Lischer","w","2011-09-15","2018-06-07T21:56:43Z","2011-11-02T11:29:17.412Z","2011","Als Erkrankung des Ellbogengelenkes wird der fragmentierte Processus coronoideus medialis ulnae (FCP) häufig schon bei jungen Hunden als Ursache für chronische Lahmheiten der Schultergliedmaße bei prädisponierten Rassen diagnostiziert. Im Rahmen der Osteoarthritis führt die Entwicklung von degenerativen Gelenkveränderungen bereits frühzeitig zur Schmerz- und Bewegungseinschränkung der Hunde. Derzeit existieren keine bildgebenden Verfahren, die eine Frühdiagnose der Osteoarthritis ermöglichen und die auch für Verlaufsuntersuchungen zur Beurteilung der Progression von Knorpelschäden herangezogen werden können. Die Analyse von knorpelspezifischen Biomarkern, die bisher noch auf experimentelle und pre-klinische Studien begrenzt ist, könnte daher eine wertvolle Ergänzung zur klinischen und bildgebenden Diagnostik darstellen. Ziel dieser Studie war die Untersuchung der Aktivitäten der MPO, MMP-2 und MMP-9 sowie der Konzentrationen von CTX-II, C2C und CPII auf ihren potentiellen Einsatz als biochemische Marker in der Diagnostik der Osteoarthritis infolge des FCP beim Hund. Weiterhin wurde untersucht, ob diese Biomarker Aussagen zur Beurteilung des Grades der Gelenkdestruktion und zum Verlauf der Erkrankung erlauben. Synovia- und Serumproben wurden von 159 an einem FCP erkrankten Tieren am Tag der Arthroskopie gewonnen. Einige Proben wurden in Verlaufsuntersuchungen nach 4 und 12 Wochen analysiert. Kontrollproben entstammen von röntgenologisch unauffälligen Tieren. Der Schweregrad der Osteoarthritis bei Tieren mit einem FCP wurde röntgenologisch in Anlehnung an die Kriterien der International Ellbow Working Group (IEWG) sowie arthroskopisch nach der modifizierten Outerbridge Klassifikation beurteilt. Im Verlauf dieser Studie wurde die Notwendigkeit einer altersabhängigen Untersuchung der Biomarker des Kollagenstoffwechsels deutlich. Da für die CTX-II-Konzentration und die MMP-2-Aktivität starke altersabhängige Unterschiede in den Tieren ermittelt wurden, erfolgte die Analyse dieser Biomarker in 2 Altersgruppen (≤ 12 Monate, > 12 Monate). Der Einfluss des Alters konnte bei der Analyse der MMP-9- und MPO-Aktivität sowie der C2C- und CPII-Konzentration vernachlässigt werden. Mittels MPO- Aktivitätsassay wurde sowohl in der Synovia als auch im Serum eine signifikant höhere MPO-Aktivität von Tieren mit FCP im Vergleich zu Kontrolltieren nachgewiesen. In Abhängigkeit vom IEWG-Grad und modifizierten Outerbridge-Grad konnte kein signifikanter Unterschied ermittelt werden. In der Gelatine- Zymographie wurde in der Synovia von Gelenken mit FCP eine höhere MMP-2- und MMP-9-Aktivität im Vergleich zu Kontrollgelenken nachgewiesen. Eine genaue Quantifizierung erfolgte mittels Aktivitäts-Immuno-Assays. Die MMP-9-Aktivität in der Synovia erkrankter Gelenke war im Vergleich zu Kontrollgelenken signifikant erhöht. Die MMP-2-Aktivität war in der Altersgruppe > 12 Monaten in der Synovia erkrankter Gelenke signifikant höher im Vergleich zu Kontrollen. Die knorpelabbauenden (CTX-II und C2C) und knorpelaufbauenden (CPII) Marker des Kollagen-Typ-II Stoffwechsels wurden mittels Immunoassays bestimmt. Zwischen der synovialen C2C-Konzentration von Kontrollgelenken und erkrankten Gelenken wurde kein signifikanter Unterschied festgestellt. Beim Vergleich von erkrankten Gelenken mit kontralateralen gesunden Gelenken desselben Tieres konnte eine signifikant höhere synoviale CTX-II-Konzentration ermittelt werden. In der Altersgruppe > 12 Monate konnte in der Synovia ein signifikanter Unterschied innerhalb der IEWG Grade mit höheren CTX- IIKonzentrationen bei stärkeren röntgenologischen Veränderungen nachgewiesen werden. Die CPII-Konzentration in der Synovia war signifikant höher in Gelenken mit FCP im Vergleich zu Kontrollgelenken. Postoperative Verlaufsuntersuchungen in der Synovia zeigten nach einem signifikanten Anstieg von Zellzahl, Proteinkonzentration, MMP-2-Aktivität sowie von CTX-II- und C2CKonzentrationen nach 4 Wochen einen Abfall nach 12 Wochen. Für einzelne Tiere wurden niedrigere Werte im Vergleich zur Ausgangskonzentration ermittelt. Die Ergebnisse der vorliegenden Studie zeigen, dass die Aktivität der MPO, MMP-2 und MMP-9 sowie die CPII-Konzentration Gelenkveränderungen in der Synovia widerspiegeln