Die Belastungs- und Beanspruchungsforschung im Rettungsdienst wird von Arbeiten dominiert, die mehrheitlich auf psychisch hochbelastende, potenziell traumatisie-rende Einsätze und ihre pathologischen Folgen, wie z. B. die Posttraumatische Belastungs-störung, fokussieren. Nur wenige Untersuchungen beleuchteten arbeitsorganisatorische Rahmenbedingungen im Rettungsdienst und ihre Bedeutung für Merkmale der arbeitsbezo-genen Gesundheit und des Engagements. Die Untersuchung der organisationalen Realität in der Wahrnehmung von Einsatzkräften (Organisationsklima) und die Bedeutung der Konstel-lation von Rahmenbedingungen in Organisationen (Anforderungen und Ressourcen: Organi-sationsprofile) erfolgte im Rettungsdienst bisher nicht. Die vorliegende Untersuchung basiert auf Annahmen der Organisationsklimaforschung (Organization Work Health System Model, Shoaf, Genaidy, Karwowski & Huang, 2004) und der arbeits- und organisationspsychologi-schen Stress- sowie Burnoutforschung (Job-Demand- Control-Modell, Karasek & Theorell, 1990, Job-Demands-Resources-Model, Demerouti, Bakker, Nachreiner & Schaufeli, 2001, Effort-Reward-Imbalance- Model, Siegrist, 2000). Beide Forschungsgebiete weisen einen breiten Überschneidungsbereich auf, sind jedoch inhaltlich weitgehend unverknüpft. Unter-suchungen der Organisationsklimaforschung betrachteten das Muster von Rahmenbedin-gungen in Organisationen in der Wahrnehmung von Organisationsmitgliedern und dessen Bedeutung für ökonomische Parameter. Nur selten interessierten gesundheitliche Effekte des Organisationsklimas. Studien innerhalb der arbeits- und organisationspsychologischen Stress- und Burnoutforschung betrachteten häufig vergleichbare Rahmenbedingungen in der Organisation, wie Arbeiten innerhalb der Organisationsklimaforschung. Diese wurden meist im Einzelnen betrachtet und nicht hinsichtlich der Bedeutung ihrer Konstellation für die Ge-sundheit von Organisationsmitgliedern. In der vorliegenden Untersuchung werden als Merk-male des Organisationsklimas Anforderungen und Ressourcen der Arbeit in Organisationen untersucht: Quantitative Anforderungen, Kontrolle, Gratifikation, Fairness, Vorgesetztenverhalten, Kommunikationskultur, Teamwork und Zusammenhalt im Team in der Wahrnehmung der Einsatzkräfte und ferner der Sense of Coherence als Personenmerkmal. Sie gehen als unabhängige und als potenzielle Moderatorvariablen (Ressourcen der Organisation/Person) in die vorliegende Studie ein. Als abhängige Variablen werden Burnout, Engagement und die Verbundenheit mit der Organisationseinheit und Dienststelle (Commitment) sowie das sub-jektive körperliche Wohlbefinden untersucht. Fragestellung. Die vorliegende Untersuchung geht zwei Kernfragestellungen nach: Erstens, ist es anhand von Organisationsklimamerkmalen möglich, Gruppen von Organisationseinhei-ten und Dienststellen im Rettungsdienst zu bilden, die sich bei ihrer Anforderungs- und Res-sourcenkonstellation sowie bei der Ausprägung von Merkmalen der arbeitsbezogenen Ge-sundheit unterscheiden? Zweitens, welche Bedeutung haben Anforderungen und Ressourcen sowie ihre Konstellation innerhalb der Organisationsprofile für subjektive Merkmale der Gesundheit im Rettungsdienst? Methoden. Insgesamt 1.048 Einsatzkräfte im Rettungsdienst wurden schriftlich per Frage-bogen befragt. Diesen sendeten 316 (30,2%) Einsatzkräfte aus 21 Organisationseinheiten und Dienststellen ausgefüllt zurück. Der Fragebogen setzt sich aus folgenden Skalen und Messinstrumenten zusammen: Organizational Check-up Survey, Maslach Burnout Inventory-General Survey, Instrument zur Erfassung von Merkmalen der Arbeitstätigkeit im Einsatzwe-sen, Utrecht Work Engagement Scale, Organizational Commitment Questionaire, Fragebo-gen zur Erfassung des körperlichen Wohlbefindens. Zur Beantwortung der Fragestellungen werden folgende Analysemethoden angewendet: Hierarchische und Clusterzentrenanalyse zur Gruppenbildung, Regressionsanalyse mit Dummyvariablen zur Untersuchung von Zu-sammenhängen zwischen der Gruppenzugehörigkeit und Merkmalen der arbeitsbezogenen Gesundheit, konfirmatorische Pfadanalysen mit AMOS 16.0 zur