Debus","m","2012-02-24","2018-06-07T18:53:59Z","2012-02-01T09:20:43.305Z","2012","Hintergrund und Ziele der Arbeit: Stenosierende kardiovaskuläre Erkrankungen gehören zu den häufigsten Todesursachen in Deutschland und anderen Industrienationen. Neben der medikamentösen und invasiven Therapie stellt die Induktion eines „natürlichen Bypasses“ durch Arteriogenese eine potentielle Therapiealternative dar. Da das Schwein eine im Vergleich zum Menschen ähnlich schwache koronare Kollateralisierung aufweist, bietet es ein interessantes Modell zur Untersuchung von koronarer Arteriogenese. Die Zielsetzung der vorliegenden Arbeit ist daher folgende: 1\. Methodenetablierung der postmortalen Koronarangiographie und der Korrosionsmethode. 2\. Untersuchung der Frage, ob präexistente Kollateralen im nativen Schweineherzen nachweisbar sind und ihr Wachstum durch die Induktion von Arteriogenese mit Hilfe eines semi-akut okkludierenden Kupferstents angeregt werden kann. Anschließend wird die Kollateralisierung im Schweineherzen analysiert. 3\. Untersuchung des Ursprungs, des zeitlichen Entwicklungsverlaufs und der Histomorphologie von Brückenkollateralen im Schweineherzen. 4\. Bewertung, ob das Tiermodell und die etablierte Methodik geeignet sind, den Prozess der Arteriogenese darzustellen. Methoden: Im I. Teil wurden 34 Herzen von Schlachthofschweinen angiographisch untersucht. Nach der Etablierung dreier verschiedener Versuchsaufbauten erfolgte eine Vorperfusion der Koronararterien mit einer Kochsalzlösung und die Kontrastmittel (KM)-Perfusion mit vier verschiedenen Kontrastmitteln, drei auf Basis von Bariumsulphat, eines auf der von Bismuth- Oxychlorid. Anschließend wurden die Herzen mit Hilfe der Mammographietechnik radiologisch untersucht und stereoskopische Bildpaare mit Hilfe von vier unterschiedlichen Versuchsaufbauten angefertigt. Für die Etablierung der Korrosionsmethode wurden die Koronararterien von sechs Schweineherzen mit einem Acrylharz injiziert und in Kalilauge korrodiert. Im II. Teil wurde bei 15 Schweinen (Deutsche Landrasse x Deutsches Edelschwein) interventionell ein Kupferstent in die linke Koronararterie implantiert. Nach 56 Tagen wurden die Tiere finalisiert, die Herzen mit Hilfe der etablierten postmortalen Angiographie und Stereoskopietechnik untersucht und abschließend Micro-CT- Aufnahmen angefertigt. Im III. Teil wurde bei elf weiteren Schweinen interventionell ein Kupferstent in die linke Koronararterie implantiert. Die Tiere wurden zu fünf unterschiedlichen Zeitpunkten nach Stentimplantation finalisiert: 0.5 Tage (D0.5) (n=2), D3 (n=2), D5 (n=1), D7 (n=3) und D12 (n=3). Anschließend wurden Untersuchungen mit Hilfe der postmortalen Angiographie, Stereoskopie und dem Micro-CT durchgeführt. Zusätzlich wurde die Stentregion histologisch (HE-Färbung) und immunhistologisch (KI-67-, CD163- und SMC-Marker) untersucht. Ergebnisse: I. Teil: Mit der postmortalen Angiographie können unter Verwendung des Versuchsaufbaus Nr. 3 und des Bariumsulphat-KM 3 arterielle Gefäße bis zur 6. Ordnung dargestellt werden. Mit Hilfe eines Tischs, mit dem das Herz in einer Ebene verschoben wird, ist es möglich, stereoskopische Bilder anzufertigen. Mit dem Acrylharz können detailreiche Korrosionspräparate angefertigt werden. II. Teil: Präexistente Kollateralen sind im nativen Schweinherzen mit Hilfe von Korrosionspräparaten nachweisbar. Diese sind induzierbar und bilden früh nach Stentimplantation langstreckige, inter- und intrakoronare Kollateralen aus, die vor allem über das Moderatorband verlaufen. Diese werden von hämodynamisch günstigeren, kurzstreckigen Brückenkollateralen ab D7 und von intrakoronaren Kollateralen ab D12 abgelöst. III. Teil: Brückenkollateralen stammen vom longitudinal verlaufenden Typ 1 Vasa Vasorum (VV) ab. Die Entwicklung von VV zu Brückenkollateralen verläuft innerhalb von 7 Tagen nach Stentimplantation Die Brückenkollateralen weisen glatte Muskelzellen auf und sind daher funktionell relevant. IV. Teil: Das Tiermodell ist gut geeignet, um koronare Arteriogenese zu induzieren. Die angewandten Methoden bilden aber jeweils nur einen Teil des Arteriogeneseprozesses im Schwein ab. Erst im Zusammenspiel der Instrumente lässt sich der gesamte Umfang des Arteriogeneseprozesses nachvollziehen. Schlussfolgerung: In der vorliegenden Arbeit konnte die hochauflösende postmortale Angiographie in Verbindung mit der Stereoskopie- und Micro-CT- Technik sowie die Korrosionsmethode etabliert werden. Durch einen nicht- invasiven, semi-akuten Gefäßverschluss mit Hilfe eines Kupferstents wurde Arteriogenese bei präexistenten Kollateralen induziert und die vor allem superfiziale Kollateralisierung im Schwein analysiert. Es konnte zum ersten Mal gezeigt werden, dass sich VV durch Arteriogenese mit einer bisher im Schwein nicht beschriebenen Wachstumsdynamik zu Brückenkollateralen entwickeln, die funktionell relevant sind. Dieses