Es gibt Hinweise, dass das Ausmaß der perioperativen Immunreaktion mit einer erhöhten Inzidenz an Organversagen und einer gesteigerten Mortalität assoziiert ist. Ziel dieser Studie war es Einflüsse auf die Immunreaktion zu eruieren, sowie den Einfluss dieser auf die postoperative Langzeitmortalität zu betrachten. Methodik: Die Daten wurden im Rahmen einer randomisierten, doppelblinden Studie erhoben, in die Patientinnen zur Zytoreduktionsoperation bei primär metastasiertem Ovarialkarzinom eingeschlossen wurden. Die intraoperative Volumentherapie erfolgte nach einem zielgerichteten Hämodynamikprotokoll mit balancierter kristalloider oder kolloidaler Lösung. Blutproben wurden präoperativ, intraoperativ sowie postoperativ am ersten und dritten Tag zur Analyse von Interleukin 6 (IL-6), Interleukin 10 (IL-10), C reaktivem Protein (CrP) und Leukozytenzahl gewonnen. Ergebnisse: Die Plasmaspiegel von IL-6, IL-10, CrP und Leukozytenzahl wiesen jeweils einen postoperativen Anstieg auf und nahmen damit einen signifikant charakteristischen Verlauf über den Beobachtungszeitraum (p<0,0001). Ein signifikant unterschiedlicher Einfluss durch Kristalloide oder Kolloide konnte dabei nicht gesehen werden. In den Regressionsanalysen zeigten sich positive Assoziationen zwischen Fresh Frozen Plasma (FFP) und IL-6 (OR 1,185; CI 1,074-1,308; p=0,001) sowie Erythrozytenkonzentraten und CrP (OR 1,157; CI 1,037-1,29; p=0,009), und eine negative Assoziation zwischen FFP und CrP (OR 0,965; CI 0,932-0,998; p=0,04). Der Sequential Organ Failure Assessment Score war mit gesteigerten IL-6-Plasmaspiegeln assoziiert (OR 1,836; CI 1,193-2,826; p=0,006). CrP zeigte positive Assoziationen mit der Schwere (OR 1,739; CI 1,38-2,191; p<0,0001) und der Anzahl (OR 1,347; CI 1,027-1,767; p=0,032) an Komplikationen. Mit dem Langzeitüberleben war insbesondere der IL-6-Spiegel am dritten postoperativen Tag (HR 1,92; CI 1,251-2,947; p=0,003) und der IL-10-Spiegel präoperativ (HR 1,168; CI 1,016-1,343; p=0,029) assoziiert. Schlussfolgerung: Die perioperative Immunreaktion wird durch Kristalloide oder Kolloide nicht unterschiedlich beeinflusst, während die Transfusion von Blutprodukten signifikante Assoziationen erkennen lassen. Ein zügiger postoperativer Abfall der IL-6-Plasmakonzentration bis zum dritten postoperativen Tag war ebenso prädiktiv für das Langzeitüberleben wie die präoperative IL-10-Konzentration.","Introduction: Major surgery is associated with an intense inflammatory response, which could evolve into a systemic inflammatory response syndrome (SIRS) in the postoperative course. There is evidence that the extent of the immune response influences the outcome and is correlated with multi organ failure and mortality rate. The aim of this study was to investigate influences on immune response in non-cardiac surgery and to evaluate effects on long term survival. Methods: In a double-blinded randomized study data was obtained from patients with ovarian cancer undergoing cytoreductive surgery. Intraoperative fluids were administered according to a goal-directed hemodynamic algorithm with balanced crystalloid or starch solutions. Serum concentration of Interleukin 6 (IL-6), Interleukin 10 (IL-10), C reactive protein (CrP) and white blood cell count were measured before, 2 hours after onset, 1 and 6 hours after operation and at day 1 and 3 following operation. Results: Levels of IL-6, IL-10, CrP and white blood cell count showed a distinct postoperative course (p<0,0001) and increased respectively compared to baseline values. Regarding fluid administrations there was no difference between groups. Regression analyses revealed a positive association between fresh frozen plasma (FFP) and IL-6 (OR 1,185; CI 1,074-1,308; p=0,001), between packed red blood cells and CrP (OR 1,157; CI 1,037-1,29; p=0,009) and a negative association between FFP and CrP (OR 0,965; CI 0,932-0,998; p=0,04). The Sequential Organ Failure Assessment Score was related to increased IL-6 levels (OR 1,836; CI 1,193-2,826; p=0,006), whereas CrP was associated with severity (OR 1,739; CI 1,38-2,191; p<0,0001) and number (OR 1,347; CI 1,027-1,767; p=0,032) of complications. Most important for survival was the IL-6 level on third day after (HR 1,92; CI 1,251-2,947; p=0,003) and the level of IL-10 before (HR 1,168; CI 1,016-1,343; p=0,029) operation. Conclusions: The immune response following non-cardiac surgery is not influenced significantly by balanced crystalloid or starch solutions. However, blood products are associated with inflammatory mediators.
