Das elektrische Remodeling beschreibt die stattfindenen Veränderungen von Ionenkanälen und damit die Beeinflussung von Ionenflüssen. Die Herzhypertrophie wird klinisch erst spät erkannt. Durch frühzeitige Diagnose kann der Hypertrophie meist auf medikamentösem Wege entgegengewirkt und so die Endorganschädigung (z.B. an Herz, Nieren, Gehirn) reduziert werden. Dadurch kann die Lebensqualität und die Prognose des Patienten gebessert werden. In dieser vorliegenden Arbeit wurden die magnetischen Signale des Herzens mittels MKG an insgesamt 20 doppeltransgene Ratten (dTGR) und 7 Sprague-Dawley-Ratten (Kontrollgruppe) im Alter von 4, 5 und 7 Wochen gemessen. Von den dTGR wurden 12 ohne medikamentöse Therapie und 8 unter medikamentöser Therapie mit Losartan untersucht. Die doppeltransgenen Ratten entwickelten auf der Basis ihrer Überexpression an humanen Renin und Angiotensinogen eine arterielle Hypertronie, eine myokardiale Hypertrophie und als Beispiel für Endorganschäden Nierenschäden. Dies wurde mittels Blutdruckmessung, der Echokardiographie und der Albuminurie nachgewiesen. Als Parameter für die magnetokardiographische Beschreibung der Hypertrophie im MKG kristallisierten sich der Inhomogenitätsindex (IHi), die Addition der absoluten Werte des Maximums und Minimums der Magnetfeldstärken (max-min) und der Winkel, welcher durch die Positionen der Extrempunkte und durch die rechts-links Linie des Torsos bestimmt wurde, heraus. Die verwandten Zeitintervalle (QRSGesamt: 0-20 ms, RSpitze: 8-12 ms, STT: 20-25 ms, TEnde: 60-70 ms) wurden zur Unterscheidung von De- und Repolarisationsvorgängen ausgewählt. Das magnetokardiographische Hypertrophiebild trennte sich von dem Normalbild zu Beginn (4. Woche) hochsignifikant durch Repolarisationsveränderungen, insbesondere im IHi und max-min in STT und im IHi und Winkel bei TEnde. Grundlage hierfür liegt in der Wirkungen von Angiotensin II. Es verursacht eine Zunahme der Dispersion der kardialen Repolarisation und inhibiert in ventrikulären Rattenmyozyten den transienten K+-Auswärtsstrom. Dies bewirkt eine Zunahme der Aktionspotentialdauer und die Repolarisationszeit wird verlängert. In den sich anschließenden MKG-Messungen setzten sich die Repolaristionsstörungen fort. In der Depolarisationsphase (QRSGesamt und RSpitze) trennten der IHi und max-min hochsignifikant, und der Winkel signifikant in QRSGesamt dTGR von den SD. Pathophysiologischer Hintergrund kann die Größenzunahme der Kardiomyozyten sein, die die Intensität der Magnetfeldstärke erhöht. Angiotensin II wirkt dabei als Wachstumsfaktor. Ebenfalls beeinflusst Angiotensin II die elektrophysiologische Ebene über Veränderungen der aktiven und passiven Eigenschaften des Myokards wie z.B. der Leitungsgeschwindigkeit, der Gap junction-Verteilung und der Ionenkanalexpression. Es resultiert eine Zunahme der Depolarisationszeit. Durch die Applikation des AT1-Antagonisten Losartan fanden Veränderungen im magnetokardiographischen Bild der Hypertrophieentwicklung statt. Dadurch näherten sich zum Studienende die Losartangruppe (dTGR/Los) den gesunden Kontrolltieren an. Nur der IHi in QRSGesamt unterschied sich. Die Wirkung von Losartan zeigte sich auch im direkten Vergleich zwischen dTGR und dTGR/Los, die sich in der Repolarisationsphase (STT und TEnde) bei den Parametern IHi, max-min und Winkel signifikant voneinander unterschieden. Die Therapie mit AT1-Antagonisten verhindert bzw. vermindert somit das elektrische Remodeling im Rahmen der Hypertrophieentwicklung. Angiotensin II wirkt dabei hauptsächlich über den AT2-Rezeptor u.a. durch Hemmung der Wachstumsfaktor- induzierte Proliferation, der Supprimierung der Myozytenhypertrophie und der Verminderung des Wachstums von kardialen Fibroblasten. AT1-Antagonisten modifizierten auch K+-Kanäle und wirken so direkt auf das elektrische Remodeling ein. Dieser Arbeit konnte zum ersten Mal ein magnetokardiographisches Hypertrophiemodell