Wirbelauer","m","2012-09-07","2018-06-07T19:58:10Z","2012-09-03T08:03:59.097Z","2012","Hintergrund: Häufig wird das gegenüber der perforierenden Keratoplastik als reduziert angenommene Risiko für das Auftreten von Immunreaktionen als Vorteil der lamellären Keratoplastik-Techniken angesehen. Aufgrund der noch relativ geringen Verbreitung der lamellierenden Keratoplastiken, insbesondere in aktuellen verfeinerten Techniken (DSAEK), liegen hierzu bisher allerdings nur wenige Daten vor. Ziel der vorliegenden Arbeit war es daher, die Inzidenz von Immunreaktionen von anterioren und posterioren lamellierenden Keratoplastiken der Augenklinik Charité Campus Virchow-Klinikum zu erfassen und mit der Inzidenz von Immunreaktionen nach perforierenden Keratoplastiken zu vergleichen. Patienten und Methoden: Einbezogen in die Untersuchung wurden die Daten von insgesamt 172 Keratoplastikpatienten, die von 1999 bis 2000 sowie von 2007 bis 2009 an der Augenklinik der Charité Campus Virchow-Klinikum operiert wurden. Die Auswertung erfolgte mittels SPSS (Statistical Package for the Social Sciences), Version 12.0. Neben dem Mittelwertvergleich und der rein deskriptiven Auswertung wurde der Chi-Quadrat-Test zur Auswertung eingesetzt. Ergebnisse: Von den 172 Patienten, die in dieser Studie untersucht wurden, erhielten 41 Patienten eine lamelläre und 131 Patienten eine perforierende Keratoplastik. Dabei zeigte sich im Verlauf bei 2,4 % der Patienten mit lamellärer KPL und bei 29,8 % der Patienten mit perforierenden KPL eine Immunreaktion (χ2=13,07, p< .001). Von den 44 Patienten mit Fuchs- Endotheldystrophie erhielten 28 Patienten eine hintere lamelläre (DSAEK) und 16 Patienten eine perforierende Keratoplastik. Dabei zeigte sich bei 3,6 % der Patienten mit hinterer lamellärer KPL und bei 18,75 % der Patienten mit perforierender KPL eine Immunreaktion (χ2=2,84, p< .092). Schlussfolgerung: Immunreaktionen treten erheblich seltener nach lamellierender als nach perforierender KPL auf. Der Gesamtvergleich ist allerdings aufgrund der heterogenen Ausgangsituationen schwierig. Auch bei gleicher Indikation (Fuchs- Endotheldystrophie) weisen die hinteren lamellären KPL (DSAEK) deutlich seltener Immunreaktionen auf.","Background. Lamellar keratoplasty is a relatively new surgical procedure for replacing the diseased layer of the cornea with a healthy layer. There are only few data regarding graft rejection in lamellar keratoplasty. The objective of this paper was to compare lamellar keratoplasty with penetrating keratoplasty for the incidence and risk of graft rejection. Methods. This retrospective study included a total of 216 consecutive patients who underwent penetrating keratoplasty or lamellar keratoplasty at the Department of Ophthalmology, Charité CVK, between 1999 and 2000 and between 2007 and 2009. Complete data were obtained in 172 cases. Statistical analysis was performed us¬ing SPSS (Statistical Package for the Social Sciences), version 12.0. Descriptive statistics, χ2-test and mean values were used to describe and assess the data. Results. Out of 172 eyes 41 were treated with lamellar keratoplasty and 131 eyes were treated with penetrating keratoplasty. The incidence of graft rejection was 2.4% in the lamellar group and 29.8% in the penetrating keratoplasty group (χ2 = 13.07, p < 0.001). Of 44 patients with Fuchs’ endothelial dystrophy (FED) 28 underwent posterior lamellar keratoplasty and 16 patients were treated with penetrating keratoplasty. The incidence of graft rejection in this group was 3.6% after posterior lamellar keratoplasty and 18.75% after penetrating keratoplasty. Conclusions. In this retrospective study with univariate analysis of risk factors, graft rejection was less frequent after lamellar keratoplasty than after penetrating keratoplasty. A comparison of both groups was limited because of different indications for surgery.