Analyse der Bedeutung von Anforderungen und Ressourcen für Burnout, Wohlbefinden, Engagement und Commitment sowie potenzieller Moderatoreffekte von Ressourcen der Organisation und Person auf Zu-sammenhänge zwischen Arbeitsanforderungen und Burnout. Diskussionslinien der gesund-heitsbezogenen Burnout- und der Organisationskultur- und -klimaforschung werden auf der Basis von systematischen Literaturrecherchen und einer anschließenden Literaturanalyse des recherchierten Materials herausgearbeitet. Ergebnisse. Vier Gruppen von Organisationseinheiten und Dienststellen, die sich signifikant bei der Ausprägung von Anforderungen und Ressourcen unterscheiden, konnten identifiziert werden: (1) Anforderungsreich und ressourcenreich – ausgewogen, (2) anforderungsarm und ressourcenreich – günstig, (3) anforderungsreich und ressourcenarm – ungünstig, (4) sehr anforderungsreich und ressourcenarm – sehr ungünstig. Die vierte Gruppe wird wegen der sehr kleinen Stichprobe von n= 3 Einsatzkräften aus weiteren Analysen ausgeschlossen. Signifikante Unterschiede zwischen den Gruppen zeigten sich ausschließlich bei der Wahr-nehmung von Ressourcen der Organisation, wie Kontrolle, Gratifikation, Fairness, Vorge-setztenverhalten und Kommunikationskultur. Die Zugehörigkeit zu Organisationseinheiten und Dienststellen mit ausgewogenem und günstigem Organisationsklima erhöht die Chan-cen für eine hohe Verbundenheit mit der Organisation und senkt offenbar das Risiko durch die Arbeit auszubrennen (Komponente Erschöpfung). Burnoutrelevant sind hohe quantitative Arbeitsanforderungen (g= 0,45) und Einsätze, die z. B. durch körperliche Angriffe oder sehr engen Kontakt mit dem Patienten geprägt sind (g= 0,24). Ein hohes Erschöpfungsausmaß (Burnout) hängt signifikant und eng mit einem geringeren körperlichen Wohlbefinden zu-sammen (b= -0,66). Günstige Auswirkungen auf das körperliche Wohlbefinden scheint die Wahrnehmung einer hohen professionellen Effizienz zu haben (b= 0,26). Commitment wird stark durch Zynismus negativ beeinflusst (b= -0,65). Ressourcen der Organisation, wie Grati-fikation/Fairness (g= 0,41), Vorgesetztenverhalten (g= -0,32), Kommunikationskultur (g= 0,39) und Teamparameter (g= 0,31) variieren signifikant gemeinsam mit dem Commitment, ein hohes Commitment scheint Prädiktor für höheres Engagement zu sein (b= 0,53). Alle untersuchten Pfadmodelle weisen, z. T. nach Modifikation, eine befriedigende Anpassung zu den Daten auf. Bei den anschließenden Moderatoranalysen wurden potenzielle Puffereffekte auf Zusammenhänge zwischen Anforderungen und Burnout deutlich. Von Bedeutung sind ins-besondere Kontrolle, Gratifikation und Fairness. Im Burnoutkontext sind Vorgesetztenverhalten, Kommunikationskultur, Teamwork und Zusammenhalt im Team bedeutsam. Diskussion. Die Ergebnisse stehen weitgehend im Einklang mit den Annahmen der zugrun-deliegenden Theorien. In Gruppen mit einer günstigen Ressourcenausstattung sind gesün-dere Einsatzkräfte mit einer höheren Verbundenheit anzutreffen. Burnout wird maßgeblich durch quantitative Überforderungen und in geringerem Ausmaß durch eine hohe persönliche Betroffenheit erklärt. Die untersuchten Arbeitsanforderungen stehen insbesondere mit der Burnoutkomponente Erschöpfung im Zusammenhang. Das körperliche Wohlbefinden wird wahrscheinlich durch Arbeitsanforderungen, vermittelt über Burnout, beeinflusst. Dabei geht der größte Einfluss von der Burnoutkomponente Erschöpfung aus. Gratifikation, Fairness, Kommunikationskultur und ein guter Zusammenhalt im Team haben offenbar eine maßgebli-che Bedeutung für das Commitment. Das Engagement ist umso höher, je größer das Com-mitment ist. Ausgehend von diesen Ergebnissen und denen der Moderatoranalysen zeich-nen sich folgende Handlungsebenen für die gesundheitsförderliche Gestaltung der Arbeit im Rettungsdienst ab: Fortführen bereits bestehender Angebote zur Vorbereitung auf belasten-de Einsätze, selektive Erweiterung von Kontrolle und Handlungsspielräumen unter klarer Bezugnahme auf die Arbeit außerhalb von Einsätzen, Ausbau von Rückmelde- und Feedbacksystemen, eine transparente und faire Ressourcenverteilung (Aufstiegschancen, Zu-gang zu Aus-, Fort- und Weiterbildung, Bezahlung), selektive Delegation von Verantwortung durch Vorgesetzte auf Einsatzkräfte, Gestaltung einer transparenten und berechenbaren Informationsweiterleitung und das Ermöglichen einer verlässlichen Zusammenarbeit in funktionierenden Teams.","Introduction. Research into the stress and demands of ambulance services is dominated by studies which mainly focus on psychologically highly stressful traumatizing emergencies and their pathological consequences such as posttraumatic stress disorder. Little research has been done on specific aspects of working conditions that the ambulance services are enduring and their importance for the characteristics of work-related health as well as for the indicators of commitment. So far, research aimed at investigating the extensive organizational reality as it is perceived by the ambulance service personnel (organizational climate) and exploring the importance of the combination of framework conditions within organizations, job demands and resources, organizational profiles, has not been carried out. The present study is based on assumptions of organizational climate research (Organization Work Health Sys-tem Model, Shoaf, Genaidy, Karwowski & Huang, 2004) as well as on work-related psychological and organizational-psychological research into stress and burnout (Job-Demand-Control-Modell, Karasek & Theorell, 1990, Job- Demands-Resources-Model, Demerouti, Bakker, Nachreiner & Schaufeli, 2001, Effort-Reward-Imbalance-Model, Siegrist, 2000). The two fields of research overlap in many aspects, however in terms of content they are not connected for the most part. Investigations in connection with organizational climate research focus on the pattern of organizational environments as perceived by members of the organi-zation as well as on its importance for economic parameters. In this light, the effects of the organizational climate on health have rarely been of interest. Studies carried out within work-related psychological and organizational-psychological research into stress and burnout of-ten examine comparable organizational environments just like studies relating to organizational climate research. However it was mostly the case that these environments were investigated individually and without regard to their constellation for the health of members of the organization. The present study aims at investigating job demands and job resources which arise from working in organizations as characteristics of the organizational climate including quantitative demands, control, reward, fairness, supervision, communication culture, teamwork and team cohesion as perceived by the ambulance service personnel, as well as the person variables of sense of coherence. They are incorporated as independent variables and as potential moderator variables in the present study whereas burnout, engagement and organizational commitment and subjective physical well-being are investigated as dependent variables. Question. The present study addresses two key questions: first, is it possible in the ambu-lance service to form organizational units and service departments into groups by identifying organizational climate characteristics where the groups differ in terms of job demand and job resource constellations as well as in the development of characteristics regarding work-related health.Second, what importance do demands, resources and their constellation (or-ganizational profile) play when it comes to subjective characteristics of health in ambulance services. Methods. A total number of 1.048 ambulance service personnel were invited in writing to fill out a questionnaire where 316 (30.2%) of them, working in 21 organizational units and ser-vice departments, completed and returned the questionnaire. Tried and tested measure-ments were used to create the questionnaire: Organizational Check-up Survey, Maslach Burnout Inventory- General Survey, Instrument to Collect Work Characteristics in Ambulance Services, Utrecht Work Engagement Scale, Organizational Commitment