Anhang","Einleitung: Die Untersuchung von Zytokinen und Chemokinen hat in den letzten Jahren zu einer rasanten Entwicklung unseres Verständnisses von Entzündungsreaktionen geführt. Für die chronische Rhinosinusitis (CRS) konnten bestimmte Zytokine und Chemokine definiert werden, die offenbar am Pathomechanismus der Erkrankung zentralen Anteil haben. Ziel dieser Arbeit war die Untersuchung CRS relevanter Chemokine und Zytokine und des auf eosinophilen Granulozyten dominierenden Chemokinrezeptors 3 (CCR3) anhand eines genau charakterisierten und damit vergleichbaren Patientengutes unter besonderer Berücksichtigung ätiologischer Faktoren. Methoden: Bei 30 Patienten mit chronisch hyperplastischer Pansinusitis (vergleichbarer CT-Score/Grad der Eosinophilie) und 10 Kontrollen wurde der mRNA Gehalt für CCR3, die CC- Chemokine Eotaxin und RANTES und die Zytokine IL-4, IL-5, IFN-γ und IL-10 mittels Real-Time TaqMan PCR in polypösem Gewebe (Patienten) bzw. der Concha inferior (Kontrollen) ex vivo quantitativ analysiert. Die Daten der Patientengruppe wurden im Vergleich zur Kontrollgruppe und innerhalb der Patientengruppen zwischen Gruppen mit verschiedenen ätiologischen Faktoren (Allergie, ASS-Intoleranz) ausgewertet. In einer weiteren Studie wurde die Proteinkonzentration verschiedener CCR3- bindender Chemokine (Eotaxin, Eotaxin-2, Eotaxin-3, RANTES, MCP-3, MCP-4) in Gewebeproben von Patienten mit CRS (n=24) und Kontrollen aus der Concha inferior (n=8) mittels ELISA untersucht und zwischen Patientengruppen ohne/mit Allergie bzw. ASS-Intoleranz verglichen sowie mit dem Grad des Gewebeeosinophilie korreliert. Ergebnisse: In der Patientengruppe ließ sich eine signifikant erhöhte mRNA Expression für CCR3 (p<0.001), Eotaxin (p=0.001), IL-10 (p=0,003), IL-4 (p=0,04) und IL-5 (p<0.001) gegenüber der Kontrollgruppe nachweisen. Die CCR3- und die Eotaxin mRNA Expression korrelierten signifikant (p=0.01). Bei Allergikern war die IL-4 mRNA Expression gegenüber nicht allergischen Patienten (p=0,008) und Patienten mit ASS-Intoleranz (p=0,032) signifikant erhöht. Dagegen war die IFN-γ mRNA Expression bei den nicht allergischen Patienten (p=0,01) und ASS- Intoleranten (p=0,029) gegenüber den Allergikern signifikant erhöht. CCR3, Eotaxin und IL-5 waren bei allen Patienten vergleichbar hochreguliert. Die Untersuchung der Proteinkonzentrationen ergab für Eotaxin (p=0,001), Eotaxin-2 (p=0,007), Eotaxin-3 (p= 0,001) sowie MCP-4 (p=0,004) signifikant erhöhte Werte der Patientengruppe im Vergleich zur Kontrollgruppe. Zudem korrelierten diese Werte mit der Anzahl der Eosinophilen (p<0,01). Eotaxin-3 wurde erstmals in polypösem Nasengewebe nachgewiesen. Die RANTES-Proteinkonzentration war unter allen untersuchten Chemokinen am höchsten, jedoch ohne signifikanten Unterschied zur Kontrollgruppe und ohne Korrelation zur Eosinophilie. Unterschiede in der Proteinkonzentration zwischen ätiologischen Gruppen waren mit signifikanten Unterschieden im Grad der Eosinophilie assoziiert. Schlussfolgerungen: Die Ergebnisse weisen auf eine Schlüsselrolle der CCR3 /Eotaxin-Interaktionen bei der selektiven Rekrutierung eosinohiler Granulozyten und in der Pathogenese der chronisch hyperplastischen Rhinosinusitis hin. Unterschiede im Zytokin- Chemokinprofil sind möglicherweise auf differenzierte Mechanismen bei der Entstehung der CRS und verschiedenen ätiologischen Faktoren zurückzuführen. Die Daten stützen die Hypothese einer multifaktoriellen Genese der CRS, wobei es durch verschiedene Faktoren zur Aktivierung von Zellen, zur Freisetzung entzündlicher Mediatoren und zur selektiven Migration von Eosinophilen kommt. Die simultane Expression verschiedener Chemokine oder das Erreichen einer bestimmten Konzentrationsschwelle ist möglicherweise eine Voraussetzung für den starken Eosinophileneinstrom. Untersuchungen verschiedener Entzündungsmediatoren könnten in Zukunft dazu beitragen, verschiedene Nasennebenhöhlen- Erkrankungen besser zu differenzieren und pathophysiologisch