Tiermodell und die Summe der Methoden sind daher gut geeignet, um Arteriogenese unter Bedingungen, die dem Menschen ähnlich sind, zu untersuchen. Die vorliegenden Ergebnisse könnten daher wichtig sein für weitere Studien zur Behandlung der koronaren Herzkrankheit und peripheren arteriellen Verschlusskrankheit durch die therapeutische Stimulation der Arteriogenese zur Induktion von Kollateralwachstum.","Background and objectives: The coronary heart disease still belongs to the leading causes of death in Germany and other industrialized nations. Besides the drug and invasive therapy, the induction of a ""natural bypass"" by arteriogenesis could be a potential alternative therapy. Since the pig heart has a similar weak coronary collateralization as the human, the pig is an interesting model for investigation of coronary arteriogenesis. Therefore, the objective of this study is the following: 1\. Establishment of methods for the post-mortem coronary angiography and the corrosion method. 2\. Investigation, if pre-existing collaterals are detectable in native pig hearts and if their growth can be stimulated by the induction of arteriogenesis using a semi-acute occluding copper stent. Finally, the collateralization in the pig heart is analyzed. 3\. Investigation of the origin, the time course of the development and the histo-morphology of bridging collaterals in pig hearts. 4\. Evaluation if the animal model and the established methods are suitable to visualize the arteriogenesis process. Methods: In part I 34 pig hearts were investigated angiographically. After the establishment of three different experimental setups a pre-perfusion of the coronary arteries with saline was performed followed by a contrast media (CM)-perfusion with four different contrast agents; three on the basis of barium sulphate, one of bismuth oxychloride. Then, the hearts were examined using the mammography technique and stereoscopic image pairs were done using four different experimental setups. To establish the corrosion method coronary arteries of six pig hearts were injected with an acrylic resin and corroded in potash lye. In part II in 15 pigs a copper stent was minimally invasive implanted into the left coronary artery. After 56 days the animals were finalized, the heart examined using the established post-mortem angiography and stereoscopic technique and finally micro-CT images were done. In part III in 11 pigs a copper stent was minimally invasive implanted into the left coronary artery. The animals were grouped into five different finalization days: 0.5 days after stent implantation (D0.5) (n = 2), D3 (n = 2), D5 (n = 1), D7 (n = 3) and D12 (n = 3). Subsequently, investigations using postmortem angiography, stereoscopy and micro-CT were performed. Finally, the stent region was examined using histology (HE staining) and immunohistochemistry (KI-67, CD163 and SMC marker). Results: I. part: With the post-mortem angiography using the experimental setup No. 3 and the barium sulphate-CM 3 arteries up to the 6th order can be investigated. Using a table, with which the heart is moved in a plane, it is possible to produce stereoscopic images. With the acrylic resin, detailed corrosion preparations are made. II. part: Pre-existent collaterals are detectable in native pig heart using the corrosion method. These collaterals can be induced by arteriogenesis and form early after stent implantation long inter-and intracoronary collaterals, which run mainly over the moderator band. From day 7 on they are replaced by hemodynamic favorable short bridging collaterals and from D12 on by short intracoronary collaterals. III. part: Bridging collaterals originate from longitudinally running type 1 vasa vasorum (VV). The development of VV to bridging collaterals proceeds within 7 days after stent implantation; the bridging collaterals have smooth muscle cells and are therefore functionally relevant. IV. part: The animal model is well qualified to induce coronary arteriogenesis. Every single used method reflects only one part of the whole arteriogenesis process. Only in the interplay of the instruments the entire scope of the arteriogenesis process can be investigated. Conclusion: In the present study the high resolution postmortem angiography in combination with the three dimensional and micro-CT technique and the corrosion method could be established. Arteriogenesis was induced in pre-existing collaterals by a non invasive, semi-acute vessel occlusion using a copper stent and the mainly superficial collaterals in the pig were analyzed. For the first time it was demonstrated that VV develop induced by arteriogenesis with a hitherto not described growth dynamic to bridging collaterals which are functionally relevant. The animal model and the sum of the methods are therefore well qualified to study arteriogenesis under conditions which are similar in humans.
