Alexandra Philipsen","w","2016-06-13","2018-06-07T17:19:19Z","2016-06-30T08:29:22.915Z","2016","Störungen des Mentalisierungsvermögens stellen eine psychopathologische Kerndomäne psychotischer Störungen wie der Schizophrenie dar und haben große Bedeutung für die Manifestation von Symptomen, interpersonelle Funktionen, aber auch für die Suche nach Vulnerabilitätsmarkern und Therapiemöglichkeiten gewonnen. Ein Gegenstand der hier vorliegenden Forschungsarbeit war die Untersuchung von Subkomponenten gestörten Mentalisierens bzw. der Theory of Mind (ToM) sowie der Empathie und deren Zusammenhang mit verschiedenen Symptomdimensionen der Schizophrenie. Hierbei konnte einerseits eine hyperintentionale Zuschreibung mentaler Zustände (Übermentalisieren) bei Patienten mit florider paranoider Symptomatik bestätigt werden, während Patienten mit vorherrschender Negativsymptomatik eher eine Tendenz zu hypointentionalen bzw. konkretistischen Attributionen (Untermentalisieren) aufwiesen. Andererseits zeigte sich ein charakteristisches Muster kognitiv- empathischer Einschränkungen bei erhaltener Fähigkeit zu emotionaler Empathie und Altruismus. Gleichzeitig fielen bei Patienten mit Schizophrenie im Vergleich zu Gesunden eine erhöhte Ansteckbarkeit mit negativen Emotionen und vermehrter empathischer Distress in sozialen Situationen auf. Letzterer bestimmte gemeinsam mit einer subjektiv erlebten Emotionsüberflutung in höherem Maße die psychosoziale Leistungsfähigkeit der Patienten als deren kognitive Empathie. Ferner wurden Theory of Mind und Empathie als mögliche Vulnerabilitätsmarker für psychotische Störungen an einer gesunden Stichprobe mit erhöhtem genetischen Risiko, nämlich bei erstgradigen Angehörigen von Schizophreniepatienten, untersucht. Diese wiesen lediglich leichtere Einschränkungen des kognitiven, aber nicht des emotionalen Mentalisierungsvermögens auf, welches somit als Ausdruck einer Vulnerabilität für psychotische Störungen angesehen werden könnte. Ziel einer weiteren Untersuchung war es, ausgewählte Polymorphismen des OXTR-Gens auf ihre Assoziation mit der Disposition zu Empathie und Prosozialität bei Patienten mit Schizophrenie im Vergleich zu Gesunden zu untersuchen. Hierbei ergaben sich Hinweise auf eine Assoziation des A-Allels von OXTR-SNP rs2254298 mit vermehrter altruistischer Fürsorge sowie mit der Ausprägung unspezifischer, insbesondere affektiver Psychopathologie in der Patientengruppe. Letztlich sollten mithilfe einer randomisiert-kontrollierten Pilotstudie erste Daten zur Effektivität von psychodynamischer Kunsttherapie gewonnen werden. Neben der Wirksamkeit hinsichtlich der psychotischen Symptomatik einer stationär behandlungsbedürftigen Patientengruppe als primärem Studienziel lag das Hauptaugenmerk auf den Effekten der Kunsttherapie in Bezug auf kognitive Empathie und affektives Mentalisierungsvermögen. Im Ergebnis zeigte die Interventionsgruppe im Vergleich zur Standardbehandlung eine signifikante Reduktion positiver und negativer psychotischer Symptome wie auch ein verbessertes Vermögen zur Reflektion von Emotionen. Insgesamt unterstreichen die Ergebnisse die Bedeutung der Mentalisierungsfähigkeit sowohl für die klinische Diagnostik, die Suche nach Vulnerabilitätsfaktoren wie auch für die Behandlung von Menschen mit psychotischen Erkrankungen.","Difficulties in mentalizing are regarded as a core psychopathological domain in psychotic disorders like schizophrenia and have importance for the manifestation von symptoms und interpersonal functioning, but also for the search for markers of vulnerability and new treatment options. The present research work focused on sub-components of mentalizing deficits like those of theory of mind (ToM) and of empathy and on their associations with symptom dimensions in patients with schizophrenia. Here, hyper-intentional mental state attribution was found to be linked with positive, e. g. delusional symptoms, and hypo-intentional or concrete mentalizing was associated with negative symptoms. Patients with schizophrenia showed a characteristic pattern of impaired cognitive empathy, while emotional empathy and altruism were preserved. Furthermore, patients reported more empathic distress in interpersonal situations and a higher susceptibility for negatively valenced emotional contagion that together with cognitive empathy predicted global psychosocial functioning. A potential role of cognitive, but not emotional, theory of mind as a marker of psychotic vulnerability was substantiated in a sample of healthy first-degree relatives of schizophrenia patients as compared to control subjects without a family history of schizophrenia. In another study, selected single nucleotide polymorphisms (SNP’s) of the oxytocin receptor gene (OXTR) that were previously found to be associated with social- emotional behaviors and disorders like schizophrenia and autism were investigated for their relations with dimensions of empathy. Results may suggest an association of OXTR SNP rs2254298 with empathic concern and general (unspecific) psychopathology in a sample of schizophrenia patients. A pilot randomized controlled trial aimed at gathering first data on the efficacy of psychodynamic art psychotherapy (AT) in a group of hospitalized patients with schizophrenia compared with standard treatment. Beside the effects of AT on symptoms and psychosocial function as a primary study outcome, its impact on mentalizing function was also to be determined. As a result, patients randomized to the AT group showed a significantly greater reduction of positive and - at follow-up - also of negative symptoms than the control group. Levels of emotional awareness, the ability to reflect on the emotions of other people, were significantly higher in patients receiving AT at the end of treatment.
