Klaus Böning","w","2011-02-04","2018-06-07T17:35:01Z","2011-01-24T10:41:09.390Z","2011","Während in zahlreichen in vitro Untersuchungen Rand- und Zementspaltbreiten von 50 µm und kleiner erreicht werden konnten [58,98,109,123], zeigten in vivo Untersuchungen große Unterschiede zwischen einer theoretisch möglichen und der klinisch erreichten Passgenauigkeit [10,13,18,30,68,83,112]. Das lässt den Schluss zu, dass die Qualität einer Krone im vollen Umfang von klinischen Faktoren bestimmt wird. Ziel dieser Arbeit war es daher, den Einfluss der Präparation der okklusalen Fläche auf die innere und die marginale Passgenauigkeit von vollkeramischen Kronengerüsten, hergestellt mit dem Cerec3-CAD/CAM-System unter Berücksichtigung der eingestellten Zementspaltbreite, des verwendeten Befestigungszementes, und der Gerüstkeramik zu untersuchen. Für diese Aufgabe wurden zwei metallische Originalmodelle vorbereitet (1. mit klassischer okklusalen Fläche, 2. mit flach präparierter okklusalen Fläche), die Originalmodelle wurden dubliert und anschließend wurden 80 Arbeitsmodelle jeweils ausgegossen. Optische Abdrücke wurden von den Arbeitsmodellen mit der digitalen Kamera des Cerec3-CAD/CAM-System gemacht. Zwei Zementspalteinstellungen (100 µm und 50 µm) wurden beim Konstruieren der Gerüste am Bildschirm des Computers ausgewählt. 160 Kronengerüste wurden aus Glasinfiltrationskeramik und Sinterkeramik jeweils 80 Kronengerüste hergestellt. Für jede Keramik wurden 40 Kronengerüste mit der klassischen Präparation der okklusalen Fläche (klassisch mit Höcker) und 40 Kronengerüste mit flach präparierter okklusalen Fläche (neue Präparation) geschliffen. Vier Zemente (ZPC: Zinkphosphatzement, P21: Panavia21, Exp.1: experimenteller Zement.1, Exp.2: Experimenteller Zement 2) wurden für die Zementierung der Gerüste auf ihren Arbeitsmodellen benutzt. Prüfkörper (Kronengerüst + Modell) wurden in transparentem Kunststoff eingebettet, so dass eine Würfelform entstand. Die Proben wurden longitudinal bukko- lingual und dann mesio- distal gesägt. Zur qualitativen und quantitativen Beurteilung im Lichtmikroskop, wurde die Rand- und Zementspaltanalyse bei 40-facher Vergrößerung durchgeführt. Es konnte folgendes gezeigt werden: 1\. Die Form der Präparation der okklusalen Fläche von Molaren hat einen signifikanten Einfluss auf die Passgenauigkeit von Cerec3 vollkeramischer Kronengerüsten. Es konnte gezeigt werden, dass mit einer ohne okklusales Relief präparierten okklusalen Fläche im Vergleich zu einer anatomisch präparierten okklusalen Fläche, eine bessere innere und marginale Passgenauigkeit, unabhängig von der Wahl der Zementspalteinstellung am Cerec–Gerät, der Wahl des Zementes und der Wahl der Gerüstkeramik, erreicht werden konnte. 2\. Die Auswahl der Zementspalteinstellung hatte einen signifikanten Einfluss auf die Zementspaltbreite und auf die Randspaltbildung. Die Verwendung von Zementspalteinstellung von 100 µm kann für alle verwendeten Zemente empfohlen werden. Denn eine ZSE von 100 µm schafft genügend Platz für den Zement zum Abfließen, so dass ein gleichmäßiger und nicht erheblich vergrößerter Zementspalt erreicht werden kann. Bei Panavia21- Zement, der sich durch eine niedrige Viskosität auszeichnet, wird dies auch mit einer Zementspalteinstellung von 50µm erreicht. Von der Verwendung von Zementspalteinstellung von 50 µm bei Zinkphosphatzementen abzuraten. 3\. Die Auswahl des Befestigungszementes hatte einen signifikanten Einfluss auf die Innere und marginale Passgenauigkeit von Cerec3 vollkeramischen Kronengerüsten. Adhäsive Zemente, die eine gute Fließfähigkeit beweisen, zeigen bessere Ergebnisse im Vergleich zu den anderen Zementen. Panavia21 brachte die kleinsten Zement- und Randspaltbreiten bei einer ZSE von 50 µm hervor und verringerten sich seine Werte als die ZSE von 100 µm benutzt wurde. 