Dabei sind von 64.317 Einsendungen in die Herzpathologie 236 Neoplasien (0,4%) identifiziert worden. Im gleichen Zeitraum sind 31.515 kardiochirurgische Eingriffe unter Einsatz der Herz-Lungen-Maschine durchgeführt worden; das entspricht unter 1% (0,8%) bei allen erfassten Tumoren. Die Mehrzahl der Betroffenen sind weiblich (n=142, 61%) mit einem Durchschnittsalter von 56 Lebensjahren. Benigne Neoplasien repräsentieren die Mehrzahl der Fälle (n=177, 75%). Das häufigste gutartige Neoplasma bei den Erwachsenen (n=228) ist das Myxom (n=103, 58%) und bei den Kindern und Jugendlichen (n=8) die Rhabdomyome (n=3, 38%) und Teratome (n=3, 38%). Myxome sind die häufigsten kardiochirurgisch relevanten Neoplasien, die in 44% (103 Patienten) in dieser Studie nachgewiesen worden sind. Sie sind überwiegend im linken Vorhof (n=93, 90%) lokalisiert. In ca. 6% finden sich die Myxome im rechten Vorhof; andere Lokalisationen sind der linke Ventrikel und die Mitralklappe. In zwei Fällen ist die Lokalisation trotz intensiver Aktenrecherche (digital und/oder konventionell) unbekannt geblieben. Frauen (n=74, 72%) sind häufiger betroffen als Männer (n=29, 28%). Das Durchschnittsalter beträgt 57 Jahre und die Alterspanne liegt zwischen 24 und 80 Jahren. Maligne Neoplasien (n=59) machen ein Viertel (25%) der Gesamtanzahl der untersuchten Neubildungen aus. Hierbei bilden Sarkome (n=18, 32%), Metastasen (n=14, 25%), Karzinome und NET (n=10, 18%) und Tumoren des hämatopoietischen Systems (n=9, 16%) die Mehrheit. Die Gruppe der Metastasen beinhaltet zu beinahe gleichen Anteilen Tochtergeschwülste von Sarkomen (n=7) und Karzinomen (n=6) sowie von einem Keimzelltumor (n=1). Zusätzlich finden sich folgende ausgewählte Fallbeschreibungen aus der Studie: Myxom mit Metastase, multiple Rhabdomyome, malignes Paragangliom, metastasierendes Teratokarzinom des Hodens, Mesotheliom, gut differenziertes Karzinoid. Kardiochirurgisch relevante Neoplasien im Brustraum stellen nach wie vor eine Rarität unter allen Neoplasien dar. Die Therapie hängt weniger von der Dignität als von der Größe und der Lokalisation ab. Hier gilt die komplette Resektion bei benignen, solide umschriebenen Neoplasien als kurativ.","Between 2000 and 2010, the Deutsches Herzzentrum Berlin (German Heart Institute Berlin) – clinic for cardiac surgery, thoracic surgery and vascular surgery – more precisely the area of work concerned with cardiac pathology, carried out a retrospective study about cardio-surgical relevant neoplasms in the thorax, including the neoplasms in the large blood vessels. Out of 64.317 submissions to the pathology, 236 neoplasms (0,4%) have been identified. 31.515 cardio-surgical operations using the heart-lung machine have been conducted over the same period of time; this correspond to less than 1% (0,8%) of all recorded tumours. The majority of the persons affected are female (n=142, 61%) with an average age of 56 years. Benign neoplasms represent the majority of the cases (n=177, 75%). The myxoma is the most frequent benign neoplasm (n=103, 58%) among adults (n=228), while rhabdomyomas (n=3, 38%) and teratomas (n=3, 38%) are the most common ones among children and adolescents (n=8). Myxomas are the most frequent cardio-surgical relevant neoplasms, which have accounted for 44% (103 patients) in this study. They are mainly located in the left atrium (n=93, 90%). In approximately 6% of the cases the myxomas are to be found in the right atrium; other locations are the left ventricle and the mitral valve. In two cases the location (of the myxomas) remains unknown despite intensive file search (digital and/or written from). Women (n=74, 72%) are more frequently affected than men (n=29, 28%). The average age is 57 years and the age ranges between 24 and 80 years. Malign neoplasms (n=59) account for a quarter (25%) of the total amount of the examined neoplasms. Sarcomas (n=18, 32%), metastases (n=14, 25%), carcinomas and neuronendocrine tumours (n=10, 18%) and tumours of the hematopoietic system (n=9, 16%) form the majority of malign neoplasms. The group of metastases includes to almost equal shares daughter tumours of sarcomas (n=7) and carcinomas (n=6) as well as one germ cell tumour (n=1). Additionally, the following selected cases are to be found in the study: myxoma with metastasis, multiple rhabdomyomas, malign paraganglioma, metastasized teratocarcinoma of the testicles, mesothelioma, well differentiated carcinoid. Cardio-surgical relevant neoplasms in the thorax still represent a rarity among the total number of neoplasms. The therapy is less depending on the dignity than on the size and the location of the respective neoplasm.
