Heesen","w","2012-11-30","2018-06-07T19:53:07Z","2012-11-28T11:58:51.408Z","2012","Shared Decision Making (SDM) ist eine spezielle Form der Arzt-Patienten- Kommunikation, in der sowohl Arzt als auch Patient gleichberechtigte Partner sind und gemeinsam ein Behandlungskonzept erarbeiten, zu dessen Umsetzung sie gleichermaßen bereit sind. Der anästhesiologische Behandlungserfolg basiert auf einem gegenseitigen Informationsaustausch im Sinne des SDM, der nur durch die Partizipation des Patienten in der präoperativen Anästhesievisite gewährleistet werden kann. Zahlreiche Studien zum Thema SDM belegen, das Patienten eine höhere Zufriedenheit, Therapietreue, Compliance und Lebensqualität aufweisen, wenn sie in Diagnose- und Therapieentscheidungen einbezogen werden 57,58,59. In diesem Zusammenhang dient die vom Patienten wahrgenommene Einbeziehung im Sinne des SDM auch der Evaluation der Gesundheitsversorgung bei zunehmender Kundenorientierung im Gesundheitswesen. Ziel dieser Studie war es zu untersuchen, wie Patienten einer Anästhesieambulanz eines innerstädtischen Universitätskrankenhauses die aus ihrer Sicht wahrgenommene Einbeziehung in die präoperative Anästhesievisite im Sinne des SDM bewerten. Darüber hinaus sollte geprüft werden, inwiefern anthropometrische und sozioökonomische Merkmale einen entscheidenden Einfluss auf die wahrgenommene Einbeziehung und den SOC haben und ob ein maßgeblicher Zusammenhang zwischen anthropometrischen, sozioökonomischen Merkmalen, dem SOC und der wahrgenommenen Einbeziehung in die präoperative Anästhesievisite besteht. Die Datenerhebung erfolgte stichprobenartig im Zeitraum vom 23.01.2007 bis 17.10.2007 im Rahmen einer Studie zum Thema „Prävention Nikotin – assoziierter Erkrankungen durch Raucherentwöhnung“ vom 15.02.2006 bis 31.12.2007 in den Anästhesieambulanzen der Standorte Charité Campus Mitte (CCM) und Charité Virchow Klinikum (CVK) der Universitätsklinik für Anästhesiologie mit Schwerpunkt operativer Intensivmedizin Charité – Universitätsmedizin Berlin. Es wurden nur die Datensätze derjenigen Patienten für die Analyse berücksichtigt, die die computerisierte Version des Brief Assessment of Sense of Coherence (BASOC) 51, eingebunden in Fragen zu anthropometrischen Merkmalen, zum sozioökonomischen Status und zu soziodemographischen Variablen vor und die Perceived Involvement in Care – Scales (PICS) 46 zur Bewertung der wahrgenommenen Einbeziehung im Sinne des SDM nach der präoperativen Anästhesievisite vollständig beantwortet haben. Es ergab sich ein Kollektiv von insgesamt 286 Patienten. In der deskriptiven Statistik wurden Unterschiede zwischen unabhängigen Gruppen für normal verteilte Variablen mittels t – Test beziehungsweise einfaktorieller Varianzanalyse (ANOVA) und für nicht normal verteilte Variablen mittels Krukal – Wallis – Test geprüft. Zusätzlich erfolgte eine multivariate lineare Regression in Form einer Backward – Analyse zum Ausschluss unabhängiger Variablen mit dem kleinsten partiellen Korrelationskoeffizienten. Das Signifikanzniveau wurde für alle Tests bei p < 0,05 festgelegt. Die statistische Auswertung erfolgte mit Hilfe des Statistik – Programms SPSS 14.0. In Bezug auf die von den Patienten wahrgenommene Einbeziehung in die präoperative Anästhesievisite zeichnete sich in unserer Studie eine hohe Bewertung ab. Einen signifikanten Einfluss darauf zeigten neben „Alter“ und „Geschlecht“ auch der „Familienstand“ und die „berufliche Situation“. In Bezug auf den SOC zeichnete sich das „Alter“ sowie der „Familienstand“ und die „Berufliche Situation“ als signifikante Einflussgrößen ab. In der mulitvariaten linearen Regression in Form einer Backward – Analyse konnte ein maßgeblicher Zusammenhang zwischen dem „Geschlecht“ und „Alter in Jahren“ sowie dem beruflichen Status „Arbeiter“, jedoch keinerlei Einfluss des SOC auf die vom Patienten wahrgenommene „Einbeziehung“ in die präoperative Anästhesievisite nachgewiesen werden. Es wurde aufgezeigt, dass junge Patienten, beruflich Selbstständige und Angestellte/Beamte eines intensiveren Informationsaustausches sowie einer verstärkten Motivation zur Partizipation durch den Arzt bedürfen. Besonders Frauen und junge Patienten sollten zur aktiven Auseinandersetzung mit Informationen bezüglich ihrer Erkrankung angehalten werden. Um individuelle