Jedoch liegen keine Daten über die Korrelation von Funktion und Morphologie in verschiedenen Gefäßgebieten in einzelnen Patienten und Probanden sowie im Vergleich von gesunden Probanden und Patienten vor. Unser Ziel war es daher, durch simultane Messungen der Funktion und Morphologie von A. carotis, thorakaler Aorta und Koronararterien intraindividuelle Unterschiede bei Patienten und Probanden zu erfassen und beide Eigenschaften zwischen gesunden Probanden und Patienten mit Diabetes mellitus Typ 2 (DMT2) zu vergleichen. Material und Methoden Insgesamt 9 gesunde Probanden (2 Männer, Alter: 53 ± 2 Jahre) und 31 Patienten mit DMT2 (20 Männer, Alter: 62 ± 10 Jahre) wurden mit einem kommerziellen Ganzkörper- MRT-Gerät bei 3.0 Tesla untersucht. Bei jedem Teilnehmer führten wir eine MRT- Untersuchung an beiden Aa. carotides, der thorakalen Aorta und einem proximalen Segment einer Koronararterie durch. Distensibilität (in mmHg-1*103) errechneten wir aus (Lumenmax – Lumenmin)/(Pulsdruck x Lumenmin) x 1000 und Vessel Wall Ratio (VWR, in mm2/m2) aus (Gefäßwandfläche/Körperoberfläche). Ergebnisse Wir fanden weder bei den Probanden noch bei den Patienten eine signifikante Korrelation zwischen der Distensibilität und VWR innerhalb der Gefäßgebiete noch zwischen den verschiedenen Gefäßgebieten, ausgenommen zwischen der Distensibilität der A. carotis und der thorakalen Aorta der Probanden (R = 0.860, p = 0.013). Die VWR der A. carotis und der thorakalen Aorta war bei den Probanden signifikant niedriger (20.75 ± 6.02 vs. 13.18 ± 1.90, respektive 147 ± 31.63 vs 85.38 ± 8.54, p > 0.001). Wir fanden signifikante Distensibilitätsunterschiede der Koronargefäße zwischen Patienten und Probanden (2.41 ± 1.66 vs. 6.35 ± 2.87, p > 0.001), jedoch nicht in der Distensibilität der A. carotis oder der thorakalen Aorta. In der Patientengruppe zeigte sich eine signifikante Korrelation zwischen der VWR der A. carotis und der thorakalen Aorta (R = 0.492, p = 0.011). Darüber hinaus zeigte sich eine signifikante Korrelation zwischen der VWR von A. carotis und thorakaler Aorta und der Distensibilität der Koronargefäße der Patientengruppe (R = -0.524, p = 0.012, respektive R = -0.624, p = 0.002). Schlussfolgerung Weder bei Patienten mit DMT2 noch in der Probandengruppe zeigte sich eine signifikante Korrelation der Distensibilitäten der Gefäßgebiete oder der Distensibilität und VWR innerhalb der einzelnen Gefäßterritorien. Diese Ergebnisse suggerieren, dass die Bewertung von frühen atherosklerotischen Veränderungen in peripheren Gefäßgebieten auf die individuelle Risikostratifizierung in Patienten mit DMT2 limitiert ist.","Introduction Arterial distensibility and vessel wall thickness can be measured non-invasively by cardiovascular magnetic resonance (CMR). However, no information is available comparing distensibility and vessel wall ratio in different anatomical territories in individual patients and healthy subjects and between the two groups. Therefore our goal was to measure the distensibility and vessel wall thickness of the carotid artery, the ascending aorta and coronary artery by CMR in order to search for individual differences in healthy subjects and patients with type 2 diabetes mellitus (DM) and to compare the results between the two groups. Material and Methods In total 9 healthy subjects (2 men, age: 53 ± 2 years) and 31 patients (20 men, age: 62 ± 10 years) with DM were studied using a commercial whole body MRI system at 3.0 Tesla. In each patient, both carotid arteries, the ascending aorta and a proximal segment of a coronary artery were imaged for cross-sectional area measurements using CMR. Distensibility (in mmHg-1*103) was determined as (Lumenmax - Lumenmin)/(Pulse pressure x Lumenmin) x 1000 and Vessel Wall Ratio (VWR, in mm2/m2) as (Vessel Wall Area/Body Surface Area). Results We found no significant correlation between the distensibility vs. VWR within the same vascular territory or between the distensibilities in all vascular territories in neither patients nor healthy subjects, except between the aortic and carotid distensibility in the healthy subjects (R = 0.860, p = 0.013). Mean values for carotid and aortic VWR were significantly lower in healthy subjects (20.75 ± 6.02 vs. 13.18 ± 1.90 and 147 ± 31.63 vs 85.38 ± 8.54, p > 0.001). We found only a significant difference in coronary artery distensibility (2.41 ± 1.66 vs. 6.35 ± 2.87, p > 0.001). In the patient group, a significant correlation was found between the carotid and aortic VWR (R = 0.492, p = 0.011). The coronary artery distensibiliy was significantly correlated with the carotid and aortic VWR in patients (R = -0.524, p = 0.012 and R = -0.624, p = 0.002). Conclusion Neither in patients with DM nor in healthy subjects, we found a significant correlation between the distensibility of different vascular territories and comparing distensibility and VWR in each single vessel territory.
