Werden zugelassene Medikamente außerhalb dieser in der Zulassung vorgegebenen Parameter verordnet, spricht man von Off-Label-Use, welcher zu verschiedenen medizinischen und rechtlichen Problemen führen kann. Gerade in der Pädiatrie wird ein relevanter Anteil der Medikamente außerhalb der Zulassung verordnet, weil bei anderweitig zugelassenen Medikamenten häufig eine Zulassung für das Kindes- und Jugendalter fehlt. Frühere Studien weisen darauf hin, dass sich Krankenhausstationen und ambulante Praxen in der Häufigkeit des Off-Label-Use unterscheiden könnten. Daher haben wir in dieser Arbeit die Häufigkeit der Off-Label-Use-Verschreibungen in pädiatrischen Krankenhausstationen und ambulanten Praxen verglichen, sowie andere mögliche Einflüsse auf die Häufigkeit von Off-Label-Use-Verschreibungen untersucht. Methodik: Die Untersuchung erfolgte anhand einer retrospektiven Analyse aller Arzneimittelverordnungen aus dem II. Quartal 2005 von zwei pädiatrischen Allgemeinstationen der Charité – Universitätsmedizin Berlin und von zwei allgemeinpädiatrischen Gemeinschaftspraxen. Ergebnisse: Die Ergebnisse zeigen, dass Off-Label-Use auf den Krankenhausstationen mit einem Anteil von 14% signifikant häufiger vorkam als in den ambulanten Praxen mit einem Anteil von 8%. Eine logistische Regressionsanalyse ergab, dass die Häufigkeit des Off- Label-Use dabei außer durch die Art der Einrichtung auch mit der jeweiligen Altersgruppe, Verschreibungsindikationen, Art des verschriebenen Medikamentes, sowie deren Darreichungsform und Applikationsform korreliert. Diskussion: Unsere Untersuchung zeigt erstmals einen detaillierten Blick auf die Unterschiede im Off-Label-Use-Verordnungsverhalten zwischen Krankenhausstationen und ambulanten Kinderarztpraxen. Diese Unterschiede sind teilweise gut durch die Einflussfaktoren Alter, Indikationen, Art des Arzneimittels, Darreichungsformen und Applikationsarten erklärbar, andere können jedoch nicht vollständig auf diese zurückgeführt werden. Bezüglich dieser Off-label-Verordnungen besteht insbesondere in der stationären Versorgung von Kindern eine relevante medizinische und rechtliche Problematik. Dies beinhaltet u.a. die Notwendigkeit von Aufklärung und Einwilligung, das Risiko von Nebenwirkungen, Schadensersatzansprüche und Regressen. Es zeigt sich, dass es zum Teil an genauen Angaben zur Anwendung an Kindern und standardisierten Maßeinheiten in den Fachinformationen fehlt. Dieses ist mit potentiellen Risiken für Über-oder Unterdosierungen verbunden. Vor diesem Hintergrund zeigt die Arbeit die Notwendigkeit einer eindeutigen Definition des Off-Label-Use und stellt Lösungsansätze zur Optimierung des zukünftigen Einsatzes dar.","Abstract Introduction: In Germany, the pharmaceutical approval of a drug is only valid for specific indications, age groups, dosages, dosage forms and application periods. The prescription of approved drugs outside of the approved restrictions is called off-label-use and can lead to a variety of medical or legal problems. The medical branch of pediatrics stands out with a high frequency of off-label prescriptions, due to a lack of approved drugs in the age groups of children and adolescents. Earlier studies led to the assumption that the frequency of off-label prescriptions might differ between pediatric hospital wards and community-based pediatrician’s practices. Therefore, we investigated and compared in this study the frequency of off- label prescriptions between pediatric hospital wards and pediatrician’s practices and possible confounding factors on the frequency of off-label use. Methods: The investigation was performed as a retrospective analysis of all drug prescriptions that occurred during the second quarter of 2005 in two pediatric hospital wards of Charité - Universitätsmedizin Berlin and two community-based pediatrician’s practices. Results: Our results show that off- label prescriptions occur in pediatric hospital wards at a significantly higher frequency (14%) in comparison to pediatrician’s practices (8%). A logistic regression analysis showed correlations of the frequency of off-label prescriptions with the patient’s age group, the prescription’s indication, the type of drug, the dosage form and the route of administration. Discussion: Our study proves for the first time, that there is a significant difference in the frequency of off-label prescriptions between pediatric hospital wards and pediatrician’s practices. These differences might be caused in part by confounder-effects due to different patient collectives, since different age groups, indications, drug types, dosage forms and routes of administration significantly influence the frequency of off-label prescriptions. However, the influences of such confounders cannot explain the differences in whole, leading to the assumption that the medical and legal aspects have to be addressed especially for pediatric hospital wards. This contains amongst others the need for informed consent, risk of side effects and hence claims for damages and claims of recourse can occur. It is also highlighted that there is a deficit of detailed information about use on children and standardised measurements in the professional information. This is associated with potential risks of drug under- and overdosing. Against this background this study shows that there is a lack of definition concerning off label use.