Bedürfnisse des Patienten zukünftig erfassen zu können, gilt es den Bildungsstand und die berufliche Qualifikation der Patienten in der Arzt – Patienten – Kommunikation zu beachten. Somit sollte SDM als Qualitätskriterium und vergleichsweise neuer Aspekt der medizinischen Ausbildung in der Fort- und Weiterbildung des Arztes berücksichtigt und im Arbeitsalltag integriert werden.","Shared Decision Making (SDM) is a special form of doctor-patient- communication, in this the doctors as well as the patients are equitable partners and work out a treatment concept both are willing to implement. The success of anesthesia treatment is based on exchange of information in the sense of SDM who can just be assure by patient´s participation in the preoperative anesthesia ward round. A numerous of studies to the theme of SDM prove that patients posses a higher satisfaction, adherence, compliance and life quality if they are involved in decisions of diagnosis and therapy options 57,58,59. In this context the patients percieved involvement in the sense of SDM is evidenced of evaluation of medicare by increasing costumer focus in public health. The aim of the study was to analyse, how patients of a anesthesia ambulance of a university hospital prove the percieved involvement in the preoperative anesthesia ward round in the sense of SDM. Above all it should be verified, in which case anthropometric and sozioeconomic characteristics have a represantive correlation at percieved involvement and the Sense of Coherence (SOC) and if there exist an important correlation between anthropometric, socioeconomic characteristics, the SOC and the percieved involvement in the preoperative anesthesia ward round. The random data ascertainment has effected between 23.01. – 17.10.2007 during a study with the theme „Prevention nicotine associated diseases because of nicotine dehabituation“ in anesthesia ambulances in the place of Charité Campus Mitte (CCM) and Charité Campus Virchow (CVK) of the university hospital of anaesthesiology with focus on operative intensive care Charité University Berlin. Only the data sets of those patients have been considered who answered completely all questions of the digital version of the Brief Assesment of Sense of Coherence (BASOC), integrated in questions of anthropometic facts, socioeconomic status and the sociodemographic variables before and of the Percieved Involvement in Care – Scales (PICS) 46 for the assessment of the percivied involvement in the sense of SDM after the preoperative anaesthesia- visit. It added up a collective of 286 patients. In the descriptive statistic the differences between the independed groups for normal disseminate variables have been verified by t – Test respectively ANOVA (analysis of variance) and for non-disseminate variables verified with the Kruskal-Wallis-Test. All above a multivariate linear regression in a form of a backward-analysis effected for the exclusion of independent variables with the smallest partial correlation coefficient. The significance level was fixed for all tests by p < 0,005. The statistc analyses happend with the help of SPSS 14.0. In matters of the patient percieved involvement in the preoperative anesthesia ward round a high assessment became apperent. A significant influence showed next to the „age“ and „sex“, the „family status“ and the „professional situation“. In matters of the SOC the „age“ as well as the „family status“ and the „professional situation“ showed a significance influence. In the multivariate linear regression in the form of a backward-analyse we could prove a relevant context between the „sex“ and „age in years“ as well as professional status „worker“, but no influence at all of the SOC at the percieved involvement in the preoperative anesthesia ward round. We could show that young patient, professional self-employed people and employees/civil cervants need a more intensive information exchange and a stronger motivation for participation by the doctor. Especially women and young patients should be more encourage to dispute themselves more intensely with informations about their diseases. For better dectecting of the individual needs in the future it is important to observe the patient´s professional qualification in the doctor-patient- communication.