4\. Die beiden in dieser Studie verwendeten Gerüstkeramiken können für das Erreichen einer guten inneren und marginalen Passgenauigkeit empfohlen werden. 5\. Mit Hilfe der Cerec3-CAD/CAM-Technik kann als klinisch akzeptable Passgenauigkeit ähnlich mit der konventionellern Gusstechnik erreicht werden. Eine Erprobung In vivo erscheint unter Beachtung der klinischen und labortechnischen Verfahrensweisen empfehlenswert.","Apart from fracture resistance and aesthetics, marginal and internal accuracy of fit is valued as one of the most important criteria for long-term success of all-ceramic crowns and bridges, because inadequate fit is potentially damaging for both abutment and periodontal tissues. This in-vitro study evaluated the influence of two different tooth preparation forms, two luting space settings (LSS) and four cement types on the marginal and internal adaptation of all-ceramic crown copings produced using Cerec3® CAD/CAM system. 160 working stone dies were made from two metal master casts [1. tooth36: with usual occlusal reduction (anatomic), 2. tooth 36: with flat occlusal reduction (new)]. 80 crown copings were milled using Vita In-Ceram® 2000 YZ- 20/15 Cubes and 80 using Vita In-Ceram® Zirkonia Blanks for Cerec CZ-12. For designing the crown copings two luting space settings were used (LSS) 0= 100 µm and -50= 50 µm. The copings were cemented using four cements (zinc phosphate cement/ZPC, Panavia21/P21, Experimental cement 1/Exp.1 and Experimental cement 2/Exp.2), then embedded and sectioned longitudinally bucco-lingually and mesio-distally. Widths of marginal and internal gaps were measured using a light microscope at magnification of 40X. Statistical assessment shows that copings with a flat occlusal preparation and milled with a luting space setting of 100 µm presented the lowest marginal gap (P< 0.05) widths and at the same time the cement film thickness of these specimens were not so much enlarged comparing with the copings with usual occlusal preparation and milled with a luting space setting of 50 µm. The shaping of the cusps at the usual occlusal surface prevents the complete flowing of the cement which leads to enlarged cement thickness at the occlusal area, therefore broad marginal gaps were produced as the usual occlusal preparation was used in this study. A luting space setting of 100 µm creates enough space for the cement film on the occlusal and axial surfaces of the prepared tooth; this allows a better flow and outflow of excess cement and gives the security of minimizing the marginal gap (MG) in all cases with complete filled internal gap (IG) independently of the kind of cement used. It can be assumed that the smaller the space between the restoration and the prepared tooth, the more impeded is the flow of excess cement out of the restoration and the higher is the stagnation rate of cement at the occlusal aspect which increased the internal gap widths and marginal discrepancies. The measurements showed that by a luting space setting of 50 µm the measured internal gap was more enlarged than that by a luting space setting of 100 µm and therefore the marginal gap widths were smaller (P< 0.05) as the LSS of 100 µm was used. The marginal gap widths of copings cemented with P21 were smaller (P< 0.05) than those cemented with the other cements regardless to the used luting space setting. The explanation is that the P21 has a lower viscosity and so it can rapidly flow resulting small marginal gaps.
