Primäres Ziel dieser Arbeit war es, molekulare Biomarker zur Prädiktion des Ansprechens auf neoadjuvante Chemotherapie zu untersuchen. Dazu wurde das prädiktive Potenzial des Proteins SPARC (Secreted protein acidic and rich in cysteine) immunhistochemisch im neoadjuvant therapierten Mammakarzinom analysiert. Zusätzlich wurde SPARC auch im adjuvant mit Gemcitabin behandelten Pankreaskarzinom untersucht. Das Protein SPARC scheint in beiden Tumorentitäten als möglicher Marker eine Relevanz zu haben. Im Mammakarzinom konnte eine hohe SPARC Expression als unabhängiger prädiktiver Marker für ein Therapieansprechen, insbesondere in der triple-negativen Situation, identifiziert werden. Im Pankreaskarzinom zeigte sich ein negativer prognostischer Einfluss von SPARC. Da SPARC Albumin binden kann, wird dies besonders im Zusammenhang mit einer möglichen nab- Paclitaxel Therapie interessant, welche nachweislich in beiden Entitäten zu einem verbesserten Therapieansprechen führt. Als weiterer Ansatz wurden HER2-positive Mammatumoren untersucht. Hier wurde zum einen der Zusammenhang zwischen Ansprechen auf eine neoadjuvante Chemotherapie plus Trastuzumab im Mammakarzinom und der Expression von HER2 auf mRNA-Ebene mittels quantitativer Real-Time PCR (qRT-PCR) analysiert. Es konnte eine Korrelation zwischen HER2-mRNA Level und Ansprechen auf Trastuzumab innerhalb der Östrogenrezeptor- positiven Patientinnen festgestellt werden. Weiterhin wurde der PIK3CA-Genotyp als Prädiktor für das Ansprechen auf neoadjuvante Chemotherapie in Kombination mit einer anti-HER2-Therapie durch Sequenzierung untersucht, mit dem Ergebnis, dass Patientinnen mit mutierten Tumoren mit einer geringeren Wahrscheinlichkeit auf die Therapie ansprachen. Zusammengefasst konnten in dieser Arbeit Biomarker auf genomischer Ebene und auf verschiedenen Ebenen der Translation evaluiert und ihr prädiktiver Wert für das Therapieansprechen gezeigt werden. Die klinische Bedeutung unserer Ergebnisse muss in weiteren Studien belegt werden.","The evaluation of biomarkers with the aim of providing the individual patients with the best possible therapy is an integral part of pathological diagnostics. The primary goal of this study was to examine biomarkers predictive for response to neoadjuvant chemotherapy. For this purpose, the predictive potential of the SPARC protein was analyzed by immunohistochemistry in patients with breast cancer who received an adjuvant chemotherapy and also in pancreatic cancer treated with adjuvant therapy with gemcitabine. SPARC appears to be of potential as a biomarker in both tumor entities. In breast cancer tumors, a high expression of SPARC was independently predictive for response to chemotherapy, especially in triple negative tumors. In pancreatic cancer, SPARC had a negative prognostic impact. As SPARC is able to bind albumin, this could be of special interest in respect to a therapy with nab- paclitaxel, which was proven to lead to a higher therapy response in breast, as well as in pancreatic cancer. In a further approach, HER2-positive breast cancer was investigated. We evaluated a connection between response to neoadjuvant therapy plus trastuzumab and HER2-RNA levels measured by qRT-PCR. A significant correlation could only be seen within the estrogen receptor- positive patients. Furthermore, the PIK3CA genotype was investigated via sequencing as a possible predictor for response to neoadjuvant chemotherapy in combination with anti-HER2 therapy. Patients with mutated tumors were less likely to respond to therapy. In summary, we were able to evaluate biomarkers on the genomic as well as translational level and show their predictive value for chemotherapy response.
