Untersucht wurde das Rezidivmuster und die vorausgegangene Therapie bei Patienten mit pathologisch nachgewiesenem Merkelzell-Karzinom (MCC). Eingeschlossen wurden 65 Patienten mit lokalisiertem MCC: 36% hatten Union internationale contre le cancer (UICC) Stadium-I-, 19% -II-, 45% -III-Tumoren (davon fünf Lymphknotenmetastasen eines unbekannten Primärtumors). 81,5% der Patienten erhielten ein chirurgisches Staging: 24 Wächterlymphknoten-Biopsien (WLKB), 22 WLKB gefolgt von Lymphadenektomie (LAD) und 7 LAD. Vollständig reseziert waren 85% der Primärtumoren (PT). Eine Radiotherapie (RT) erhielten 51% der Patienten, 47% eine RT der PT-Region, 32% (8/25 Patienten) eine regionäre RT bei snN0 und 64% (18/28 Patienten) auf pN+ mit einer mittleren Gesamtdosis von 51,5 bzw. 50 Gy. Die mittlere Nachbeobachtungszeit lebender Patienten betrug 6 Jahre (Spanne 2 – 14 Jahre). Insgesamt 43% (27/65 Patienten) entwickelten ein Rezidiv, von denen 22 lokoregionäre (LRR) und 5 distante Rezidive waren. Die 5 Jahres-LRR- Rate betrug 36%. Im Stadium I/II (snN0) traten regionale Rezidive (RR) in 5 von 17 unbestrahlten und 0 von 8 bestrahlten Regionen auf (p = 0,17, 5-Jahres- RR-Rate 24% vs. 0%). Die krude RR-Rate war im Stadium I (12%, 2/17 Patienten) niedriger als im Stadium II (43%, 3/7 Patienten) (p = 0,03). Im Stadium III (pN+) traten weniger RR nach Bestrahlung auf (17%, 3/18 Patienten, in bestrahlten vs. 30%, 3/10 Patienten, in unbestrahlten Regionen, p = 0,46). Unsere Daten zeigen, dass auch bei sN0 Patienten eine adjuvante RT eine wichtige Rolle spielt. Die adjuvante regionäre RT von Lymphknoten bei Patienten mit Stadium IIa (snN0) sowie nach LAD im Stadium III erscheint sinnvoll und sollte prospektiv evaluiert werden. The relapse pattern and prior therapy of patients with pathologically proven merkel cell carcinoma (MCC) was retrospectively studied in a single institution. A total of 65 patients with MCC, 36% Union internationale contre le cancer (UICC) stage I, 19% II, 45% III (five lymph node metastases of unknown primary) were eligible. 81,5% of the patients had surgical staging: 24 patients sentinel node biopsy (SNB), 22 patients SNB followed by lymphadenectomy (LAD) and 7 patients LAD. 85% of the primary tumors (PT) were completely resected; 51% of patients received radiotherapy (RT), 47% of known PT sites, 32% (8/25 patients) regional RT to snN0 nodes and 64% (18/28 patients) to pN+ nodes, mean reference dose 51.5 and 50 Gy, respectively. Mean follow-up was 6 years (range 2 – 14 years). A total of 43% (27/65) patients developed relapses; of which 22 were locoregional (LRR) and 5 were distant. The 5-year LRR rate was 36%. In snN0 sites (stage I/II), regional relapse (RR) occurred in 5 of 17 non irradiated vs. none of 8 irradiated sites (p = 0.17), resulting in a 5-year RR rate of 24% versus 0 %. The crude RR rate was lower in stage I (12%, 2/17 patients) than for stage II (43%, 3/7 patients) (p = 0,03). In stage III (pN+), RR appeared to be less frequent in irradiated sites (17%, 3/18 patients) compared with non irradiated sites (30%, 3/10 patients, p = 0.46). Our data suggest that adjuvant nodal RT plays a major role even if the sentinel nodes were negative. Adjuvant RT of the lymph nodes in patients with stage IIa tumors and RT after LAD in stage III tumors is proposed and should be evaluated prospectively.