Questionnaire, Questionnaire on the Assessment of Physical Well-being: FEW 16. In order to answer the questions, the following analysis methods were applied: hierarchical clustering analysis and partitional clustering analysis (formation of groups), regression analysis with dummy vari-ables to investigate the correlation between group affiliation and characteristics of work-related health, structural equating modeling with AMOS 16.0 to analyze the importance of job demands and job resources regarding burnout, well-being, engagement, commitment as well as research of potential moderating effects and the person with a view to identifying correla-tions between job demands and burnout. Discussion threads on health-related burnout, or-ganizational culture and organizational climate research were elaborated on the basis of sys- tematic literature research and by subsequently carrying out a qualitative content analysis of the researched material. Results. Four groups of organizational units and service departments demonstrating signifi-cant differences in the development of job demands and job resources could be identified: (1) high in demand and rich in resources – balanced, (2) low in demand and rich in resources – favorable, (3) high in demand and low in resources – unfavorable, (4) very high in demand and very low in resources – very unfavorable. The fourth group is not included in the further analysis due to the very small partial sample (n = 3 staff members. Significant differences between the groups could be exclusively observed in the perception of resources of the or-ganization (control, reward, fairness, supervision, communication culture). Belonging to an organizational unit/service department with a balanced and favorable organizational climate increases the possibility of developing a strong commitment to the organization while at the same time diminishing the risk of suffering from work burnout (exhaustion component). Fac-tors that lead to burnout are high quantitative job demands (g= 0,45) and assignments char-acterized by physical assaults or very close contact to patients (g= 0,24). A high level of ex-haustion (burnout) is significantly correlated to a low physical well-being (b= -0,66). However the awareness of a high professional efficiency seems to have a positive impact on the physical well-being (b= 0,26). Cynicism has a substantially negative influence on commit-ment (b= -0,65). Resources of the organization, reward/fairness (g= 0,41), supervision (g= -0,32), communication culture (g= 0,39) and team parameters (g= 0,31), significantly vary together with the commitment, where a strong commitment seems to be a predictor of a higher engagement (b= 0,53). It can be said that the tested models fit the data well. In the following moderator analyses, indications were that buffer effects existed regarding correla-tions between job demands and burnout (in this case control, reward and fairness are par-ticularly significant) as well as the burnout context (based on supervision, communication culture, teamwork and team cohesion). Discussion. The results are in line with the assumptions determined by the underlying theo-ries. Groups enjoying an adequate and convenient availability of resources are staffed with healthier ambulance service personnel who also show a stronger commitment. Burnout is significantly related to quantitative excessive demands and, to a lesser extent, associated with a high personal concern or empathy. The investigated job demands are particularly cor-related with exhaustion as a burnout component. The physical well-being is likely to be influ-enced, via burnout, by job demands with exhaustion as a burnout component having the largest impact. Reward, fairness, communication culture and a good team cohesion appar-ently play a decisive role when in comes to commitment. The higher the commitment, the higher the level of engagement. Based on these results and the outcome of the moderator analysis, the following action levels can be identified with a view to creating health promoting work conditions in the ambulance services.
