Conrad","w","2013-03-22","2018-06-07T18:32:51Z","2013-03-19T08:47:57.285Z","2013","Objective: Aim of this study were to assess a) the diagnostic value of autoantibodies against recombinant ribosomal P0, P1, P2 proteins and their native heterocomplex in SLE, b) their prognostic value, c) clinicolaboratory associations. Patients and methods: Serum samples were obtained from patients with systemic lupus erythematosus (SLE; n=163), rheumatoid arthritis (RA; n=90), systemic sclerosis (SSc; n=66), primary Sjögren’s syndrome (pSS; n=54), and healthy donors (n=100). Disease activity of SLE patients was characterized using the activity index SLEDAI-2000. Serum autoantibodies to recombinant ribosomal P0, P1, P2 proteins and their native heterocomplex were measured by ELISA. Test results were correlated to ACR criteria, SLEDAI-2000, laboratory data and medications of all SLE patients. Results: Sensitivities of 22.0% for anti-RibPR0, 14.9% for anti-RibPR2, 14.3% for anti-RibPNH and 10.7% for anti- RibPR1 autoantibodies were obtained at a specificity of 99%. Anti-RibPR0 has the best diagnostic value among all anti-Rib autoantibodies. 10% of anti-Sm and anti-dsDNA negative sera were positive for anti-RibPR0 at a specificity of 100%. Anti-RibPR0 positive patients had significantly lower lymphocyte counts, and anti-RibPR1 positive patients had higher γ-glutamyltransferase (GGT) levels than their negative counterparts. No specific damage occurred in anti- RibP positive lupus patients compared to a group of age-, gender- and nephritis-matched anti-RibP negative SLE patients within 3 years. Conclusions: The measurement of autoantibodies against ribosomal P proteins improves the diagnosis of SLE and should therefore be considered in upcoming criteria for the diagnosis or classification of SLE. Lymphocytopenia is associated with high titers of anti-RibPR0, and elevated GGT levels with high titers of anti- RibPR1. Anti-RibP autoantibodies have not shown any evidence for a damage prediction in SLE.||Ziel: Zielstellung dieser Arbeit war a) den diagnostischen Wert b) die klinischen und laborchemischen Assoziationen sowie c.) den prädiktiven Wert bezüglich krankheitsassoziierter Schäden von Autoantikörpern gegen die rekombinanten ribosomalen P0, P1, P2 Proteine sowie ihren nativen Heterokomplex beim systemischen Lupus erythematodes (SLE) zu untersuchen. Material und Methoden: Es wurden Serumproben von Patienten mit SLE (n=163), Rheumatoider Arthritis (n=90), Systemischer Sklerose (n=66), primärem Sjögren Syndrom (n=54), und gesunden Probanden (n=100) aquiriert. Alle SLE Patienten wurde retrospektiv ausführlich charakterisiert. Serumautoantikörper gegen rekombinante ribosomale P0, P1, P2 Proteine und ihren nativen Heterokomplex wurden mittels ELISA gemessen. Die Testergebnisse wurden unter anderem mit ACR Kriterien, SLEDAI-2000-Index, Laborwerten und Medikamenten aller SLE Patienten auf Assoziationen getestet. Ergebnisse: Bei einer vergleichbaren Spezifizität von 99% wurden Sensitivitäten von 22.0% für aRibPR0, 14.9% für aRibPR2, 14.3% für aRibPNH und 10.7% für aRibPR1 berechnet. ARibPR0 boten den höchsten diagnostischen Nutzen unter allen aRibs. So waren 10% aller anti-Sm und anti- dsDNS negativen Seren positiv für aRibPR0 bei einer Spezifizität von 100%. ARibPR0+ Patienten wiesen signifikant niedrigere Lymphozytenzahlen, und aRibPR1+ Patienten signifikant höhere GGT-Werte als SLE-Patienten ohne diese Autoantikörper auf. In einem Nachbeobachtungszeitraum von 3 Jahren waren bei SLE-Patienten mit aRibP nicht signifikant mehr spezifische Organschädigungen nachweisbar als bei aRibP- Lupuspatienten, die nach Alter, Geschlecht und Häufigkeit einer Nierenbeteiligung angepaßt wurden. Schlussfolgerungen: Autoantikörper gegen ribosomale P-Proteine, insbesondere jene gegen RibPR0, bieten zusätzlichen diagnostischen Wert beim SLE und sollten daher in zukünftigen Kriterien für die Diagnose oder Klassifikation des SLE berücksichtigt werden. ARibPR0 waren mit Lymphozytopenie und aRibPR1 mit erhöhten GGT-Werten signifikant assoziiert.
