In der vorliegenden klinischen, prospektiven Studie wurde die passive in vivo Rotationsstabilität des Tibiofemoralgelenkes sowohl von HKB-insuffizienten Kniegelenken im Vergleich zum gesunden kontralateralen Kniegelenk als auch im Verlauf nach kombinierter oder isolierter HKB-Rekonstruktion analysiert. Methodik: 13 Patienten (11m, 2w; ø36,8J; Bereich 18-48J) mit Einzelbündelrekonstruktion des HKB und gesundem kontralateralem Kniegelenk wurden eingeschlossen. Die Analyse der passiven tibiofemoralen Rotationsstabilität erfolgte an einer am Julius Wolff Institut (JWI) entwickelten und evaluierten Vorrichtung unter Darstellung der 3D-in-vivo Rotation der Gelenkpartner bei einem Drehmoment von 3 Nm. Die Patienten wurden präoperativ sowie 6 und 12 Monate postoperativ bei einem Knieflexionswinkel von 90°, 60° und 30° untersucht. Zusätzlich wurde die Stressradiographie zur Analyse der anterioren und posterioren (AP) Stabilität sowie eine subjektive Analyse durch Fragebögen durchgeführt (KOOS, IKDC 2000, Lysholm, SF-36). Ergebnisse und Schlussfolgerung: Erstmalig konnte in einer prospektiven Studie die passive tibiofemorale Rotationsstabilität von Patienten mit Rekonstruktion des HKB direkt, quantitativ und objektiv in vivo dargestellt werden. Patienten mit kombinierter oder isolierter Rekonstruktion des HKB zeigten präoperativ eine signifikant erhöhte Außenrotationsinstabilität des verletzten Kniegelenkes im Vergleich zur gesunden Gegenseite. Nach erfolgter Rekonstruktion des HKB in Kombination mit einer posterolateralen Stabilisierung (PLS) konnte eine Reduktion der Außenrotationsinstabilität beschrieben werden. Nach erfolgter Rekonstruktion des HKB ohne PLS zeigte sich keine Reduktion der Außenrotationsinstabilität. Eine standardisierte, objektivierbare Methode zur Messung der tibiofemoralen Rotationsstabilität zusätzlich zur Messung der hinteren Schublade kann eine sinnvolle Ergänzung der klinischen Diagnostik und Therapieentscheidung sein.","In addition to its primary function as the posterior stabilizer of the tibiofemoral joint, the posterior cruciate ligament (PCL) is also secondary stabilizer for rotational stability. In the present prospective clinical study, the passive in vivo rotational stability of the tibiofemoral joint was analyzed in PCL-deficient knee joints in comparison to the healthy contralateral knee joint and after combined or isolated PCL reconstruction. Methods: 13 patients (11m, 2f; ø36.8J; range 18-48J) with single bundle reconstruction of the PCL and healthy contralateral knee joint were included. The analysis of passive tibiofemoral rotational stability was performed on a device developed and evaluated at the Julius Wolff Institute (JWI), showing the 3D-in-vivo rotation of the joint partners. A torque of 3 Nm was applied to the joint. Patients were examined preoperatively and at a follow up of 6 and 12 months postoperatively at knee flexion angles of 90°, 60° and 30°. Stress radiography was performed to analyze the anterior-posterior stability and a subjective analysis was performed by following scores: KOOS, IKDC 2000, Lysholm, SF-36. Results and conclusion: For the first time the passive tibiofemoral rotational stability of patients undergoing PCL reconstruction could be demonstrated directly, quantitatively and objectively in a prospective in vivo study. Patients with combined or isolated reconstruction of the PCL showed preoperatively a significant higher instability in external rotational of the injured knee joint compared to the healthy contralateral side. A reduction of the external rotational instability could be observed after PCL reconstruction with posterolateral stabilization (PLS). No reduction in external rotational instability was observed after reconstruction of the PCL without PLS.