einzuordnen. Die Untersuchung einzelner Mediatoren ist zudem wichtig für die Identifizierung selektiver Therapieziele.","Introduction: The study of cytokines and chemokines in recent years has triggered a rapid improvement in our understanding of inflammatory reactions. For chronic rhinosinusitis (CRS), certain cytokines and chemokines were defined, which obviously play a central role in the pathomechanism of the disease. The objective of this paper was to study both CRS-relevant chemokines and cytokines and the chemokine receptor 3 (CCR3) that is dominant on eosinophils. This was achieved by means of precisely characterized and thus comparable case material, with special consideration being given to etiological factors. Methods: In 30 patients with chronic hyperplastic pansinusitis (comparable CT score/degree of eosinophilia) and 10 controls, the mRNA-content for CCR3, CC-chemokines eotaxin and RANTES and cytokines IL-4, IL-5, IFN-γ and IL-10 were quantitatively analysed by means of real-time TaqMan PCR in polypous tissue (patients) or the concha inferior (controls) ex vivo. Data obtained from the patient group were compared with those from the control group and, within the patient group, assessed between groups featuring different etiological factors (allergy, ASS-intolerance). In another study, the protein concentration of various CCR3-binding chemokines (eotaxin, eotaxin-2, eotaxin-3, RANTES, MCP-3, MCP-4) in tissue samples taken from CRS patients (n=24) and controls from the concha inferior (n=8) were analysed by means of ELISA; they were compared between the patient groups without/ with allergy or ASS-intolerance and correlated with the degree of tissue eosinophilia. Results: Significantly increased mRNA expression for CCR3 (p<0.001), eotaxin (p=0.001), IL-10 (p=0.003), IL-4 (p=0.04) and IL-5 (p<0.001) in the patient group compared to the controls was detected. CCR3- and eotaxin mRNA expression correlated significantly (p=0.01). IL-4 mRNA expression in allergic patients was significantly increased compared to that in non-allergic patients (p=0.008) and patients with ASS-intolerance (p=0.032). In contrast to that, IFN-γ mRNA expression in non-allergic patients (p=0.01) and ASS-intolerant patients (p=0.029) was significantly higher than in allergic patients. CCR3, eotaxin and IL-5 were comparably up-regulated in all patients. Analysis of the protein concentrations for eotaxin (p=0.001), eotaxin-2 (p=0.007), eotaxin 3 (p= 0.001) as well as for MCP-4 (p=0.004) showed significantly increased values for the patient group than for the control group. In addition, these values correlated with the number of eosinophils (p<0.01). Eotaxin 3 was detected in polypous nasal tissue for the first time. RANTES protein concentration was the highest of all chemokines analysed, but we did not see a significant difference to the control group nor a correlation with eosinophilia. Differences in the protein concentration between etiological groups were associated with significant differences in the degree of eosinophilia. Conclusions: The findings highlight a key role of the CCR3/ eotaxin interaction in both the selective recruitment of eosinophils and the pathogenesis of chronic hyperplastic rhinosinusitis. Differences in the cytokine-chemokine profile are possibly due to different mechanisms influencing the development of CRS and to various etiological factors. The data corroborate the hypothesis of a multi-factorial genesis of CRS, with various factors triggering the activation of cells, the release of inflammatory mediators and the selective migration of eosinophils. The simultaneous expression of different chemokines or the reaching of a particular concentration threshold may be a prerequisite for the massive influx of eosinophils. The analysis of various inflammation mediators could help to better differentiate diseases of the paranasal sinuses and facilitate their pathophysiological classification in the future.