und als zusätzliche Parameter in der Diagnostik der Osteoarthritis infolge des FCP beim Hund herangezogen werden können. Erste Ergebnisse von Verlaufsuntersuchungen weisen auf die potentielle Eignung dieser Biomarker für die Therapiekontrolle beim FCP hin.","As a condition affecting the elbow joint, fragmented medial coronoid process (FMCP) is frequently diagnosed as causing chronic lameness of the thoracic limb even in young dogs of predisposed breeds. In the form of osteoarthritis, the development of degenerative processes within the joint typically causes pain and restrictions to movement early on. Currently, no imaging techniques exist allowing for an early diagnosis of osteoarthritis and for follow-up examinations to assess the progression of cartilage damage. The analysis of cartilagespecific biomarkers, which so far has been limited to experimental and pre-clinical trials, could therefore prove a valuable complement to clinical and imaging diagnostics. The aim of this study was to examine MPO, MMP-2 and MMP-9 activities as well as CTX-II, C2C and CPII concentrations with a view to their potential use as biochemical markers in the diagnostics of osteoarthritis due to FMCP in dogs. Furthermore, it was examined whether these biomarkers allow an assessment of the degree of joint destruction and of the progression of the disease. Samples of synovia and serum were taken immediately before arthroscopy from 159 dogs suffering from FMCP. Some samples were analysed in follow-up examinations after 4 and 12 weeks. Control samples were obtained from radiologically normal animals. The severity of osteoarthritis of animals with FMCP was determined by radiography according to the International Elbow Working Group’s (IEWG) criteria and by arthroscopy according to themodified Outerbridge classification. In the course of this study the necessity of an age-specific analysis of biomarkers for the collagen metabolism became evident. For the CTX-II concentration and the MMP-2 activity sizeable age-dependent differences were found. Therefore the analysis of these biomarkers was conducted in separate within two age groups of animals (≤ 12 months, > 12 months). Age-dependency could be disregarded for the analysis of MMP-9 and MPO activities as well as for C2C and CPII concentrations. Using an MPO activity assay, this study revealed a significantly increased MPO activity both in the synovia and in the serum of animals with FMCP relative to the control group. As measured by the IEWG-score and the modified Outerbridge- score no significant difference could be found. By way of gelatine-zymography an increased MMP-2 and MMP-9 activity could be detected in the synovia of FMCP affected joints relative to control joints. Immunocapture activity assays were then employed for exact quantification. MMP-9 activity was significantly elevated in the synovia of FMCP affected joints. MMP-2 activity was significantly increased in the synovia of FMCP affected joints within the age group > 12 months as compared to the controls. Biomarkers of cartilage degradation (CTX-II and C2C) and synthesis (CPII) of collagen type-II metabolism were detected by way of immunoassays. No significant difference was found between the synovial C2C concentration within FMCP affected and control joints. A comparison of FMCP affected joints with contralateral healthy joints of the same animal revealed a significant increased synovial CTX-II concentration. In the age group > 12 month a significant difference within the IEWG scores with elevated CTX-II concentration together with severe radiographic change could be established. The CPII concentration in the synovial was significantly elevated in joints affected by FMCP relative to the control. Post-surgical follow-up examination of the synovia revealed first a significant increase after 4 weeks in cell counts, protein concentration, MMP-2 activity as well as CTX-II and C2C concentrations and then a reduction after 12 weeks. For some animals, values were found below their initial concentration. The results of the present study show that activities of MPO, MMP-2 and MMP-9 as well as CPII concentration in the synovia reflect changes in the joints and are, therefore, suitable for use as additional parameters in the diagnosis of osteoarthritis due to FMCP in dogs.
