Von den untersuchten Patienten wurden 872 Patienten konventionell und 852 Patienten laparoskopisch elektiv koloreseziert. Die Gesamtkomplikationsrate allgemeiner postoperativer Komplikationen betrug insgesamt 9,1%. Die Gesamtkomplikationsrate chirurgischer Komplikationen lag bei 12,8%. Hierbei kam es zu signifikanten Unterschieden innerhalb der beiden untersuchten Gruppen. Es konnte festgestellt werden, dass es zu signifikanten Unterschieden in den demographischen, epidemiologischen Daten bezüglich der Altersverteilung, der Nebenerkrankungen sowie bei den Operationsindikationen kam. Multimorbide, ältere Patienten mit Malignom wurden eher offen chirurgisch versorgt. Demgegenüber wurden jüngere Patienten mit geringerer Morbidität und benigner Operationsindikation laparoskopisch versorgt. Einflüsse für das Auftreten allgemeiner postoperativer Komplikationen waren das Alter (Odds-Ratio OR: 2,51 [95% Konfidenzintervall 95%-KI: 1,22-5,19]; p= <0,0001), die konventionelle vs. laparoskopische Operationstechnik (OR: 0,62 [95%-KI:0,43-0,90];p=0,01) sowie die ASA-Klassifikation I-II vs. III-IV (OR: 1,64 [95%-KI:1,15-2,35];p=0,0070). Einflüsse für das Auftreten postoperativer lokaler Komplikationen waren Alter (OR: 1,78 [95%-KI:0,94-3,38];p=0,01), konventionelle Operationstechnik vs. laparoskopischer Operationstechnik (OR: 0,65 [95%-KI:0,48-0,89];p=0,0071), die ASA-Klassifikation I-II vs. III-IV (OR: 1,40 [95%-KI:1,01-1,92];p=0,04) sowie erhöhter BMI (OR: 2,88 [95%-KI:1,25- 6,62];p=0,02). Vergleiche mit aktuellen Studien, die eine konventionelle perioperative Therapie anwendeten, zeigten, dass die postoperative Komplikationsrate, Krankenhausverweildauer und Mortalität unter „Fast-track“- Rehabilitation reduziert werden kann. Weitere randomisierte Studien zum Einfluss der Operationstechnik sind vonnöten.","In German hospitals using the multi-modal interdisciplinary ""Fast- track""rehabilitation concept, 1724 patients (undergoing elective colon resections) from 28 surgical centres that had established 'Fast-track' rehabilitation were prospectively examined within a quality assurance programme regarding the occurrence of general and surgical postoperative complications. Results: 872 of 1724 fast-track patients underwent conventional surgery, 852 laparoscopic surgery. The total rate of general postoperative complications was 9.1%, while the rate of surgical postoperative complications was 12.8%. Significantly more of the patients who underwent open surgery showed complications than those who underwent laparoscopic surgery. Significant differences occurred within the two groups in relation to demographical, epidemiological data, patients' age, secondary diagnoses, and medical indication for operation.
