Reinke","w","2011-04-08","2018-06-07T18:19:33Z","2011-03-28T07:58:21.623Z","2011","Die allogene, hämatopoetische Stammzelltransplantation (HSCT) wird zur kurativen Behandlung maligner und nicht-maligner Erkrankungen des lympho- hämatopoetischen Systems in der Pädiatrie eingesetzt. Es können jedoch schwerwiegende Komplikationen unabhängig von der Grundkrankheit und mit nennenswerten Mortalitätsrisiken auftreten. Dazu gehören das Transplantatversagen, die Graft-versus-Host-Krankheit (GVHD) sowie Infektionen durch Bakterien, Pilze und Viren. Cytomegalovirus (CMV), Epstein-Barr-Virus (EBV) und Adenoviren (AdV) zählen zu den kritischen, transplantationsrelevantesten Viren. Bisher liegen vorwiegend Studien zum Einfluss einzelner Viren auf den posttransplantationären Verlauf vor. Im Gegensatz dazu beschäftigte sich diese Arbeit mit der Bedeutung multipler Virusreaktivierungen nach allogener HSCT. In einer retrospektiven Analyse von 124 Kindern und Jugendlichen wurde mit Hilfe univariater und multivariater Testverfahren untersucht, wie hoch die Prävalenz multipler Virusreaktivierungen ist, welche Transplantationsparameter mit dem Auftreten von Virusreaktivierungen assoziiert sind und welchen Einfluss die Virusreaktivierungen auf den Verlauf und die auftretenden Komplikationen haben. Multiple Virusreaktivierungen traten bei 40 % der Patienten nach allogener HSCT auf. Transplantationsparameter wie virologische Serostaten, Geschlecht, Spendertyp und Gaben von Antithymozytenglobulin waren mit dem Auftreten der Virusreaktivierungen signifikant assoziiert. Die Virusreaktivierungen wiederum hatten trotz präemptiver, antiviraler Therapie einen Einfluss auf die im Verlauf auftretenden Komplikationen. So wurde die therapieassoziierte Mortalität (TRM) von der Anzahl der Virusreaktivierungen je Patient beeinflusst. Je mehr Virusreaktivierungen pro Patient auftraten, desto höher war das Risiko zu versterben. Außerdem kam den Adenoviren die größte Bedeutung für die TRM zu. Weniger bedeutsam für die TRM waren EBV- Reaktivierungen, am wenigsten CMV-Reaktivierungen. Weiterhin war die höhergradige akute GVHD signifikant mit EBV-Reaktivierungen und im Trend auch mit CMV-Reaktivierung und der Anzahl der Virusreaktivierungen assoziiert. Die chronische Form der GVHD hingegen war ausschließlich mit AdV-Reaktivierungen assoziiert. Ebenfalls konnten für verschiedene Organschäden Assoziationen mit spezifischen Kombinationen von Virusreaktivierungen gezeigt werden. Diese Ergebnisse führen zu der Schlussfolgerung, dass risikoadaptierte Überwachungsstrategien nötig sind, um Virusreaktivierungen so früh wie möglich zu erkennen, und dass wirksame neue, antivirale Therapiestrategien entwickelt werden müssen, um fatale Verläufe bei multiplen Virusreaktivierungen zu verhindern. Jedoch ist zu beachten, dass diese Arbeit ausschließlich qualitative Virusdaten berücksichtigte. Zusätzliche Informationen mögen sich aus den Viruslasten und Kinetiken ergeben, die prospektiv und möglichst auch multizentrisch mit vergleichbarer Messmethodik erhoben werden sollten.","Allogeneic hematopoietic stem cell transplantation (HSCT) is used to treat a variety of malignant and non-malignant disorders of the lympho-hematopoietic system in childhood. But allogeneic HSCT is still associated with a high treatment related risk due to serious complications. Those complications are transplant failure, graft-versus host disease, and severe bacterial, fungal and viral infections. Cytomegalovirus (CMV), Epstein-Barr-Virus (EBV) and Adenoviruses (ADV) are very common and dangerous viral reactivations following HSCT. Many studies already documented the influence of each of these viruses on the outcome of allogeneic transplants. In contrast, this study wanted to analyse the impact of multiple viral reactivations of CMV, EBV and AdV. Therefore, data of 124 children and adolescents have been analysed retrospectively using univariate and multivariate tests in order to assess the prevalence of multiple viral reactivations, the association between different transplantation-related factors and viral reactivations, and the impact of viral reactivations on the outcome of allogeneic HSCT. Multiple viral reactivations occurred in 40 % of all patients after HSCT. Several transplant related factors like viral serology, gender, donor type, and antithymocyte globulin treatment were significantly associated with the occurrence of viral reactivations. On the other side, viral reactivations had a significant impact on several complications and outcome of HSCT. Treatment-related mortality (TRM) increased when multiple viral reactivations occurred. Among the three viruses AdV had the greatest impact on TRM followed by EBV while CMV did not influence TRM. Furthermore severe acute GVHD was significantly associated with EBV reactivations, and showed a tendency to be related with CMV reactivations and the number of viral reactivations. Severe chronic GVHD was exclusively associated with AdV reactivations. Additionally, specific viral combinations had an impact on the occurrence of different organ damages. In conclusion, risk-adapted surveillance strategies are needed to recognize viral reactivations as soon as possible, and effective antiviral treatment options have to be developed to prevent the fatal risk of multiple viral reactivations.
