Konventionell treten Fehlplatzierungen meist infolge schlechter intraoperativer Visualisierung auf. Bis zur optimalen Platzierung der Bohrung sind zum Teil mehrere Versuche notwendig. Intraoperativ muss die Lage mit dem C-Arm in mehreren Ebenen kontrolliert werden, wodurch unter Umständen erhebliche Durchleuchtungszeiten entstehen. Die 2D-C-Arm-Navigation stellt als alternatives Verfahren, eine gute Methode dar, die am Becken bislang nur in wenigen Kliniken zur Anwendung kommt. In dieser Arbeit wird die Genauigkeit und Sicherheit passiver 2D-C-Arm- basierter optoelektronischer Navigationssysteme durch Präzisionsbohrungen anhand 5 standardisierter Schraubenpositionen am Becken nachgewiesen. Die abschließende Kontrolle erfolgte durch CT. Mit dem SurgiGATE®- System (Medivision®) wurden, im Rahmen der experimentellen Hauptstudie an 6 Kunststoffbecken, insgesamt 60 Bohrungen durchgeführt. 55 von 60 Bohrungen (91,7%) wurden korrekt platziert. 5 von 60 Bohrungen (8,3 %) wiesen eine geringe Perforation der Kortikalis auf. An 3 humanen Beckenpräparaten erfolgte die Evaluation mit der Stealth Station® (Sofamor Danek®). Es wurden insgesamt 19 Schrauben gesetzt. 17 von 19 Schrauben (89,5%) waren korrekt platziert. 2 von 19 Schrauben (10,5%) zeigten eine Perforation. Das Patientenkollektiv im klinischen Teil bestand aus 36 Patienten. 35 von 36 Schrauben (97,2%) konnten perkutan korrekt platziert werden. 1 von 36 Schrauben (2,8%) perforierte Kortikalis ohne klinische Komplikation. Die Durchleuchtungszeit betrug bei den Kunststoffbecken weniger als 30 Sekunden, am Humanpräparat unter 2 Minuten und im klinischen Teil 0,5 Minuten. Im Vergleich zur konventionellen Methode konnte für die 2D-C-Arm-Navigation eine hohe Präzision, mit geringeren Perforationsraten, bei deutlich reduzierten Durchleuchtungszeiten festgestellt werden. Ergebnisse und Indikationen wurden mit aktueller Literatur verglichen und der Stand der navigierten Beckenchirurgie beschrieben.","Complicated injuries, like instability pelvic fractures, are often openly operated and lead to considerable soft tissue damage. Conventionally false placements mostly appear as a result of bad intraoperative visualisation. Up to the optimum placement of the drilling several attempts are necessary partly. Intraoperativ must be checked the situation with the C-arm in several levels from what under circumstances considerable fluoroscopy times originate. The 2D-C-arm navigation explains as an alternative procedure, a good method which comes at the pelvis up to now only in few medical centres to the application. Inside this work the accuracy and safety is proved more passively of 2D-C-arm-based optoelectronic navigation systems by precision drillings with the help of 5 standardised screw placements at the pelvis. The final control was carried out by CT. In comparison to the conventional method, a high precision, with lower perforation rates, with clearly reduced fluoroscopy times could be ascertained for 2D-C-Arm- Navigation.
