Das Ziel des Eingriffs ist, das muskuläre Ungleichgewicht zu vermindern und damit die Funktion zu verbessern und möglicherweise die Entstehung von Kontrakturen und Hebelarmerkrankungen zu reduzieren. Damit soll auch das Gehen als hohe und im Alltag relevante motorische Funktion nachhaltig verbessert werden. Die vorliegende Habilitationsschrift ist eine systematische Zusammenfassung der bisher veröffentlichten anhand der Berliner Kohorte durchgeführten Untersuchungen hinsichtlich des Einflusses der SDR auf Motorik, Funktion, Gehen sowie Entstehung von Deformitäten. Im Vordergrund des Projekts steht zum einen die Etablierung der Methode im deutschsprachigen Raum. Zum anderen wird anhand der Aufarbeitung der prospektiv gesammelten Daten der positive Effekt der SDR bei geringem Risiko verdeutlicht. Die Daten werden hinsichtlich bisher kontrovers diskutierter Parameter evaluiert, um einen Beitrag leisten zu können zur Verbesserung der Indikationsstellung, der Prognosebeurteilung und der interdisziplinären Erstellung eines individuellen optimalen multimodalen Therapiekonzepts für gehfähige Kinder mit spastischer Cerebralparese. Durch kritische Evaluierung der Operationsauswirkung ist zukünftig das intraoperative Vorgehen für den einzelnen Patienten besser dosierbar.","Included in a multimodal treatment concept, the selective dorsal rhizotomy (SDR) is a promising treatment option to improve motor function in patients with bilateral spastic cerebral palsy. The aim of this intervention is to reduce the muscular imbalance and thus to improve function and possibly to reduce the development of contractures and lever arm diseases. Thereby walking as a high and relevant everyday motor function might also be improved sustainably. This manuscript is a systematic synopsis of to date published data from our patient cohort concerning the influence of SDR on motor function, walking and the development of deformities. The focus of the project is on the one hand the establishment of the method in Germany. On the other hand, the positive effect of the SDR with its low risk is elucidated based on the analysis of the information collected prospectively. The data are evaluated for previously controversially discussed parameters, to contribute further knowledge to improve the recommendation of this procedure, the prognosis assessment and the interdisciplinary development of an individual optimal multimodal therapy concept for ambulatory children with spastic cerebral palsy.