an der Ratte aufgestellen. Anhand dieses Modells ist es möglich, pharmakologische Einflüsse auf das elektrische Remodeling, z.B. mit einem AT1-Antagonisten (Losartan), zu untersuchen.","The myocardial hypertrophy plays a central role in the process of heart failure development and the emergence of malignant arrhythmias. This could be prevented pharmacologically by an early diagnosis of the hypertrophy, which results in a reduction of end organ damage. This would improve the quality of life and the prognosis of the patient. The goal of this paper was to investigate the electrical remodeling of the myocardial hypertrophy development induced by hypertension at a double-trangenic rat model (overexpression of human renin and angiotensinogen) by the means of the magnetocardiography (MCG). The electrical remodeling describes changes on the basis of ion channels, which affect the electric currents. In a hypertrophied heart the tangential currents increase. The MCG is very sensitive and therefore very suitable to recognize hypertrophy induced changes. In this study, MCG measurements were accomplished on a control-group of Sprague Dawley rats (SD) and doubletransgenic rats (dTGR). Further, the effect of the AT1-antagonist Losartan was tested to dTGR. It could be demonstrated that dTGR developed a hypertension, a myocardial hypertrophy, and kidney damage due to the overexpression of human renin and angiotensinogen. These pathological processes were proven by blood pressure and albuminuria measurements and the echocardiography. During the period of the 4th and 7th life week, dTGR showed significant differences in the MCG measurements compared to the SD. These were detected by the magnetokardiographic parameters of the inhomogeneity index (IHi), the addition of of the absolute values of the maximum and minimum of the magnetic field strength (max-min) and the angle. The used time intervals (QRStotal: 0-20 ms, Rpeak: 8-12 ms, STT: 20-25 ms, Tend: 60-70 ms) were selected to separate de- and repolarisation time. At the beginning (4th week) high-significant repolarisation changes showed up, in particular in IHi and in max-min in the time interval of STT, and also in IHi and angle in Tend. One reason is the effect of angiotensin II. It causes an increase of the dispersion of the cardial repolarisation and inhibits the transient potassium current in ventricular myozytes of the rat. In the following MCG measurements (5-7th week) a high-significant change was detected in the depolarisation phase (QRStotal and Rpeak) in the parameter IHi and max-min; likewise, the angle was significantly changed in QRStotal. The pathophysiological background is the increase of the size of cardiomyozyts. Angiotensin II acts as a growth factor. Likewise, angiotensin II affects the electrical physiology by changes of the active and passive characteristics of the myocardium, e.g. the transmission rate, the gap junction distribution, and the expression of the ion-channels. It results in an increase of the depolarisation time in the parameters already mentioned. By the application of the AT1-antagonist Losartan, a nearly complete prevention of the development of the myocardial hypertrophy of the dTGR could be obtained, which exhibited thereby approximately wild-typical (SD) values in the particular parameters. Thus, the Losartan-therapy can decrease and/or prevent the electrical remodeling in the context of the development of myocardial hypertrophy. AT1-antagonists modify potassium-channels and affect therefore directly the electrical remodeling. Moreover they promote the angiotensin II-effect of inhibition of the growth- factor-induced proliferation, the suppression of myocyte hypertrophy, and the reduction of growth of cardiac fibroblasts obtained by the AT2-rezeptor. On the basis of this paper, a magnetocardiographic hypertrophy-model on double- transgenic rat could be set up for the first time.
