Eine verlässliche Prognoseabschätzung ist somit essentiell für eine adäquate Therapieentscheidung. Der Erkrankungsverlauf wird durch das Tumorvolumen (TV) entscheidend beeinflusst. Als vielversprechender, neuer Marker ist der Prostate Health Index (PHI) ein präziser Prädiktor eines TV ≥ 0,5 cm³. Ob jedoch ein linearer Zusammenhang zwischen PHI und TV besteht wurde bislang noch nicht untersucht und ist Gegenstand dieser Arbeit. Methoden: Es wurden 196 Prostatektomiepräparate (pT2a─4) von PCa-Patienten, die sich im Zeitraum von 2001 bis 2014 an der Charité einer radikalen Prostatektomie unterzogen haben, untersucht. Anhand der histologischen Schnitte wurde das Tumorareal eingezeichnet und mittels Quadratrasterfolie der prozentuale Tumoranteil ermittelt. Das TV wurde durch die Multiplikation des prozentualen Tumoranteils mit dem gesamten Prostatavolumen berechnet. Die präoperativ gemessenen Serumparameter tPSA, fPSA, [ 2]proPSA und PHI wurden mit den pathologischen Befunden (pathologischer Gleason-Score (pGS), pT Stadium und TV) verglichen. Ergebnisse: Von allen Markern war PHI der akkurateste Prädiktor eines TV ≥ 0,5 cm³ (AUC: PHI (0,79), [-2]proPSA (0,72), tPSA (0,74), %fPSA (0,68)). Es bestand eine starke Korrelation zwischen PHI und TV (r = 0,59, p < 0,001). Die Serummarker tPSA, %fPSA, [ 2]proPSA und PHI erreichten in der univariaten Regressionsanalyse allesamt einen eigenständigen Prädiktorstatus bei der Vorhersage des TV (alle p < 0,001). Dabei stellten sich PHI und [ 2]proPSA als die stärksten Prognoseparameter dar. Bei der multivariaten Regressionsanalyse konnte die Vorhersagegenauigkeit des Basismodells (pGS, pT-Stadium, tPSA, %fPSA) durch die Integration von PHI (+ 14,3 %) bzw. von [-2]proPSA (+ 18,8 %) signifikant (beide p < 0,001) gebessert werden. Schlussfolgerung: Die Serummarker PHI und [-2]proPSA sind akkurate Prädiktoren des TV im Präparat der radikalen Prostatektomie. Da das TV einen entscheidenden Einfluss auf die Erkrankungsprogression hat, erscheint es vorteilhaft, PHI (oder [-2]proPSA) als ein Kriterium für die aktive Überwachung von PCa-Patienten zu nutzen.","Objective: Prostate cancer is, in many cases, a slowly evolving tumor and therefore not likely to cause clinical symptoms. Therefore predicting the progression of the disease is essential for adequate therapeutic decision- making. Tumor volume (TV) is one of the major determinants of cancer aggressiveness. The PHI test is a new promising serum marker which is able to predict the risk of having a TV ≥ 0.5 cm³. A possible linear relationship between PHI and prostate cancer volume has not yet been evaluated and is the objective of this study. Methods: For this study 196 prostate specimens (pT2a─4) of patients with prostate carcinoma, who had been treated with radical prostatectomy between 2001 and 2014 at the clinic of the university Charité, Berlin, have been evaluated. First, the percentage of tumor, as related to total prostatic tissue, was measured by plotting the cancer on the histological sections and estimating the amount of affected area via a grid pattern transparency. Next, TV was calculated by multiplying the percentage of tumor with total prostate volume. The markers tPSA, %fPSA, [ 2]proPSA and PHI , which had been measured before prostatektomy, were compared with pathologic Gleason-Score (pGS), pT-Stage and TV. Results: Of all markers, PHI was the most accurate predictor of a TV ≥ 0.5 cm³ (AUC: PHI (0.79), [-2]proPSA (0.72), tPSA (0.74), %fPSA (0.68)). PHI correlated strongly with TV (r = 0.59; p < 0.001). At univariate analysis all serum markers were significant predictors of TV in prostatectomy specimens (all p < 0.001); of which PHI and [-2]proPSA proved to be most accurate. Including PHI or [-2]proPSA in a multivariate base model, consisting of pGS, pT-Stage, tPSA and %fPSA, the accuracy of predicting the TV was enhanced by 14,3 % (PHI) or 18,8 % ([-2]proPSA) respectively (both p < 0.001). Conclusion: Both PHI and [-2]proPSA are accurate predictors of TV in specimens of radical prostatectomy.
