Kürzliche Untersuchungen zeigten, dass genetische Polymorphismen von OATP1B1 dessen Transportfunktion beeinflussen. Die mögliche Konsequenz einer veränderten Pharmakokinetik mit individueller Medikamenten-Disposition, auch hinsichtlich einer hohen intersubjektiven Variabilität in der Pharmakotherapie, ist interessanter Diskussionsgegenstand einer zunehmenden Zahl von Untersuchungen. Zielstellung: Die pharmakogenetischen Effekte der Haplotypen OATP1B1 (organic anion transporting polypeptide 1B1) *1a, *1b (A388G) und *5 (T521C) auf die Pharmakokinetik einer Einzeldosis des Cholesterinsenkers Pravastatin bei weißen Europäern wurden untereinander verglichen. Methoden: Nach Genotypisierung von 423 Freiwilligen wurden 30 gesunde männliche Probanden kaukasischer Abstammung entsprechend ihres OATP1B1 – Haplotyps ausgewählt und einer Gruppe zugeordnet. Jede der drei Gruppen umfasste zehn Individuen, die entweder homozygote oder heterozygote Träger der Variante OATP1B1 *1a, *1b oder *5 waren. Nach einer Einzeldosis von 40 mg Pravastatin wurden die kinetischen Parameter der Disposition für Pravastatin gemessen. Hauptzielgröße war die Plasmakonzentrations–Zeit–Kurve in der Zeit null bis sechs Stunden nach Medikamenteneinnahme [AUC(0-6)]. Die Pravastatin-Konzentrationen in Plasma und Urin wurden mittels Hochleistungsflüssigkeitchromatographie (HPLC) bestimmt. Ergebnisse: Die Messwerte für AUC(0-6) bei Probanden der Genotypen *1a/ *1a, *1a/ *1b oder *1b/ *1b und *1a/ *5 betrugen jeweils 114,5 ± 68,6µg . L –1 . h, 74,8 ± 35,6 µg . L –1 . h und 163,0 ± 64,6 µg . L –1 . h mit hoch signifikanten Unterschieden zwischen allen drei Studiengruppen (P = 0,006) und zwischen den Trägern des Haplotyps *1b und *5 (P = 0,002). Eindrucksvoll war, dass die Werte für AUC (0-6) der Gruppe OATP1B1 *1b um mehr als 60 % niedriger waren gegenüber der Gruppe des Haplotyps OATP1B1 *1a, obwohl dieser Unterschied keine statistische Signifikanz erreichte. Allerdings war in der Gruppe der OATP1B1 *1b - Allelträger die Menge an über den Urin ausgeschiedenem Pravastatin in der Zeit null bis zwölf Stunden [Ae (0-12)] signifikant vermindert ( 1729 ± 907 µg ) im Vergleich zur Kontrollgruppe des Wildtyps OATP1B1 *1a ( 2974 ± 1590 µg) (P = 0,049). Innerhalb der Gruppe OATP1B1 *5 wurden die höchsten Werte für AUC (0 – 6) gemessen. Diese unterschieden sich signifikant von den Werten der für OATP1B1 *1a homozygoten Studienteilnehmer (P = 0,049 ) und den Ergebnissen für die Gruppe der homo- und heterozygoten Träger von OATP1B1 *1b (P = 0.002); der Mittelwert für AUC (0 – 6) der Gruppe *5 betrug 142 % im Vergleich zur Referenzgruppe *1a. Schlussfolgerung: Ein signifikanter Effekt der untersuchten OATP1B1- Haplotypen auf die Disposition von Pravastatin war feststellbar. Während die Expression von OATP1B1 *5 die hepatozelluläre Aufnahme von Pravastatin verzögerte, schien die Variante *1b die OATP1B1– abhängige Aufnahme des Medikaments zu beschleunigen. Die Transportfunktion von OATP1B1 zeigte sich somit interessanterweise gegensätzlich beeinflussbar. Dies gilt als einer der ersten Berichte, die darauf hinweisen, dass verbesserte Transporteigenschaften in vitro, bedingt durch polymorphe Transportergene, tatsächlich die Pharmakokinetik eines Medikaments in vivo verändern. Vorhergehende klinische Studien konnten den Effekt der Haplotypen OATP1B1 *1b und OATP1B1 *5 nicht parallel isoliert darstellen. Vermutlich trägt die vermehrte Aufnahme einiger Arzneimittel aus dem Pfortaderblut zu einer niedrigeren oralen Bioverfügbarkeit bei und darüber hinaus zu einem vermehrten hepatischen first- pass Mechanismus bestimmter Komponenten. Weitere in vivo- Studien sind erforderlich, um die hier vorgestellten Ergebnisse zu bestätigen. Dennoch lässt sich auf deren Grundlage vermuten, dass