Eine immunmodulatorische Behandlung zur (Re )Aktivierung des Immunsystems und Über-windung der HPV- bzw. tumorinduzierten Immuntoleranz bietet neue Möglichkeiten der Therapie [2]. Dabei rücken auf die Immunsuppression spezialisierte, regulatorische T-Zellen (Tregs) in den Fokus [3]. Bei unbehandelten Zervixkarzinompatientinnen sind im Vergleich zu gesunden Frauen die im Blut zirkulierenden Tregs signifikant erhöht [4]. Erstmals untersucht diese retrospektive Studie den prognostischen Wert des im Blut zirkulierenden Treg-Anteils von 49 Patientinnen mit einem primären Zervixkarzinom vor und nach einer primär kombinierten Radiochemotherapie (RCT). Neben der klassischen phänotypischen Quantifizierung der Tregs als CD3+CD4+CD25+FOXP3+ Zellen mittels Durchflusszytometrie (TregsFlow) erfolgte für Zervixkarzinompatientinnen das erste Mal eine epigenetische Quantifizierung der zirkulierenden Tregs anhand einer Treg-spezifischen Demethylierung von FOXP3 mittels methylierungsspezifischer quantitativer Real-Time-PCR (TregsPCR). Ein hoher Anteil zirkulierender Tregs vor RCT erwies sich als prognostisch günstiger Biomarker für Zervixkarzinompatientinnen. Im Vergleich zur phänotypischen Treg-Quantifizierung bildet die epigenetische Quantifizierung verlässlichere, vom Immunstatus unabhängige und zwischen Studien besser vergleichbare Ergebnisse ab. So hatten Patientinnen mit einem hohen TregPCR-Anteil signifikant seltener Fernmetastasen (Kruskal-Wallis-Test; P = 0,003) und ein signifikant besseres erkrankungsfreies Überleben und Gesamtüberleben (Log-Rank-Test; P = 0,006 und P = 0,006). Der TregPCR-Anteil vor RCT ist damit ein Indikator für eine Fernmetastasierung (empirische ROC- Kurve; AUC = 0,853; P = 0,001) und ein unabhängiger Prognosefaktor für beide Überlebensparameter (multivariate Cox-Regression; P = 0,009 und P = 0,009). Der TregFlow-Anteil korreliert hingegen nicht signfikant mit diesen beiden Überlebensparametern (Log-Rank-Test; P = 0,615 und P = 0,828). Ergebnisse aus laufenden klinischen Studien zur Immunmodulation beim Zervixkarzi-nom werden zur weiteren Klärung des prognostischen Werts der zirkulierenden Tregs beitragen können.","Cervical carcinoma is the fourth most common tumor disease worldwide [1]. Immuno-modulatory therapies provide new possibilities through (re-)activation of the immune system to overcome HPV- or tumor-induced immune tolerance [2]. Regulatory T-cells (Tregs), which are specialized in immunosuppression [3] and are increased in the blood of non-treated cervical carcinoma patients compared to healthy women [4], are at the center of attention. To date, this retrospective study, is the first attempt to investigate the prognostic value of the Treg frequency in peripheral blood of 49 cervical carcinoma patients before and after a primary radio-chemotherapy (RCT). In addition to the phenotypic quantification of Tregs as CD3+CD4+CD25+FOXP3+ T cells by flow cytometry (TregsFlow) an epigenetic quantification of blood Tregs was performed for the first time using a methylation-specific quantitative real- time-PCR based on Treg-specific DNA demethylation within FOXP3 (TregsPCR). In this study an increased blood Treg frequency before RCT was identified as a favora-ble biomarker for cervical carcinoma patients. Compared to phenotypical Treg quantifi-cation, epigenetic quantification was more reliable due to independence of immune status and superior comparability between different studies. Cervical carcinoma patients with increased TregPCR frequency before RCT had significantly less distant metastases (Kruskal-Wallis test; P = 0,003) and significantly improved disease-free survival and overall survival (Log- rank test; P = 0,006 and P = 0,006). TregPCR frequency before RCT was an indicator for distant metastases (empirical ROC curve; AUC = 0,853; P = 0,001) and an independent prognostic factor for both survival parameters (Cox model; P = 0,009 and P = 0,009). In contrast, TregFlow frequency before RCT was not significantly associated with disease-free survival or overall survival (Log- rank test; P = 0,615 and P = 0,828).
