Zielstellung dieser Arbeit war es, einen Interferon-Surrogatmarker zu etablieren, der die Krankheitsaktivität und die Wirkung neuer IFN-suppressiver Therapien bei SLE-Patienten widerspiegelt. Mithilfe der Affymetrix GeneChip Technologie wurden genomweite differentielle Genexpressionsanalysen hoch aufgereinigter Blutmonozyten von 9 SLE-Patienten und 8 Normalspendern erstellt. Siglec-1 (Sialoadhesin, CD169), ein Interferon-induziertes Adhäsionsmolekül, wurde ausgewählt und in der Durchflusszytometrie auf inflammatorischen und residenten Blutmonozyten bei 52 SLE-Patienten und 38 Kontrollen als Interferon-Surrogatmarker beurteilt. Blutmonozyten weisen eine prominente IFN-Signatur in der differentiellen Genexpressionsanalyse beim SLE auf. Siglec-1 wurde als das am stärksten hochregulierte IFN-induzierte Oberflächenprotein identifiziert. Der Prozentsatz Siglec-1 positiver (exprimierender) Monozytensubtypen korrelierte mit der Krankheitsaktivität (SLEDAI) und mit dem Komplementverbrauch von C3 und C4. Die Höhe der Anti- dsDNA-Antikörpertiter korrelierte dagegen nur mit dem Prozentsatz Siglec-1 positiver residenter, nicht aber dem der inflammatorischen Monozyten. Eine drastische Reduktion von Siglec-1 auf beiden Monozytensubpopulationen konnte beispielhaft durch die Gabe ultrahoher Glukokortikoiddosen bei vier aktiven SLE Patienten gezeigt werden. Erstmals konnte durch die Kombination von Zellseparation und Genexpressionsanalysen das monozytäre Adhäsionsmolekül Siglec-1 als Biomarker beim SLE identifiziert werden. Siglec-1 wurde auf Proteinebene mittels Durchflusszytometrie als IFN-induzierter Aktivitätsmarker validiert und könnte zukünftig helfen, unter IFN-inhibitorischen Therapien individualisierte Dosisanpassungen vorzunehmen.","Type I Interferon (IFN) has a pivotal role in the pathogenesis of systemic lupus erythematosus (SLE) and is therefore discussed as a potential therapeutic target. The aim of our study was to establish a feasible biomarker for IFN effects with respect to monitor disease activity and effectiveness of potential IFN suppressive therapy in SLE patients. Transcriptomes of purified monocytes from 9 SLE patients and 8 healthy controls were analysed by Affymetrix GeneChip technology. In comparison to 38 healthy controls, Siglec1 (Sialoadhesin, CD169) was validated at the protein level by multicolor flow cytometry in inflammatory and resident monocyte subsets of 52 SLE patients. Transcriptomes of peripheral monocytes from SLE patients revealed a dominant type I IFN signature. Siglec1 was identified as one of the most prominent type I IFN-regulated candidate genes. At the protein level, frequency of Siglec1 expressing monocyte subsets correlated with disease activity (SLEDAI) and inversely with levels of complement factors. Most interestingly, levels of anti-dsDNA antibodies highly correlated with the percentage of Siglec1 expressing resident, but not inflammatory monocytes. High dose glucocorticoid treatment of active SLE patients resulted in a dramatic reduction of Siglec1 expression on cells. For the first time, Siglec-1 could be identified as a biomarker in SLE by combination of cell separation and gene expression analysis.