OATP1B1-Haplotypen auf bemerkenswerte Weise zur interindividuellen Variabilität in der Disposition von Arzneistoffen beitragen, die dem Substratspektrums dieses Transportproteins angehören.","The organic anion transporting polypeptide OATP1B1 (SLCO1B1) is a transport protein, which is located at the sinusoidal membrane of hepatocytes. It regulates the uptake of a various number of substrates, including drugs, from portal venous blood into the liver. Recent studies demonstrated, that genetic polymorphisms in OATP1B1 have impact on its transport function. The possible consequence of altered pharmacokinetics and individual drug disposition, also regarding the high intersubject variabilities in pharmatherapy, is an interesting matter of discussion of a growing number of studies. Objective: The pharmacogenetic effects of OATP1B1 (organic anion transporting polypeptide 1B1) *1a, *1b (A388G) und *5 (T521C) haplotypes on single-dose pharmacokinetics of the cholesterol-lowering drug pravastatin in white Europeans were compared. Methods: 423 volunteers were genotyped, of which 30 healthy male Caucasians were chosen and grouped according to their OATP1B1 haplotype. Each of the three groups comprised 10 individuals who were either homozygous or heterozygous carriers of the OATP1B1 *1a, *1b or *5 variant. After a single oral dose of 40 mg pravastatin, the pharmacokinetic parameters of pravastatin disposition were analyzed. The primary parameter was the area under the plasma concentration-time curve from time 0 to 6 hours [AUC (0-6)] after oral drug intake. The concentration of pravastatin in plasma and urine was determined by high performance liquid chromatography (HPLC). Results: Values for AUC(0-6) in individuals of *1a/ *1a, *1a/ *1b or *1b/ *1b and *1a/ *5 genotype were 114,5 ± 68,6µg . L –1 . h, 74,8 ± 35,6 µg . L –1 . h and 163,0 ± 64,6 µg . L –1 . h, respectively, with highly significant differences across all 3 study groups (P = 0,006) and between carriers of the *1b and *5 haplotype (P = 0,002). Interestingly, AUC(0-6) values from the OATP1B1 *1b group were more than 60% lower than those derived from subjects carrying the reference OATP1B1 *1a haplotype, although this difference did not reach statistical significance. However, the amount of pravastatin excreted into the urine from time 0 to 12 hours [Ae (0-12)] was significantly reduced in the OATP1B1 *1b allele group ( 1729 ± 907 µg ) compared with the *1a wild-type control group ( 2974 ± 1590 µg) (P = 0,049). Within the OATP1B1 *5 group, the highest values for AUC (0-6) were recorded and were significantly different from values of homozygous individuals with the OATP1B1 *1a haplotype (P = 0,049 ) and results of the group of homo- or heterozygous carriers of the OATP1B1 *1b variant (P = 0.002); the mean for AUC (0-6) of the *5 group was 142 % compared to the *1a reference group. Conclusion: A significant effect of tested OATP1B1 haplotypes on pravastatin disposition could be demonstrated. Whereas expression of OATP1B1*5 delayed the hepatocellular uptake of pravastatin, the variant *1b seemed to accelerate OATP1B1-dependent uptake of the drug. The transport function of OATP1B1 interestingly showed to be inversely affectable. This is one of the first reports indicating that improved in vitro transport characteristics by polymorphic transporter genes do actually alter in vivo pharmacokinetics of a drug. In earlier clinical trials the effect of the haplotypes OATP1B1 *1b and OATP1B1 *5 could not be investigated separately. Enhanced uptake of certain drugs from portal venous blood presumably contributes to a lower oral bioavailability and moreover to an enhanced hepatic first-pass mechanism of certain compounds. Further in vitro studies are necessary to confirm these results.
