv ergab (p<0,001). Die multivariable Analyse bestätigte den signifikanten Einfluss der bilateralen TAPP auf postoperative Komplikationen (p=0,0382; OR=1,20 [95%-KI: 1,01; 1,42]) und Reoperationen (p<0,0001; OR=2,13 [95%-KI: 1,58: 2,86]). Schlussfolgerung: Die sehr guten Ergebnisse der uni- und bilateralen TAPP eines high-volume Zentrums lassen sich mit den Daten des Herniamed Registers nicht reproduzieren. Die bilaterale TAPP ist im Vergleich zum unilateralen Vorgehen mit einer signifikant höheren postoperativen Komplikations- und Reoperationsrate verknüpft. In addition, the incidence of bilateral inguinal hernia increases with higher age. Based on these findings, questions arise whether a comparable outcome can be achieved on a larger scale. Furthermore, a prophylactic operation of the contralateral side at the time of unilateral hernia repair is an approach necessitating investigation. Methods: Between September 2009 and April 2013 15 176 patients from the Herniamed Registry underwent TAPP operation. Of these patients, 10 887 have been operated on because of unilateral (71.7%) and 4 289 because of bilateral (28.3%) inguinal hernia. These groups were then compared in respect of perioperative complication rates. To identify differences between the groups in univariate analysis, Fisher`s exact test and the t-test were performed. The results of univariate analysis were confirmed through multivariate analyses. Results: A significant difference was noted in the rate of postoperative complications, which was 4.9% for bilateral compared with 3.9% for unilateral inguinal hernia (p=0.009). Complications necessitated reoperation in 0.9% of patients after unilateral and in 1.9% of patients after bilateral inguinal hernia repair, thus attesting to the significantly higher risk presented by bilateral inguinal hernia repair (p<0.001). Multivariate analysis confirmed the significant influence of bilateral TAPP on increased postoperative complication (p=0.0382; OR=1.20 [95%-CI: 1.01; 1.42]) and reoperation rates (p<0.0001; OR=2.13 [95%-CI: 1.58; 2.86]). Conclusion: Data from the Herniamed Registry show that the excellent perioperative outcome of a high-volume center can neither be reproduced for unilateral, nor for bilateral inguinal hernia repair in TAPP technique. Bilateral TAPP operation demonstrates significantly worse postoperative complication and reoperation rates compared with unilateral TAPP.
