Der hohen klinischen Relevanz dieses Erregers stehen dabei die Problematik der zunehmenden Resistenzentwicklung und die eingeschränkt wirksame Impfstrategie gegenüber. Zur Entwicklung innovativer Therapiestrategien kann die Erforschung der Wirt-Erreger-Interaktion wesentlich beitragen. Verschiedene Studien legten nahe, dass dem Epithel, neben seiner Rolle als mechanische Barriere, durch die Erkennung eindringender Keime und die daraus resultierende Zytokinaktivierung eine wichtige Wächter -Funktion zukommt. In dieser Arbeit konnte gezeigt werden, dass die Stimulation humaner Bronchialepithelzellen mit S. pneumoniae zu einer zeit- und dosisabhängigen Steigerung der Expression von GM-CSF und weiterer Zytokine (TNFα, IL-1, IL-8) führt. Mithilfe eines Mauspneumoniemodells konnte die pneumokokkenvermittelte GM-CSF-Induktion in vivo nachvollzogen werden. Auch der isolierte Virulenzfaktor Pneumolysin führte zu einer GM-CSF-Expression. Sowohl die Detektion der Pneumokokken durch TLR-2 als auch von Pneumolysin durch TLR-4 führte zur Expression von GM-CSF. In Folge kam es zu einer Aktivierung des kanonischen NF- κB-Signalpfades sowie von AP-1, zwei Transkriptionsfaktoren, die wesentlich zur beobachteten Zytokinbildung beitrugen. Zusammenfassend führt eine komplexe, zumindest in Teilen TLR-2- und TLR-4-abhängige Stimulation von Transkriptionsfaktoren (NF-κB, AP-1) zur Expression von Zytokinen in pneumokokkeninfizierten Epithelzellen. Weiterführende Untersuchungen, welche die Verifizierung dieser in vitro gewonnenen Experimente in vivo beinhalten sollten, sind notwendig, um das Verständnis der komplexen Pneumokokken-Wirts-Interaktion zu vertiefen.","Community aquired pneumonia belongs to the most frequently diagnosed infectious diseases worldwide and is in about 30 % of the cases caused by S. pneumoniae. The high clinical relevance faces the rising problem of germ resistence and vaccination strategies of restricted effectiveness. Further exploration of germ-host-interaction may contribute to the development of innovative therapeutic strategies. Recent evidence suggests that the lung epithelium functions, besides acting as a mechanical barrier, as an important sentinel system against pathogens through recognition of invading germs and induction of cytokine expression. Pneumococci infection induces the expression of GM-CSF and other cytokines (TNFα, IL-1beta, IL-8) in human bronchial epithelial cells (Beas-2B) dose- and timedependently. In mouse lungs in vivo the pneumococci-induced GM-CSF release was demonstrated. Also pneumolysin, an important virulence factor of S. pneumonia leads to GM-CSF induction. Living pneumococci are recognized by Toll-like receptor 2, whereas pneumolysin mediates GM-CSF-secretion through Toll-like receptor 4. The recognition of S. pneumonia leads to activation of the canonical NF-κB-pathway and AP-1-induction, two transcription factors, that contribute to cytokine secretion. Thus, the complex, at least in parts TLR-2 and TLR-4 dependent, stimulation of transcription factors (NF-κB, AP-1) contributes to expression of cytokines in pneumococci-infected epithelial cells.
